Methods of "killing and slaughtering" (referring to aggressive surgical interventions such as excision). Once incised, moxibustion or cauterization should be applied immediately. If recurrence is observed, among the hook-cutting methods in ophthalmology, this approach is generally the most effective.
**Chicken Comb and Clam Flesh Conditions**: These two conditions are similar in morphology and color, share the same meridians, and require identical treatment. They often arise between the spleen wheel (sclera/conjunctiva area associated with Spleen) and subsequently affect the qi wheel (cornea/sclera area associated with Lung), thereby obscuring the eye surface. Treatment requires cutting, followed by cauterization to stabilize the condition. It is suitable to ingest *Sanhuang Wan* (Three-Yellow Pill) with *Mangxiao* (Mirabilite) held in the mouth for dissolution. Externally, apply *Jiangxue Gao* (Crimson Snow Ointment) with *Shexiang* (Musk) removed and *Awei* (Asafoetida) added for eye instillation. If there is a piece of red flesh in the inner canthus resembling a chicken comb or clam flesh, this represents the essence of the Heart meridian's blood aspect. Misguided excision will, at best, damage the eye, and at worst, cause death. Exercise extreme caution.
**Fish Roe and Pomegranate Conditions**: The meridians involved are not different, and the treatment methods are also the same. Their appearance presents as a sheet of fleshy tissue, resembling pomegranate seeds exposed within a chamber, completely covering the divine pearl (pupil/cornea). This indicates blood stasis and excess, representing a malignant stage of ocular disease. Treatment involves excision; if the patient can perceive the "three lights" (sun, moon, lamp) post-operatively, it is treatable. Internally administer *Zaojiao Wan* (Gleditsia Sinensis Pill) and externally apply *Jiangxue Gao*. If the perception of the three lights is dim or blackened, it implies internal damage to the pupil spirit (Tongshen), which is incurable.
**Condition of a Red Bean-Sized Nodule on the Wheel**: There is infusion of red vessels in the Qi wheel (white part of the eye), and a reddish nodule accumulates on the Wind wheel (black part/cornea), caused by internal blood stasis. Guidance and dredging are appropriate; as blood circulation gradually improves, the nodule will gradually dissipate. However, at this stage, nine out of ten cases result in permanent impairment. If there is a solitary, bright red nodule on the white sclera, it is also due to stasis. If there are no vascular connections above or below, medicated powder insufflation and instillation can lead to spontaneous resolution. If vascular connections exist, it implies blood influx from the collaterals; one must trace the direction of blood flow to identify the source and guide/dredge according to severity.
**Condition of a Silver-Star-Like Spot in the Eye**: A single white spot, smooth like a star, covers the center of the cornea/pupil. Although it does not enlarge significantly over time, the pupil remains visible upon lateral gaze. This results from engaging in sexual activity without restraint during episodes of eye pain, and excessive consumption of diuretic and cold-natured drugs targeting the lower jiao. Although the fire pathogen recedes, the Kidney collaterals are injured. This constitutes a lifelong affliction.
**Five-Flower Obstruction Condition**: Arises on the divine pearl (cornea/pupil), appearing mottled and heterogeneous. This occurs because the qi of the meridians of all five zang organs is damaged, knotting into this disease. The colors are variegated and mixed. If there is a central black spot, it indicates injury to the Kidney meridian qi; even with treatment, it cannot be entirely eliminated. This presentation is slightly similar in form to *Banziyi* (Speckled Greasy Membrane) and *Manao Neishang* (Agate Internal Injury). *Banziyi* refers to scar tissue formed after rupture that cannot be removed. *Manao Neishang* refers to small, thin lesions that do not obscure the pupil and are mild. In contrast, this condition is elevated, dense, and prominent, arising outside the stroma/gel layer, allowing for regression; hence, they are distinct. Suitable treatments include *Shenxiao San* (Divine Dissolution Powder) and *Zaojiao Wan*, combined with topical eye medications.
**Hun Jing Obstruction Condition**: Comes in red and white types. The red type fears external entanglement by red vessels; the white type fears becoming smooth like moss. One manifestation involves the white sclera becoming bright red, followed by itching, pain, tearing upon exposure to wind, and difficulty opening the eyelids due to blockage. It may remain asymptomatic for periods but recurs quickly. With prolonged duration, the eye turns greenish-blue, appearing full of congealed fat-like exudate with exposed redness, resembling horizontal red threads. This is caused by accumulated toxic wind and heat. Suitable treatment involves decoctions that tonify the Liver and regulate blood; as blood circulates, wind naturally subsides. External insufflation and instillation will gradually reduce the membrane.
**Black Membrane Like a Pearl Condition**: This is not comparable to *Xiejing* (Crab Pupil) or *Mugan* (Wood Withered). *Xiejing* results from rupture and discharge; this condition arises from excess Liver qi attempting to protrude. Thus, it originates from the periphery of the Wind wheel. Black blisters resemble pearls, varying in number. When fire pathogen is substantial and excess, it causes pain; when deficient and slow, it does not hurt. Treatment uses a fine-bladed needle to horizontally pierce each black membrane individually. Malignant fluid inside flows out, and the lesion flattens. Post-puncture, apply *Luganshi San* (Calamine Powder) with *Nao* (Borneol) and *Shexiang* (Musk) removed for instillation. First, ingest *Lingyangjiao Yinzi* (Antelope Horn Decoction) with *Wuweizi* (Schisandra) removed and *Chishaoyao* (Red Peony Root) added. Subsequently, take *Liujun Wan* (Six-Ingredient Pill), followed by *Bushen Wan* (Kidney-Tonifying Pill). If one is unfamiliar with this method and instead uses cooling agents and cooling topical medicines, efficacy is rarely achieved.
