Qian Zhongyang stated: Pediatric epilepsy occurs because the qi and blood are not yet full, and the spirit (Shen) is not yet solid. It may be caused by invasion of wind pathogens or triggered by fright from strange objects/events. It can also result from the wet nurse's emotional fluctuations and terror. Manifestations include eyes staring straight, eyeball traction, locked jaw, drooling, abdominal distension, convulsions and spasms, opisthotonos (backward arching of the head and neck), and stiffness of the waist and spine, with the body appearing like a corpse. The child may regain consciousness after one or two hours. If the body arches backward and moves upward, with a sound resembling that of a chicken, it belongs to the Liver. Forceful convulsions indicate an excess pathogen; treat with Chaihu Qinggan San (Bupleurum Liver-Clearing Powder). If accompanied by constipation, use Xieqing Wan (Drain Green Pill). For yin deficiency, use Dihuang Wan (Rehmannia Pill).
If the eyes stare straight and the tongue protrudes, with a sound resembling that of a horse, it belongs to the Heart. Fever with craving for cold drinks indicates excess heat; treat with Zhusha Anshen Wan (Cinnabar Calming-the-Spirit Pill). Fever with craving for warm drinks indicates deficient heat; treat with Chen Sha Miaoxiang San (Cinnabar Wonderful Fragrance Powder).
If the eyes stare straight and there is abdominal pain, with a sound resembling that of an ox, it belongs to the Spleen. If accompanied by fever and twitching, lying supine, with a lustrous complexion and a floating pulse, the disease is in the Fu organs and is a Yang syndrome, which is easy to treat; use Jiawei Daotan Tang (Modified Phlegm-Resolving Decoction). If the body is cold without convulsions, lying prone, with a dark complexion and a deep pulse, the disease is in the Zang organs and is a Yin syndrome, which is difficult to treat; use Liujunzi Tang (Six Gentlemen Decoction) added with Muxiang (Aucklandiae Radix) and Chaihu (Bupleuri Radix).
If there is startle jumping, backward arching, and flaccid hands, with a sound resembling that of a sheep, it belongs to the Lung. A sallow complexion indicates that Earth (Spleen) fails to generate Metal (Lung); treat with Wuwei Yigong San (Five-Ingredient Extraordinary Efficacy Powder). A red complexion indicates ministerial fire rushing up to the Lung; treat with Dihuang Wan (Rehmannia Pill).
If the limbs are stiff like a corpse and saliva/foam is vomited from the mouth, with a sound resembling that of a pig, it belongs to the Kidney. Use Dihuang Wan (Rehmannia Pill), substituting honey with Ziheche Gao (Placenta Heminis Extract) to make pills. Since the Kidney has no purgative method, all treatments address deficiency.
For any such condition, first inspect the area between the high bone behind the ear for blue-purple veins. Pricking them to bleed can prevent the onset. These are all syndromes of insufficient primal qi (Yuan Qi). One must commonly use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) added with Lujiaojiao (Deer Antler Glue), or Shengui Bashen Wan (Eight-Ingredient Pill from the Golden Cabinet) using fresh Ziheche Gao (Placenta Heminis Extract) instead of honey, to assist the congenital foundation. Additionally, use Liujunzi Tang (Six Gentlemen Decoction) and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) to assist the acquired foundation. If harsh purgatives or draining methods are indiscriminately used, further damaging the primal qi, the condition will inevitably recur frequently. Over time, it becomes critical and often leads to untreatable death.
In cases of epilepsy, if the pulse at the Tiger's Mouth (Hukou point area) is chaotic, it indicates disharmony of qi. If the nail bed veins show five colors, it signifies severe illness where color changes occur. If the color turns purely black, it is incurable.
