Tremors during sleep in infants are caused by insufficiency of the Heart and Kidney. Since the Zang-Fu organs are fragile, they are easily affected by fright. The Shen (Spirit) of a person circulates through the eyes when awake, and resides in the Kidney when asleep. Now that both the Heart and Kidney are deficient, they cannot calm and hold the Spirit, thus causing tremors during sleep. Treatment should focus on clearing the Heart and calming the Spirit. There are also cases caused by fright, where physical impact disturbs the Liver, leading to an unsettled Hun (Ethereal Soul). In such cases, Baolong Wan (Dragon-Holding Pill) is suitable to resolve phlegm and settle fright. If food stagnation generates phlegm, causing restlessness and tremors, Sijunzi Tang (Four Gentlemen Decoction) should be used to strengthen the Spleen, added with Shenqu (Medicated Leaven) and Banxia (Pinellia Rhizome) to resolve phlegm, and Zhizi (Gardenia Fruit) and Chaihu (Bupleurum Root) to clear heat.
After an infant suffers from fright, slight eye movements and teeth grinding indicate that body fluids have been depleted after illness, failing to nourish the sinews and vessels. It may also be due to deficiency-heat in the Liver channel generating wind. One must examine whether Qi and Blood are in excess or deficiency to determine treatment. If fever occurs during the day, accompanied by a desire for cold drinks and movement, it indicates Qi excess; Xieqing Wan (Drain the Green Pill) is indicated. If night sweats occur and sleep is restless with movement, it indicates Blood deficiency with heat in the Liver and Kidney; Dihuang Wan (Rehmannia Pill) is indicated. If wind-pathogen from the Liver channel transmits to the Spleen and Kidney, it also causes teeth grinding. First use Chaihu Qinggan San (Bupleurum Liver-Clearing Powder), followed by Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder) and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill). If due to deficiency-heat in the Liver and Stomach, Buzhong Yiqi Tang (Middle-Warmer-Boosting Qi-Supplementing Decoction) added with Shaoyao (Peony Root) and Zhizi (Gardenia Fruit) is used. For excess heat, Xiehuang San (Drain the Yellow Powder) is used, because the gums belong to the Hand and Foot Yangming channels.
The Classics state: "All wind syndromes characterized by tremors and dizziness belong to the Liver." The Liver belongs to Wood; Wood shakes when exposed to wind. This represents deficiency-wind generated by excessive fire in the Liver channel. Hematochezia (blood in stool) occurs because the shaking Wind-Wood overcomes Earth (Spleen), impairing its ability to control blood. It may also result from a wet nurse's anger, causing intense wind-heat, injuring Liver-Wood and damaging Spleen-Earth, preventing the clear Yang from ascending. Alternatively, wind-pathogen may invade the Large Intestine. Treatment methods: If due to wind-heat, use Chaihu Qinggan San (Bupleurum Liver-Clearing Powder); if due to angry fire, use Xiaochaihu Tang (Minor Bupleurum Decoction) added with Baishao (White Peony Root) and Danpi (Moutan Cortex); if clear Yang fails to ascend and Spleen-Qi sinks, use Buzhong Yiqi Tang (Middle-Warmer-Boosting Qi-Supplementing Decoction); if Liver-channel blood-heat causes reckless bleeding, use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill); if Spleen-Earth fails to nurture Liver-Wood, use Liujunzi Tang (Six Gentlemen Decoction) added with Chaihu (Bupleurum Root) and Gouteng钩 (Uncaria Ramulus cum Uncis).
