Second visit: The feverish condition fluctuates without reduction; chest tightness and nausea are present. Whenever the fever rises, dizziness and vertigo are felt, and sweating is not smooth. The pulse is stagnant, rapid, and not vigorous; the tongue coating is pale yellow with dry hairs in the center. Formless summerheat and tangible dampness steam each other without transforming, causing the chest and heart region to become turbid and muggy. Furthermore, due to the menstrual flow being scanty and sluggish, blood is stagnated by heat; there is deep concern that pathogenic heat may penetrate inward leading to syncope.

Third visit: Today, the onset of fever is significantly reduced, nausea has also stabilized, and dizziness, vertigo, irritability, and thirst are markedly different from yesterday. This demonstrates that latent qi steams together with dampness, making the chest and heart region like clouds and mist. However, the pulse remains muddy and rapid. The pathogenic factor's momentum is still strong; fluctuations and relapses are still feared.

Fourth visit: Although the feverish condition has not risen significantly, it lingers and has not receded. Chest tightness and nausea persist, pain occurs above the umbilicus. The menstruation has cleared, but compared to normal cycles, it is still scanty and sluggish. The left guan pulse is wiry and large. This is because summerheat and dampness steam each other in the qi level, causing the qi of the liver and stomach to lose harmony. Further measures to regulate qi should be considered.

Patient Zhang: The feverish condition fluctuates; upon rising, there is irritability and restlessness, confused speech, chest tightness, frequent thirst with desire to drink, and profuse sweating that does not resolve the condition. The tongue is red with a white coating, and the pulse is soft (ru). This is summerheat emanating from within. Summerheat is the hot qi of heaven; once it scorches, it immediately disorders the mind spirit. Since the menstrual flow has just arrived, there is deep concern that heat may enter the blood chamber (xue shi), leading to syncope.
