[Appendix: Regurgitation]
Patient Song (Left side record) After vomiting blood, food intake is obstructed with a choking sensation. This is due to blood stasis stagnating at the stomach opening. It is feared that it may develop into dysphagia (Yige).
Prescription: Yanhusuo (Corydalis Rhizome, 1.5 qian, stir-fried with wine), Wulingzhi (Trogopterus Dung, 3 qian), Zhi Xiangfu (Prepared Cyperi Rhizoma, 2 qian, ground), Dan Taoren (Single Peach Kernel, 3 qian), Chao Zhike (Stir-fried Aurantii Fructus, 0.8 fen), Walengzi (Arca Shell, 5 qian), Chao Suzi (Stir-fried Perillae Fructus, 3 qian, ground), Chao Zhuru (Stir-fried Bambusae Caulis in Succo, 1.5 fen), Jiangxiang (Daemonorops Draco, 1.5 qian, split), Shang Xiangjun (i.e., Dahuang/Rhei Radix et Rhizoma, 1.5 qian, soaked in good wine, stir-fried until dry before adding).
Left Case Food intake is obstructed with a choking sensation, phlegm-saliva surges upward. Stomach Yang fails to transport. This is a severe case of dysphagia (Yige), difficult to treat.
Prescription: Xiebaitou (Allii Macrostemonis Bulbus, 3 qian), Chuan Yalian (i.e., Huanglian/Coptidis Rhizoma, 4 fen), Zhi Banxia (Prepared Pinelliae Rhizoma, 1.5 qian), Jupi (Citri Reticulatae Pericarpium, 1 qian), Baitanxiang (Santalum Album Lignum, 3 qian), Dan Ganjiang (Dried Ginger, 6 fen), Guang Yujin (Curcumae Radix, 1.5 qian), Zhuru (Bambusaes Caulis in Succo, 1 qian), Shang Chenxiang (Aquilariae Lignum Resinatum, 3 fen), Gong Dingxiang (Eugeniae Caryophyllatus Flos, 3 fen; the latter two are ground into powder and taken first mixed with water).
Patient Shen (Left side record) Pain in the Zhongwan area, food intake is obstructed with a choking sensation. Last winter, hard black stools were passed. This is mainly due to blood stasis stagnating at the stomach opening, likely forming dysphagia (Yige).
Prescription: Yanhusuo (Corydalis Rhizome, 1.5 qian, wine-fried), Xiebaitou (Allii Macrostemonis Bulbus, 3 qian), Wuyao (Linderae Radix, 1.5 qian), Jing Sanleng (Sparganii Rhizoma, 1 qian), Walengzi (Arca Shell, 5 qian, crushed), Dan Taoren (Single Peach Kernel, 3 qian, crushed), Pengzhu (i.e., Ezhu/Zedoariae Rhizoma, 1 qian), Heibai Chou (i.e., Qian Niu Zi/Pharbitidis Semen, each 7 fen), Xuanfuhua (Inulae Flos, 2 qian, wrapped), Wulingzhi (Trogopterus Dung, 3 qian).
Left Case Chronic epigastric fullness, food intake is obstructed with a choking sensation. Surging saliva and Qi-stagnation block each other. This is a severe case of dysphagia (Yige).
Prescription: Yanhusuo (Corydalis Rhizome, 1.5 qian, wine-fried), Walengzi (Arca Shell, 1 liang), Zhi Xiangfu (Prepared Cyperi Rhizoma, 2 liang, ground), Xiebaitou (Allii Macrostemonis Bulbus, 3 qian), Xuanfuhua (Inulae Flos, 2 qian, wrapped), Zhi Banxia (Prepared Pinelliae Rhizoma, 3 qian), Wulingzhi (Trogopterus Dung, 3 qian, wine-fried), Yizhiren (Alpiniae Oxyphyllae Fructus, 1 qian), Wuyao (Linderae Radix, 1.5 qian), Shengjiangzhi (Fresh Ginger Juice, 1 spoon, taken by mixing).
