Qinshi Qice (Slanting Pupil). This refers to the slanting of the pupil. Its shape may resemble an almond or a peach pit, appearing triangular or crescent-shaped. This is caused by the scorching of the Liver and Kidney Yin fluids, where Kidney Water is exhausted and Deficient Fire flares up, thereby consuming and damaging the Qinshi (pupil). Treatment should employ Liuwei Wan (Six-Ingredient Rehmannia Pill) supplemented with Jili (Tribulus Terrestris), Danggui (Angelica Sinensis), and herbs that clear fire. If the black cornea (Jinghuang) is damaged, and the Shengao (vitreous humor/ocular contents) leaks out causing the pupil to slant, the Qinshi is on the verge of total loss. Urgent treatment to tonify the Kidney is required. If there is Xiejing (iris prolapse through a corneal perforation) outside the cornea, even if the Xiejing flattens, the pupil cannot return to a round shape; if accompanied by Zhiyi (corneal opacity/scarring), it will never detach for life.
The Classics state: "When the Liver is deficient, the eyes become blurred and unable to see." It also states: "When the Foot-Shaoyin Kidney Meridian is disturbed, disease manifests as a desire to rise from sitting, and the eyes are blurred and unable to see." It further states: "What the Shaoyin meridian describes as 'blurred vision upon rising' is due to the internal depletion of Yin essence, hence the eyes are blurred and unable to see." This eye disease is caused by eye pathology combined with sexual indiscretion (Fanglao). Generally, symptoms are particularly severe after sexual activity. Yeguang Jiaohong Wan can be used. Ni Zhongxian stated: The Classics say, "The Foot-Jueyin Liver meridian governs the eyes, and its emotion is Anger. Severe anger injures the Liver, which then injures the Spleen and Stomach, causing Qi to fail to gather; injuring the Liver causes Shenshui (aqueous humor) to scatter. Shenshui is also formed by the gathering of Qi." The symptoms include no discharge or tears, pain, itching, photophobia, and a tight, astringent sensation in the eyes. Initially, vision is merely blurred as if walking in clouds or mist; gradually, one perceives black spots floating in the air; later, objects appear double; over time, light fails to converge, resulting in permanent disability. This is because Shenshui gradually scatters, scattering again and again, until it is completely exhausted. This condition should not be treated as Phlegm; rather, one must nourish the Yin and Blood of the Liver and Kidneys. However, this requires accumulation over years, avoiding irregular eating, excessive labor, and uncontrolled Seven Emotions (Joy, Anger, Worry, Thought, Sadness, Fear, Fright) and Five Thieves (pathogenic factors), so that efficacy may be achieved. If light fails to converge after a long illness, it is incurable. One syndrome occurs due to sudden violent anger, causing Shenshui to scatter immediately and light to cease converging; this is incurable forever. Another syndrome results from being struck by an object, causing Shenshui to scatter, similar to the violent anger case, and is also incurable. This is commonly known as Qingmang (blindness/severe visual impairment). Patients initially disregard it; once established, physicians often fail to recognize it, attributing it directly to Heat and using cooling/astringent medicines. They do not realize that cooling properties correspond to Autumn and Metal, which further injure Liver Wood, often leading to disability before stopping.
Jinghuang Shimiao Syndrome (Yellow Eye with Blurred Vision). The black cornea appears yellow and bright like gold, and vision is slightly blurred. This is due to heavy Damp-Heat and turbid Qi steaming upward, while Clear Yang Qi ascends into the cornea, changing its color. Individuals who enjoy alcohol, crave rich/greasy foods, and have constitutions prone to Damp-Heat, dryness, and stickiness often suffer from this disease. Treating the Damp-Phlegm leads to recovery. Wuling San (Five-Ingredient Poria Decoction) added with Yinchen (Artemisia Capillaris) and Danshen (Gentiana Scabra) is indicated. In severe cases, formulas such as Zhizi Bopi Tang (Gardenia and Phellodendron Bark Decoction) may be used.
