Jin (Right) Depression injures the Liver, causing Liver Qi to become excessive and Spleen/Stomach to weaken, leading to the Stomach losing its descending function. After eating, there is distension and fullness, a sensation of nausea and desire to vomit, a palpable mass on the right side of the abdomen, amenorrhea, and alternating chills and fever. The pulse is fine, wiry, and rapid. The Gallbladder is the exterior mansion of the Liver; when Wood Qi is excessively vigorous, the pivot mechanism of Shaoyang does not turn smoothly. It is appropriate to calm the Liver, regulate Qi, and assist in dispersing depression.

Second visit: The Liver and Stomach have been harmonized twice, assisted by opening depression; bowel movements are slightly smoother. However, Qi stagnation persists in the epigastrium, and the Stomach has lost its descending function. Distension and fullness occur after eating. The opening and closing mechanisms are imbalanced, causing alternating chills and fever. Harmonize the Liver and Stomach again to soothe Wood depression.

Jin (Left) Initially, Wood depression within Earth caused a palpable mass and distension in the epigastrium. The Spleen and Stomach are connected by membranes; when Stomach Earth is oppressed by Liver Wood, Spleen Earth also becomes deficient, gradually leading to abdominal distension and enlargement, limb edema, and a yellowish floating complexion. This condition has persisted for several months. Now, at the start of the Li Dong (Start of Winter) solar term, sudden diarrhea occurs with difficult passage, mixed with pus and blood. Upper symptoms include nausea and vomiting, with black material in the vomitus. The limbs are cold. The pulse is deep and hidden. Liver excess and Spleen deficiency have reached an extreme. The Liver is the sea of storing blood; when Qi of the Liver meridian moves wildly and disrupts, the blood of the Liver meridian cannot return to storage, posing a risk of syncope. In the Jueyin chapter of *Jin Gui Yao Lue*, bitter, sour, and pungent herbs are often combined; this method is followed here. Whether it will be effective is uncertain.
Wu Mei (Mume Fructus, 5 fen) Chuan Ya Lian (Coptidis Rhizoma, 5 fen, stir-fried with 7 Dantian Zhu Yu/Drynariae Rhizoma? Re-visit: Previously using the bitter-sour-pungent method from *Jin Gui*, pus and blood have receded, bowel movements have decreased significantly, sleep is peaceful, stomach Qi has slightly recovered, and distension has eased somewhat. However, Qi still sinks downward, vomiting of phlegm remains black, eyes are sensitive to light, urine is red and scalding, tongue is dry, mouth is parched, hands are slightly warm, but feet remain cold. The pulse rises slightly but is fine, wiry, and powerless. Yin deficiency with Wood hyperactivity causes Qi and Fire to escape outward. The Classics state, "Excess heat causes swelling." Although there is a turning point, it is not yet reliable. Plan to nourish and soften the Liver to calm Qi and Fire. When Qi moves and Fire calms, the principles for treating swelling and distension are contained therein.

Third visit: Limbs have turned warm, facial edema has greatly subsided, distension has decreased, upward-rushing Qi has calmed, and urination has gradually smoothed. However, diarrhea continues. Stopped medication for one day yesterday; today pus and blood are mixed again. Pulse is fine and wiry. Liver excess and Spleen deficiency mean Ying (Nutrient) blood fails to contain and secure. Damp-Heat takes advantage of the weakness to invade, further injuring Ying blood. Re-nourish blood and harmonize Ying, while clearing Damp-Heat.

Fourth visit: Modified prescription combined with Bai Tou Weng Tang (Pulsatilla Decoction); pus and blood have greatly reduced, bowel movements are less frequent, distension has loosened, vomiting has retreated, phlegm color has turned white, and slight food intake is possible. However, diarrhea continues, and swelling of both feet is particularly severe, with occasional nausea. Pulse is fine and wiry. Liver excess and Spleen deficiency, Damp-Heat injuring Ying blood. Although there have been repeated turning points, they ultimately do not help the overall situation. Must warn in advance. Again, drain Damp-Heat from Spleen and Stomach, assisted by differentiating and separating turbidity.