Fangbo Hu Yuntai Over sixty years old, Yin fluids are already deficient. Added to this is disharmonious Liver Qi invading Spleen/Stomach Earth, causing the Yang Qi in the Stomach to fail to rotate and circulate. Food intake is obstructed with a choking sensation, and severely, saliva and foam surge upward. Both Guan pulses are wiry. The root cause of dysphagia (Yige) cannot be discussed alongside ordinary diseases. Temporarily rotate Stomach Yang, slightly incorporating wind-dispersing methods to clear new external pathogens.
Prescription: Zhuli Banxia (Pinelliae Rhizoma processed with Bamboo Sap, 1.5 qian), Chao Zhuru (Stir-fried Bambusaes Caulis in Succo, 1 qian), Chuan Yalian (i.e., Huanglian/Coptidis Rhizoma, 5 fen), Dan Huangqin (Scutellariae Radix, 1.5 qian), Dan Ganjiang (Dried Ginger, 3 fen), Bai Fuling (Poria, 3 qian), Sangye (Mori Folium, 1 qian), Chi Juhua (Chrysanthemi Flos, 1.5 qian), Bai Jili (Tribuli Fructus, 1.5 qian), Baitanxiang (Santalum Album Lignum, 1 qian, split).
Second Visit Using acrid-opening and bitter-descending methods, the obstruction has slightly relieved. However, the left arm aches, and upon waking, there is a sensation of dryness and thirst. The pulse is fine with a wiry Guan position; the tongue is red with yellow coating and peeled center. The human body's Spleen is Yin Earth, and Stomach is Yang Earth. Yin Earth prefers dryness, while Yang Earth prefers moisture. For example, when a healthy person experiences slight dietary obstruction, drinking soup moistens it, and the obstruction stops immediately. This is clear evidence that the Stomach favors softness and moisture. Now, at an advanced age, the Five Fluids are all deficient, compounded by internal burning of Liver Fire, leading to Stomach Yin deficiency and inability to soften and moisten. Thus, the stomach opening is dry and astringent, preventing food entry. However, if the Stomach is dry and astringent, where does the Phlegm come from? It is unknown that stagnant Jin-Ye (Body Fluids) all transform into Phlegm; the more Phlegm, the more Jin-Ye is consumed. Next, plan to soothe the Liver Wood and drain Qi-Fire; draining Qi-Fire is precisely how to preserve Jin-Ye. Whether this is correct, please correct me.
Prescription: Xiang Douchi (Fermented Soybean, Guang Xingren (Apricot Seed), Yujin (Curcumae Radix), Chao Loupi (Stir-fried Trichosanthis Pericarpium), Jiegeng (Platycodonis Radix), Zhuru (Bambusaes Caulis in Succo), Chuan Yalian (i.e., Huanglian/Coptidis Rhizoma, received into decoction using juice from Ganjiang/Dried Ginger 6 fen), Pipaye (Loquat Leaf), Hei Shanzhi (Gardeniae Fructus), Baitanxiang (Santalum Album Lignum).
Third Visit Promoting Qi transformation and circulating Jin-Ye, for several days the condition felt very peaceful, and obstruction receded. Unfortunately, Jin-Ye secretly withered away; the power of herbs cannot last long, so obstruction receded but then worsened. The Five Zang organs govern the Five Emotions: in Lung it is Grief, in Spleen it is Worry. Now, inexplicable grief and worry intersect, which is also a sign of Zang Dryness. Again promote Qi transformation, simultaneously advancing moistening and nourishing agents.
Prescription: Guang Xingren (Apricot Seed, 3 qian), Guang Yujin (Curcumae Radix, 1.5 qian), Hei Shanzhi (Gardeniae Fructus, 3 qian), Zhuli (Bamboo Sap, 7 qian, taken by mixing), Jiangzhi (Ginger Juice, a little, taken by mixing), Chao Loupi (Stir-fried Trichosanthis Pericarpium, 3 qian), Bai Fuling (Poria, 3 qian), Zhike (Aurantii Fructus, 5 fen), Chao Suzi (Stir-fried Perillae Fructus, 3 qian), Da Tiandong (Asparagi Radix, 3 qian), Chi Juhua (Chrysanthemi Flos, 1 qian), Baitanxiang (Santalum Album Lignum, 8 fen), Pipaye (Loquat Leaf, de-haired, four leaves).
