(Prior context) The manifestation of Qi and Fire ascending and floating cannot be controlled without nourishing with Water. However, substances that tonify Water tend to be cloying and obstructive; if Phlegm-Heat blocks the pathways, they cannot descend effectively. The *Classic* states: "When there is deficiency, tonify its mother." Lung Metal is the mother of Kidney Water. Therefore, it is proposed to nourish the upper source of Water (i.e., moisten the Lungs to generate Kidney Water), while simultaneously incorporating herbs to clear and drain Qi-Fire and resolve Phlegm-Heat.

**Sixth Consultation:** By nourishing the upper source of Water and incorporating methods to resolve Phlegm, gastric appetite has gradually recovered, and various ailments have stabilized. However, occasional choking coughs persist; when triggered by coughing, Qi rushes upward, causing slight throat pain. The pulse is fine and wiry. This indicates Liver Qi-Fire ascending and floating, leading to failure of Upper Lung Qi to descend and Lower Kidney Yin to secure. It is proposed to tonify Kidney Water to nourish Liver Wood, allowing Yin Qi to settle downward, with a slight addition of phlegm-resolving herbs to avoid stagnation.

Pulse fine-wiry, choppy at Chi. Nourish Kidney fluid to extinguish Wind-Wood.

**Third Consultation:** *Suwen: Shengqi Tongtian Lun* states: "Yang Qi, when refined, nourishes the Spirit; when soft, nourishes the sinews." It also says: "Yang Qi, when subjected to overwork, expands; essence exhausts; accumulating through summer leads to Jian-Jue (Frying Jue)." The *Neijing* emphasizes internal burning of Yang-Fire, frying Qi and Blood, Yin failing to embrace, Yang-Fire becoming turbid and hot, leading to near separation of Yin and Yang, resulting in Jian-Jue. This is the general meaning. Current diagnosis: pulse fine-wiry, left Chi small-choppy, right Chi fails to store. Disease started years ago with frequent Jue attacks, followed by delayed menstruation, even only twice a year. This shows Yang sucking essence and consuming Yin fluids. Now shoulder/back/waist/thighs hurt, fire rises, palpitations/tinnitus/dizziness. Overwork excites Yang, strong Fire consumes Qi, causing Yangming collaterals to empty, Wind-Yang enters collaterals, Ying-Blood fails to nourish sinews, losing the function of "softly nourishing sinews." Heart is Yang, Spirit is Yang within Yang. If Spirit mechanism moves, Spirit is bright; if not, Spirit is dull. Thus Li (Fire) must have emptiness in the center; Yin fluids transport Yang Spirit. Now Wind-Yang is hyperactive, Yin fluids daily deplete. Though Yang Spirit exists, mechanism doesn't move, causing deep sleep/confusion, losing "refined nourishing Spirit." Tongue coating varies yellow/white/greasy, suggesting Dampness complicating Yin deficiency. Actually, stomach has turbidity; turbidity rises with deficient fire, changing tongue coating. Turbidity is false, deficiency is true. Treatment: sweet to benefit Stomach, nourish Kidney, clear heat, extinguish Wind-Wood. Must rest and avoid labor.

Liver ascending transforms Heart Ying; Gallbladder descending transforms Ministerial Fire. Stomach is middle pathway. Gallbladder/Stomach should descend. Now Stomach blocked by Phlegm-Qi, Stomach Qi fails to descend, Gallbladder Wood Qi fails to descend alone. Result: only Liver ascending, no Gallbladder descending. Body feels like sitting in a boat, lifted from void. Eyes red, indicating Gallbladder Qi-Fire carrying Mingmen Fire floating upward. Pulse wiry-slippery. Root of stroke. **Patient Yu (Left):** Since childhood, Wind-Phlegm entered collaterals. Spasms cause vomiting of Phlegm-saliva, then recovery. Now as Yang regenerates, Liver-Yang surges, Liver-Qi invades horizontally. After spasm, Qi pushes epigastrium causing pain/vomiting. Wind-Wood offends Stomach-Earth, Stomach fails to descend, Liver Qi transforms into Fire. Fever, head distension, insomnia all night, thirst, constipation. Pulse fine-wiry-rapid, tongue red with white rough coating, gray center. Wood offends Stomach, damaging Stomach Yin. Fear of recurrence of Jue.