Left: Long-term emotional depression causes Liver Wood to lose its free flow. The Chong Mai (Penetrating Vessel) belongs to the Liver system; it originates at Qi Chong (ST30), ascends alongside the navel, and disperses in the chest, leading to Qi rushing, epigastric stuffiness, and throat obstruction. Bi (Left): Depression injures the Liver, Liver Qi moves wildly, Wood attacks Earth, upper vomiting and lower diarrhea, resembling Sha Qi (plague-like qi). Such severe cold, where would Sha turbidity come from? It is clearly Wood and Earth being enemies. For over a month, there is a mass in the abdomen, occasionally attacking and pounding, which is the Qi of Liver depression and anger conflicting. At this time, soothing depression is crucial, but treatment was neglected, leading to Qi stagnation and abdominal fullness. Spleen Earth cannot rotate, turbid Phlegm becomes difficult to transform, spreading through the Shen (Spirit) mechanism, causing dull expression. Pulse is deep and depressed, wiry upon heavy pressure, and powerless at the Chi position. Deep in dangerous territory, cannot speak of cure. Tentatively plan to transform Phlegm to open the Shen mechanism. Wood is excessive, Zheng (Upright) Qi is deficient, no time to ask about other matters.

Zhang (Right): Gallbladder is Jia Wood, Liver is Yi Wood, Stomach is Wu Earth, Spleen is Ji Earth. Among the Five Elements, Wood conquers Earth. Internally, the Gallbladder is attached within the Liver lobes. Startling disturbs Gallbladder Wood, and due to advanced age and deficient Zheng Qi, it cannot be controlled, causing Liver Qi to move with it. Pressing down is Qi; Qi conquers Ji Earth, causing distension, fullness, and disharmony, even loose stools and desire to defecate. Floating up is Yang; Yang invades Wu Earth, causing vomiting of phlegm and saliva, even Qi reversing to the vertex, causing acidity and distension. Pulse is wiry and slippery, weak upon pressure, coating white, substance greasy. These are all diseases caused by Jue Yang invading Spleen and Stomach, stirring up turbidity in the Stomach. If it lingers, fear entering a state of decline. Treatment method: Tonification may cause stagnation and Qi blockage; calming Liver may be too remote from reality. Only by first descending Stomach Qi and harmonizing Middle Qi, stopping vomiting and allowing food intake, can we discuss further. Correct this.

Classics say: The Chong Mai originates at Qi Chong, runs alongside the Shaoyin Meridian, ascends beside the navel, and disperses in the chest. Dragon and Minister Fire do not submerge, Chong Mai is disharmonious, indeed there is reason for this.

Second visit: Harmonize Middle Qi, drain Shaoyang, pulse is stable. Coating thin white, base substance red. More phlegm, epigastric discomfort, drowsiness, severe cases vomiting, phlegm feels sticky. Stomach is the Sea of Water and Grain; receiving grain Qi, it transforms Jin and Qi to harmonize the Five Zang and distribute to the Six Fu. Xi He suffering pain means Wood depression generates Fire, Wood-Fire disturbs Middle, causing epigastric stuffiness. Water and Grain Qi, refined by Fire, cannot transform into Jin and Qi, but instead condense into turbid Phlegm. Yangming loses its normal descending function, Taiyin loses its clear and肃 (purifying) command, hence choking cough, more phlegm, dry and hairy throat. Among the Six Meridians of Cold Damage, only Shaoyin has the symptom of desire to sleep. Since it is not Kidney Yang deficiency with turbid Yin filling the chest, it must be Kidney Yin deficiency with True Yin unable to ascend to the Heart. Hence, one is treated with Si Ni (Four Inversion), the other with Fu Mai (Restoring Pulse). Proceed step by step.