Fourth Visit Promoting Qi transformation was originally intended to drain Qi-Heat and preserve Jin-Ye. For several days, the peeled area on the tongue center has become slightly paler. However, the left arm still occasionally aches, and obstruction fluctuates in severity. This is nothing but Jin-Ye failing to assist, and Stomach Earth failing to moisten. Coughing with much phlegm also belongs to the steaming and refining of Jin-Ye. The Lung collaterals are scorched, thus Zang Dryness generates grief. Again resolve Phlegm and drain Heat to treat the branch, moisten and nourish Jin-Ye to treat the root.
Prescription: Bai Jili (Tribuli Fructus, 3 qian), Hei Shanzhi (Gardeniae Fructus, 3 qian), Guang Xingren (Apricot Seed, 3 qian), Huai Xiaomai (Tritici Fructus, 6 qian), Chi Juhua (Chrysanthemi Flos, 1.5 qian), Guang Yujin (Curcumae Radix, 1.5 qian), Chao Loupi (Stir-fried Trichosanthis Pericarpium, 3 qian), Sheng Gancao (Glycyrrhizae Radix et Rhizoma, 3 fen), Da Nanzao (Jujubae Fructus, four pieces, split and pitted), Yanshui Chao Zhuru (Salt-water stir-fried Bambusaes Caulis in Succo, 1 qian).
Follow-up Prescription: Xian Shengdi (Rehmanniae Radix Recens, 5 qian), Tianhuafen (Trichosanthis Radix, 1.5 qian), Da Maidong (Ophiopogonis Radix, 3 qian), Tian Xingren (Sweet Apricot Seed, 3 qian), Sheng Huaiyao (i.e., Dioscoreae Rhizoma, 3 qian), Bai Jili (Tribuli Fructus, 3 qian), Jiao Shimii (i.e., Sorghi Fructus, 2 qian), Qingguo (Canarii Fructus, three pieces, crushed), Lizhi (Pyrus Sinkiangensis Juice, 1 liang, taken warm by mixing).
Patient Jiang Long-term alcohol consumption damaged Middle Yang, Qi fails to function and transform. Food intake is obstructed with a choking sensation, phlegm-saliva surges upward. Pulse is sluggish, coating white and substance greasy. This is a severe case of dysphagia (Yige), treatment is arduous.
Prescription: Daizhe Shi (Hematitum, 4 qian), Bai Fuling (Poria, 3 qian), Guang Yujin (Curcumae Radix, 1.5 qian), Zhuru (Bambusaes Caulis in Succo, salt-water stir-fried, 1 qian), Xuanfuhua (Inulae Flos, 1 qian), Chao Suzi (Stir-fried Perillae Fructus, 3 qian), Bai Jiegeng (Platycodonis Radix, 8 fen), Zhishi (Aurantii Fructus Immaturus, 8 fen), Zuojin Wan (Left Metal Pill, 7 fen, added to decoction), Zhuli (Bamboo Sap, 8 qian, mixed with 3 drops of Jiangzhi/Ginger Juice).
Patient Guo (Left side record) Long-standing intestinal bleeding (Changhong) and hemorrhoidal prolapse, Ying (Nutritive) fluids greatly depleted. When food enters the Stomach, it chokes and causes pain. Both Guan pulses are wiry and slippery. This is Stomach Yin withering and drying up. Severe dysphagia (Yige), how can one easily speak of treatment.
Prescription: Jin Shihu (Dendrobii Caulis), Beishashen (Glehniae Radix), Hang Baishao (Paeoniae Radix Alba), Sheng Gancao (Glycyrrhizae Radix et Rhizoma), Jiao Shimii (i.e., Sorghi Fructus, stir-fried), Bai Jili (Tribuli Fructus), Banxia Qu (Pinelliae Fermentum), Huoshui Lugeng (Fresh Reeds Rhizome).
