Non-crying upon birth may have various causes: some result from difficult labor causing neck injury by maternal compression; some from the umbilical cord winding around the neck; some from cold weather and pestilential qi forcing upon the infant; others from a membrane forming at the anus, blocking the infant's qi mechanism. For neck injuries, massage the area with warm hands and gently suckle the site with the mouth. For those affected by cold, bathe the infant in warm water, and have a clean woman use her tongue to suckle the infant's tongue to unblock the qi. For those with an anal membrane, pat it with the hand to break it; if it does not break, pierce it with a silver hairpin.
If the infant does not know how to suckle milk three days after birth, this is due to foul turbidity entering the abdomen. Abdominal fullness and shortness of breath are also caused by cold contracted in the womb, leading to abdominal pain and vomiting. This can be regulated using the Seven Qi Decoction (see "Fetal Cold") from *Zhi Mi*.
Umbilical tetanus occurs after cutting the umbilical cord, when water-dampness, wind, and cold pathogens invade, entering the navel and flowing into the heart and spleen, causing abdominal distension, swollen navel, excessive crying, inability to suckle, and in severe cases, wind-induced convulsions. The Zhimi Decoction (see "Fetal Cold") should be used with discretion. If the skin around the navel turns cyanotic-black, and the mouth is tightly closed, it indicates internal convulsion, which is incurable. Black fingernails also indicate an incurable condition. If the cause is heat in the chest, manifesting as body extension and straining, it may also cause navel swelling; the Mutong San (see "Fetal Heat") should be used with discretion. "Discretion" means one cannot fully rely on the original formula. Treatment varies according to wealth or poverty at birth; constitution differs based on innate endowment (thick vs. thin); efficacy varies with seasonal cold and heat. A physician who does not understand these principles cannot bear the responsibility of saving lives.
Mouth-locking presents with panting, absence of crying sounds, visible cyanotic veins on the abdomen, and pain drawn inward by hernia-related internal qi. This is caused by fetal qi combined with wind-pathogens invading the navel. Treatment should focus on dispersing and draining stagnation. Use Zhimi Decoction (see "Fetal Cold"), remove Cinnamon Twig (Guizhi), add Rhubarb (Dahuang), Uncaria (Gouteng), and Bombyx Batryticatus (Jiangcan), or combine with Schizonepeta Decoction.
Schizonepeta Decoction consists of: Coptis Rhizome (Chuan Huanglian), Schizonepeta Spike (Jingjie Sui), Rehmannia Root (Sheng Dihuang), and Licorice Root (Sheng Gancao).
Add Rhubarb (Dahuang) for constipation. Add Wood Vessel (Mutong) for oliguria.
Lockjaw presents with closed eyes, tightly clenched jaw, gradually diminishing cry, fleshy accumulations on the tongue resembling millet grains, inability to suckle, foaming at the mouth, and obstruction of urination and defecation. In such cases, first check the upper palate for small spots; if present, gently scrape them open with a fingernail. Then administer Six-Directional Cooling Clearing Decoction, adding Costus Root (Muxiang) and White Cardamom Seed (Baikou Ren) to promote bowel and organ movement. Subsequently, regulate with modified Zhimi Decoction (see "Fetal Cold"). If the jaw cannot be opened, use raw Arisaema Rhizome (Sheng Nantianxing), peeled and de-navelled, ground into powder, mixed with a small amount of Borneol (Longnao), and applied to the molar roots with ginger juice using a finger, which will immediately open the mouth. (This method is also effective for lockjaw caused by stroke in adults.)
Six-Directional Cooling Clearing Decoction consists of: Angelica Sinensis Radix (Danggui), White Peony Root (Baishao), Licorice Root (Gancao), and Rhubarb (Dahuang).
Although these three conditions differ in presentation, their source is identical. Generally, they are all caused by fetal qi stagnation and blockage. It is advisable to feed the infant with fermented soybean juice (Danshichidou Zhi) to expel fetal toxins. In severe cases, Zhu Yin Pill (see "Fetal Fright") may also be used. At birth, feeding a few mouthfuls of fermented soybean juice not only expels fetal toxins but also helps nourish spleen qi. For umbilical tetanus, mouth-locking, muteness, and refusal to suckle, moxa acupuncture on the Yongquan acupoint (KI1) on the left foot for males and the right foot for females can cure the condition immediately. Another method: roast centipede powder, mix with a small amount of Musk (Shexiang), and administer with pig milk to immediately relieve lockjaw. Alternatively, remove the legs and mouth of a spider, roast until charred, grind finely, and administer with pig milk, which is also effective. Adding a small amount of Musk can also treat gingival gangrene.
In remote areas where medicines are scarce, these empirical treatments are recorded to beg for life for infants in swaddling clothes. Do not neglect them.
Subsequent volumes follow this pattern.
