Patient Sun (26 years old): The damage from overwork has not yet recovered; in his youth, he was thin and had reduced appetite.
Patient Gu (Panmen): Previously, when hungry, he passed foul blood with stool freely; when full, bowel movements were unsmooth. This indicates dysfunction of the Taiyin Spleen in transportation and transformation. Initially, damp-heat resolving methods were used, followed by intestinal astringing and lubricating agents, all without effect. It is now proposed to use acrid-sweet yang-moving methods.
Formula: Lizhong Tang (Regulate the Middle Decoction) minus Ginseng Radix et Rhizoma (Renshen), plus Longan Aril (Guiyuan Rou).
Patient Tan (Xianrentang, 48 years old): Any condition of overwork involves both physical and mental exertion; exertion generates heat, which scorches collateral vessels causing blood to overflow upwards. Muscle wasting and rapid pulse are present. In middle age, the generation of yin essence diminishes daily. Sweet-cold moistening agents may be used (Sweet-cold herbs generate Stomach Yin; bodily Yin fluids are distributed from the Stomach. First nourish Stomach Yin, then gradually distribute it from the Central Palace to the four Zang organs; this is the underlying principle.)
Herbs: Shengdi Huang (Rehmanniae Radix Praeparata), Maidong (Ophiopogonis Radix), Bian Dou (Dolichoris Semen), Beishashen (Glehniae Radix), Zhuganshi (Sacchari Canae Succus), Bai Yuzhu (Polygonati Odorati Rhizoma).
Patient Wang (63 years old): Complexion is dark and emaciated, eyes are yellow, pulse is wiry. He has long suffered from Qi counterflow causing epigastric pain. Now the pain has eased, but Qi counterflows to the throat. This indicates excessive ascending Liver Qi on the left and insufficient descending Lung Qi on the right. Generally, this disease arises from excessive mental effort and calculation.
Herbs: Pipa Ye (Loquat Leaf), Hei Shanzhi (Gardeniae Fructus Carbonized), Chuan Bei Mu (Fritillariae Cirrhosae Bulbus), Su Zi (Perillae Fructus), Jiangxiang Mu (Aquilariae Lignum Resinatum), Xinhui Pi (Citri Reticulatae Pericarpium), Chao Taoren (Persicae Semen Ustums).
Patient Zhao (50 years old): Lower Jiao is cold, testicles are enlarged.
Herbs: Chuan Wutou (Aconiti Radix Lateralis Preparata), Boshang Huixiang (Foeniculi Fructus), Chuanjiao (Zanthoxyli Pericarpium), Huluba (Trigonellae Semen), Chuanlianzi Rou (Meliae Semen), Wu Zhu Yu (Evodiae Fructus), Shu Chuan Fuzi (Aconiti Radix Preparata).
Preparation: Pill form made by coating with black soybean juice.
Patient Chen: Heart deficiency with palpitations; Ministerial Fire and Sovereign Fire tend to ascend.
Herbs: Shengdi Huang (Rehmanniae Radix Praeparata), Tiandong (Asparagi Radix), Fushen (Poria cum Radix), Baizi Ren (Platycladi Semen), Zaoren (Ziziphi Spinosae Semen), Zhi Gancao (Glycyrrhizae Radix et Rhizoma Preparata).
Patient Shen (Xinshi, 34 years old): Postpartum recovery is incomplete; blood loss has injured Yin, causing bone-steaming fever. Generally, excess fire can be treated with cooling methods, while deficiency heat should be treated with warming tonification. Select herbs that are sweet in taste and warm in nature to warmly nourish Qi and Blood, allowing recovery (This is an irrefutable principle.) However, the root of postpartum injury and Ru Lao (postpartum consumption) lies entirely in the Liver and Kidney, extending to the Eight Extraordinary Vessels. It is not suitable for scattered or miscellaneous treatments (The Liver and Kidney and the Extraordinary Vessels belong to one family; however, injury must spread from the Liver and Kidney to the Extraordinary Vessels, representing a deeper level of pathology.)
Herbs: Renshen (Ginseng Radix et Rhizoma), Xian Heche (Placentae Humanae), Gouqi (Lycii Fructus), Zishiying (Hematitum), Fushen (Poria cum Radix), Ziyi Hetao (Juglandis Semen Cum Tecto Rubro), Guishen (Angelicae Sinensis Radix), Dan Rourongrong (Cistanches Herba).
Patient Zhou (Jiaxing, 41 years old): Pain and hardness in the lower abdomen, radiating to the umbilicus; pain occurs inevitably every afternoon. Qi distension penetrates through the waist, sacrum, Huantiao acupoint, and muscle layers; skin gradually becomes edematous. Further inquiry reveals delayed menstruation, with only slight yellow discharge. This indicates Yin-Cold has entered the blood collaterals; the disease originated during the puerperium (Diagnosis is clear and precise, written with masterful brushwork.) After menstruation, the Eight Extraordinary Vessels are involved; the body is stooped and cannot straighten, unable to bend forward or arch backward. These are manifestations of Liver and Kidney pathology affecting the Extraordinary Vessels (Liver and Kidney pathology entering the Extraordinary Vessels represents another deepening level.)