Master's Note: Take a small pot separately to cook rice. When the rice is nearly cooked and water absorbed, place slices of green-skinned sugarcane on top of the rice. Once cooked, remove the sugarcane and eat the rice. (Recorded by a Qing Dynasty scholar).
Second Visit Pulse is slippery and wiry. Pain in the tongue center. When food enters the Stomach, it still feels choking and painful. Stomach Yin is withering and drying up, cannot be viewed lightly. Again plan the method of Banxia Tang from Jin Gui Yao Lue.
Prescription: Taishenxu (Ginseng Radix Filament, separately decocted, 7 fen taken by mixing), Zhi Banxia (Prepared Pinelliae Rhizoma, 3 qian), Bai Mi (White Honey, 2 qian, decocted together, taken mixed with Ginseng decoction).
Take this prescription for seven doses. After decoction, stew in boiling water and swallow slowly. Finish the decoction, then decoct twice more. For the second decoction, use 1.5 qian of honey.
Third Visit Pulse is larger on the left than the right, a sign of Yin injury not recovering. Food intake is obstructed with a choking sensation, Stomach Yin is especially withering and drying up, cannot be viewed lightly. The previously planned Banxia Tang from Jin Gui Yao Lue should be appropriate. Expand upon its intent.
Prescription: Taishenxu (Ginseng Radix Filament), Zhi Banxia (Prepared Pinelliae Rhizoma, decocted with White Honey, taken mixed with Ginseng decoction), Zuojin Wan (Left Metal Pill, 4 fen, taken with decoction).
Fourth Visit Choking and pain upon eating have gradually stabilized, the wiry pulse has slightly flattened, but intestinal bleeding (Changhong) continues for days. Liver Fire burns internally, Stomach Yin is withering and drying up. The Liver and Gallbladder house Ministerial Fire internally; the Kidney opens into the two yin orifices. As copper rings eastward when sounding westward, bells in Lu ring when struck in Xi (metaphor for mutual resonance).
Prescription: Taishenxu (Ginseng Radix Filament, 1 qian), Zhi Banxia (Prepared Pinelliae Rhizoma, 2 qian), Bai Mi (White Honey, 3 qian, same method), Xi Shengdi (Rehmanniae Radix, 4 qian), Guijiaxin (Testudinis Carapax et Plastrum Heart, 5 qian), Diyu Tan (Sanguisorbae Radix Charred, 3 qian), Chao Huaihua (Sophorae Flos Stir-fried, 3 qian), Zexie (Alismatis Rhizoma, 1.5 qian), Danpi Tan (Moutan Cortex Charred, 2 qian), Zuojin Wan (Left Metal Pill, 4 fen).
Patient Sun (Right side record) Discomfort in Zhongwan, pressing feels hard and distended, severely Qi rebels like panting. Both Guan pulses are wiry and slippery. This is depression stirring Liver Qi, Liver Qi rushing into the Stomach, about to form a severe case of dysphagia (Yige). Only an open mind can achieve efficacy.
Prescription: Ding Daizhe Shi (Nailed Hematitum), Chao Suzi (Stir-fried Perillae Fructus), Zhi Xiangfu (Prepared Cyperi Rhizoma), Dan Wuzhuyu (Evodiae Fructus), Xuanfuhua (Inulae Flos), Xiebaitou (Allii Macrostemonis Bulbus), Chao Zhike (Stir-fried Aurantii Fructus), Sharen (Amomi Fructus), Chenxiang (Aquilariae Lignum Resinatum, 3 fen, ground and taken by mixing), Binglang (Arecae Semen, 2 fen, ground and taken by mixing).
Patient Yin (Left side record) After eating, Qi rushes upward, followed immediately by vomiting phlegm-water. Inquired that previously purple-black vomitus occurred, containing blood-water, phlegm sometimes green. This comes from the Lower Jiao Liver and Kidney. The lower outlet of the Stomach is blocked by phlegm and stasis. Prevent progression to Ge (Obstruction).
