From Hu Yuhai's *Shanghan Yishu* (A Book on Cold Damage), the main purport is to discuss that damp-warmth disease belongs to the latter half of the course of cold damage.
Headache, body heat, aversion to cold, and floating-slippery pulse indicate concurrent involvement of the Yangming and Taiyang meridians. Body heat, dry mouth, and wiry-slippery pulse indicate concurrent involvement of the Yangming and Shaoyang meridians. Body heat, white tongue coating, and surging-slippery pulse indicate Zhengyang Yangming disease.
Slightly yellow tongue coating indicates Zhengyang Yangming disease. White coating at the front and yellow at the back indicates Zhengyang Yangming disease. (Note: This represents cold above and heat below, exterior cold and interior heat.) Yellow coating at the front and white at the back indicates Zhengyang Yangming disease. (Note: This indicates heat transformation in the upper jiao while fluid retention exists in the middle jiao.) White coating surrounding and yellow in the center indicates Zhengyang Yangming disease. White coating with gray tints suggests Yangming disease is about to transmit to the Taiyin meridian. White coating with distinct patterns involves both Yangming and Taiyin. Slightly white and dry-yellow coating involves Yangming and Taiyin. Pinkish-white with slight redness involves Yangming and Shaoyang. Absence of white with slight peach-red hue involves Yangming and Shaoyin.
Redness at the front half and whiteness at the back half involves Shaoyang and Taiyin. Redness at the front half and yellowness at the back half involves Shaoyin and Taiyin. Yellowness at the front half and blackness at the back half involves Yangming and Taiyin. Yellowness at the front half and redness at the back half involves Taiyin and Shaoyin. Yellowness at the front half and purpleness at the back half involves Taiyin and Jueyin. Purely black coating indicates Taiyin disease. Purely yellow coating indicates Taiyin disease. Eight variations of yellow coating involve Yangming and Taiyin. Half yellow and half white involves Yangming and Taiyin. Half black and half yellow involves Yangming and Taiyin. Scorched-black coating indicates Taiyin disease. Moist-black coating indicates Taiyin disease. Mottled yellow-gray-black coating involves Yangming and Taiyin. Purely red, mirror-like coating indicates Taiyin disease. (If the tongue surface is glossy like a lacquered table, bright but not moist, and the Huachi under the tongue is completely dry, the condition is severe. Careful examination is required.) A thick tongue resembling three "nong" characters indicates Shaoyin disease. A wide tongue resembling three "kuo" characters indicates Shaoyin disease. A round tongue indicates Shaoyin disease. A flat tongue without a tip indicates Shaoyin disease. (Note: Old texts state that a tongue edge missing like saw teeth indicates death.) White coating with a tiny bit of red involves Yangming and Shaoyang. White coating with a tiny bit of black involves Yangming and Taiyin. White coating with a tiny bit of yellow indicates Zhengyang Yangming disease. Red tip and red back involves Shaoyin and Shaoyang. Red tip and purple back involves Shaoyin and Shaoyang.
The above thirty-five methods are general principles for differentiation; others can be inferred by analogy.
In Guangdong, Fujian, Zhejiang, and Jiangnan Yangzhou regions, which are coastal areas of fish and salt, people have fragile intestines and stomachs, abundant qi, and hot blood. Thus, wind pathogen causes disease upon invasion. Although there is headache, it is not as severe as an axe blow; although there is neck stiffness, turning is still possible; although there is back tension, movement is still possible. Wind pathogen enters the stomach, causing lung qi stagnation. Therefore, day one presents as wind, day two as heat, and day three as fire, due to intense heat. Heat and wind pathogen struggle, condensing into toxin. This toxin affects the muscle layer governed by the Stomach, rather than the flesh governed by the Spleen. The muscle layer is within the hair pores. Hence, spots appear dot-like like macules or papules, bright red, occurring in tidal waves three times a day, nine times over three days. Thus, the toxin must pass through three days; even without treatment, it will disperse. Pulse: Left Cun floating, Right Guan slippery, Qi Kou large. This is not typical Shanghan. Thus, although the Taiyang meridian is affected, it is not the primary pathology; this is the proper pathology of the Yangming meridian, called Yangming-Taiyang. Tongue coating white, no thirst on day one, no bowel movement on day two. By days three to six, when stool passes, the couli open. Relief comes with sweating.