Prescription: Zhi Banxia (Prepared Pinelliae Rhizoma), Chuan Lian (i.e., Huanglian/Coptidis Rhizoma), Dan Taoren (Single Peach Kernel), Tai Wuyao (Linderae Radix), Danggui Xu (Angelicae Sinensis Radix Filament), Tu Chao Chishao (Earth-stir-fried Paeoniae Radix Rubra), Ganjiang (Dried Ginger), Chuan Guizhi (Cinnamomi Ramulus), Jiuchao Yanhusuo (Wine-stir-fried Corydalis Rhizoma).
Second Visit
Prescription: Xiebaitou (Allii Macrostemonis Bulbus), Jupi (Citri Reticulatae Pericarpium), Zhi Banxia (Prepared Pinelliae Rhizoma), Xuanfuhua (Inulae Flos), Fuling (Poria), Yanhusuo (Corydalis Rhizoma), Zhishi (Aurantii Fructus Immaturus), Daizhe Shi (Hematitum), Tai Wuyao (Linderae Radix), Bianque Yuhu Wan (Bianque Jade Pot Pill, 1.2 qian, taken first).
Third Visit Obstructed food cannot go down,有形 (solid mass) in Zhongwan. For several days, vomiting purple-black blood stasis, stool also passes black matter. Previously stated blood stasis blocks the stomach opening, now credible. Unfortunately, although stasis is vomited out, pressing the left side of Zhongwan remains hard. This shows the knotted and stagnant stasis still occupies internally. This is a major syndrome of Blood Ge (Blood Obstruction). The treatment method: if stasis is not removed for one day, Ge will not heal for one day. Now focus on resolving stasis to observe movement and stillness.
Prescription: Shanjiapian (Manis Squama, 1 qian, coated with Ganqi/Dry Lac, roasted until smoke ceases), Wulingzhi (Trogopterus Dung, 3 qian, wine-fried), Walengzi (Arca Shell, 4 qian), Yanhusuo (Corydalis Rhizome, 2 qian), Shanzha Tan (Crataegi Fructus Charred, 3 qian), Tai Wuyao (Linderae Radix, 1.5 qian), Danggui Wei (Angelicae Sinensis Radix Tail, 2 qian), Taoren (Pruni Semen, 2 qian), Tubiechong (Eupolyphaga Steleophaga, five pieces, head and feet removed, roasted).
Another Prescription: Damp-phlegm and blood stasis accumulate at the stomach opening, causing food and drink unable to enter the Stomach. Previous prescriptions promoted blood transformation and stasis resolution, resulting in chest and hypochondriac fullness. This is because stasis obstruction is not unblocked; moving it fails, so they mutually resist, hence feeling fuller. Blood Ge major syndrome is extremely difficult to treat. Plan to use pill medicine to descend.
Prescription: Wulingzhi (Trogopterus Dung, 2 qian, wine-fried), Chuan Yujin (Curcumae Radix, 1.5 qian), Xi Xuepo (i.e., Hupo/Amber, 7 fen, separately ground), Dahuang (Rhei Radix et Rhizoma, 2 qian, wine-fried), Tubiechong (Eupolyphaga Steleophaga, sixteen pieces, head and feet removed, roasted), Dan Taoren (Single Peach Kernel, 1.5 qian), Sheng Puhuang (Typhae Pollen Raw, 1 qian), Yanhusuo (Corydalis Rhizome, 2 qian), Shanjiapian (Manis Squama, 1 qian).
Grind the above medicines into fine powder. Use Jiuzhi (Chinese Leek Juice) as paste to make pills the size of mung beans. Take 3 qian per dose.
Right Case Morning food, evening vomit, food undigested. Pulse deep and fine, coating white and substance greasy. Middle Yang fails to rotate, severe case of Fanwei (Regurgitation).
Prescription: Zhi Banxia (Prepared Pinelliae Rhizoma), Dan Wuzhuyu (Evodiae Fructus), Gong Dingxiang (Eugeniae Caryophyllatus Flos), Jupi (Citri Reticulatae Pericarpium), Zhuru (Bambusaes Caulis in Succo, stir-fried with Ginger Juice), Yun Fuling (Poria), Paohēi Jiang (Charred Black Ginger), Guang Huoxiang (Agastaches Herba), Fulonggan (Dragon's House Earth, 7 qian, decocted to replace water).