If on the fourth day of Yangming disease, blood heat forms toxin that cannot vent, toxin stagnates in the middle, couli do not open, trapping evil heat, then it transmits to the Shaoyang meridian. Day one: thirst, Left Guan surging-large, Right Guan surging-slippery, Right Cun Qi Kou blocked. This is lung meridian heat pathogen blocking, airway discomfort. Macules are in the muscle-couli, blood coagulates in the skin. Shaoyin receives heat but is not diseased. Day two: red eyes, red tongue coating, tinnitus, Left Guan surging-large-rapid. This indicates intense heat and victorious evil. Day three: delirious speech, desire not to sleep, Right Guan surging-slippery-excessive. Day four: if macules appear, symptoms ease; if not, manic screaming and unrest. Right Guan slippery-excessive-forceful, Left Guan surging-rapid-slightly wiry, Left Chi empty-large. This indicates evil pathogen entering the interior. Day five: deafness, desire not to sleep, restless sitting/standing, delirious speech nearing madness. This is macule toxin unable to vent, dry mouth consuming fluids. Tongue coating red-yellow, evil has not yet transmitted to the interior; tongue coating red-purple, about to enter the Spleen. Left Guan wiry, Right Guan excessive, Liver Wood angry extreme, heat stagnation severe. Deafness is closure of Kidney Fire. Macules emerging from chest, neck, spine indicate excess Yang evil hiding in these areas; this is Yang toxin about to sink into Yin divisions. Day six: stool passes, evil descends, macules must emerge; if not resolved by day six, fire closes at Youmen, urine short/astringent, toxin steams the Stomach, Lung closed, Large Intestine hot, eyes fixed staring, desire not to see people, pulse rapid, tongue scorched.
Evil transmits to Taiyin, yellow eyes, yellow face, indicating wind dominance and damp stagnation. Day seven: deafness, thirst, yellow eyes, yellow-red cheeks, scorched tongue coating, pulse same as day six. Disease remains in Yang division unremoved; although evil entered interior, it can still be rescued.
Shaoyang unresolved, evil transmits interior, flowing into Taiyin Spleen meridian. Day one: Right Guan surging-large-soft, Left Guan wiry-moving, Left Cun blocked. Heat pathogen lodges in Pericardium, mental confusion, short breath, white of eyes red, Lung meridian depressed. Macule toxin appears red on neck, absent on cheeks/chest, micro-points on hypochondrium, red dots on abdomen, red on anterior arms, tongue coating yellow-black. Although transmitted interior, Yang syndrome remains. Day two: Right Cun wiry, wind pathogen lodges in Lung, white macules about to emerge. Panting indicates death, nasal flaring indicates death, deafness indicates survival, red cheeks indicate survival, closing eyes avoiding people indicates survival, fish-mouth crow-voice indicates death. Day three: Right Guan rapid, Left Cun absent, Right Chi surging-large. Heat pathogen lodges in Kidney, purple lips, scorched-black tongue, fixed staring, avoiding people. Toxin stagnates in Small Intestine, dry feces cannot descend, macules hidden in flesh. Manic screaming/cursing indicates survival, thirst consuming water indicates survival, non-moist urine indicates death. Day four: Left Cun blocked, Left Guan wiry, Left Chi surging-large, Right Guan empty-soft, Right Cun knotted-pulse, Right Chi absent. Blood heat in Middle Jiao, macules appear blue. Day five: Left/Right Cun/Guan pulses absent, both Chi surging-large, hoarse voice wanting to cry, macule toxin stagnant unable to vent. Yellow eyes/body yellow indicates death. Day six: Cun/Guan all absent, both Guan wiry-tight, tongue coating wet-slippery. Fire extreme sensing cold; head cool indicates death. Tongue coating dry-cracked, still treated as fire.
Or, on fifth day of Yangming, macules fail to emerge fully, evil toxin does not enter Shaoyang but directly enters Taiyin. This is not skipping meridians, but injury by diet or medication error. Taiyin days 1-2: hypochondriac pain, dysentery, Left Guan wiry-soft, Right Guan wiry-long, Qi Kou surging, Chi large, severe thirst, dry lips, dry tongue, clear spirit, thick yellow tongue coating, black-gray. Day three: black root, yellow middle, white tip, red eyes green face, Left Guan rapid, Right Guan slippery-large-forceful, Lung pulse large, both Chi blocked. Macules on head/face/chest/back, none on neck/abdomen. Yang qi unable to vent, Yin qi condensed. Taiyin day four: Left three positions blocked, Right Guan soft, Lung pulse large, Chi surging, severe thirst, red eyes red face, green nose, black lips indicate death, sunken macules. Taiyin day five: Cun/Guan all floating, Right Guan knotted, Left Guan tight, chills, headache, red eyes, green nose, green tongue, purple lips indicate death. Macule toxin attacks Liver, not transmitting to Jueyin, but evil strikes Jueyin.
Taiyin pulse favors weakness. Evil enters Spleen, qi abundant blood hot, flows to limbs, distributes to bones, penetrates Heart. Heart Spirit loses exclusive control, thus Ministerial Fire pathogen becomes fierce. Heart Spirit and Ministerial Fire lose position, body has no master. Hence, pain in limbs/bones, abdominal fullness, dry mouth, yellow/black tongue, dry lips. Five Zang and Six Fu including Large/Small Intestine, Bladder, Sanjiao are controlled. Fine-small pulse: Stomach qi uninjured. Slippery-large pulse: Stomach qi damaged. Stomach governs receiving grain, Spleen governs digesting grain; Stomach governs reception, Spleen governs transformation/moistening. Stomach reception depends on Lung, Spleen transformation depends on Liver. Upper part becomes phlegm, lower part becomes dregs. Diaphragm qi excess leads to phlegm stagnation in Danzhong. Heart qi heat leads to dreg stagnation in Small Intestine. Thirst for cold drinks indicates diaphragm heat. Drinking water without urination indicates scorched Lung lobes. Lung qi excess blocks Large Intestine pathway. Evil in Yin division unfavorable to see Yang pulse; disease in Yang division unfavorable to see Yin pulse. Taiyin disease favors fine-small-empty-soft, unfavorable to surging-large-slippery-excessive. Unblock meridians, guide blockages, prevent Wind-Wood from becoming evil, causing nine orifices blockage and death. Taiyin pulse not only seen at Right Guan; Left Cun, Left Guan, Right Chi also visible. Lung resides under Canopy, Lung qi stagnant, favors fine-small, unfavorable to solid-large. Similar to typical Shanghan transmitting to Taiyin: large pulse disease advances, small pulse retreats; forceful advances, weak retreats; slippery-excessive advances, empty-soft retreats; tight-excessive advances, knotted-soft retreats; surging-rapid advances, fine-soft retreats.
External manifestations: red eyes/face/tongue/lips, shaking hands/feet, restlessness, scorched-black coating. Toxin/pathogen fierce, pulse fine-small. Presence of Stomach qi means not untreatable. Green eyes/face/lips, white tongue, minute-fine pulse. Toxin sinks, sweat will occur leading to death. Taiyin disease: red face/eyes, purple lips/black tongue, rapid pulse at both Guan critical, choppy pulse fatal. White face red eyes, green nose/lips, gray coating, tight pulse at Left Chi/Right Guan fatal. Red eyes yellow face, nasal flaring green lips, rapid Right Cun, wiry Guan, absent both Chi fatal. Normal spirit, slightly black coating,革 pulse at both Guan fatal. Yellow tongue green eyes, white face/lips, weak pulse, cold extremities, white macules fatal. Mirror-like smooth tongue, red eyes green face, surging-large both Guan, surging-rapid both Chi, absent both Cun. Toxin sinks Lower Jiao, neck macules not emerging fatal. Thorny tongue, gray-black coating, abdominal/chest distension, severe thirst not desiring drink. Extreme Right Cun, soft Right Guan,涩 Left Guan. Chest binding fatal. Flat tip, hypochondriac pain, scorched-black coating, passing clear water, thirst not desiring drink. Knotted Left Chi, intermittent Left Cun, firm Right Guan. Heat binding Small Intestine fatal.
Yellow-white coating, mottled red-purple, green lips, white face, red eyes, soft Left Guan,涩 Right Guan, absent Lung pulse fatal. Thick white coating, upper gray-black, dry Spleen area, red lips, red eyes, white face, absent both Chi, soft Left Guan, deep-solid Right Guan, absent both Cun. White macules on neck fatal. Red-purple coating, red eyes yellow face, dry lips chest full, dull spirit, cold extremities, absent Right Guan, wiry-tight Left Guan, empty-large Left Chi. Macule toxin sinking fatal. Scorched-round-thick tongue, dry Huachi, scorched lips black teeth, red eyes red face, confused spirit, fine-small pulse, frequent calling, unconsciousness fatal. Consciousness retained may survive. Frequent stool, unconsciousness fatal. Frequent stool, gradually conscious may survive. Tongue does not return to normal gate, shivering fatal. After stool, tongue not moistening fatal untreatable. After stool, spirit suddenly clear, tongue moist but sweating fatal. After stool, wild-large pulse, blood from mouth/nose inevitable. Urgently take Gengyi San to cool Liver, store blood may survive. Delayed, vomiting blood fatal. Disease reaching Taiyin, many death signs; transmitting to Shaoyin, evil excess righteous deficiency, nineteen percent critical, many deaths; transmitting to Jueyin, Wind-Wood becomes evil, nine orifices closing, no need for detailed discussion. (This chapter discusses aspects overlapping with Warm Diseases, Mr. Zhou also stated it is not typical Shanghan disease.)
Any red tongue red face, with Yin pulses at both hands, or pulse swaying rootless, vague unreliable. Purple blisters like soybeans at Liver/Gallbladder areas on tongue. Heat toxin returning to Zang organs, untreatable. Left side severe, right side mild, middle not in Liver/Gallbladder area lighter. Examine pulse, if salvageable, must save. (Old saying: tongue edge missing like saw teeth indicates death, which is this meaning.)
