When one dreams of sexual intercourse without ejaculation, it indicates that the Qi of the Heart and Kidney is substantial; when one dreams of sexual intercourse and ejaculates immediately, it indicates that the Qi of the Heart and Kidney is deficient. This disease originates from the Heart and Kidney and cannot be cured by medicine alone. One should roll paper into a wick eight cun long, tie it tightly around the scrotum every night, and untie it at dawn; naturally, emission will cease. If Kidney Qi is exhausted and cold semen issues spontaneously, moxibustion on Guanyuan (CV4) for six hundred moxa cones will result in recovery. If a person ejaculates upon merely seeing a woman's essence, this signifies extreme deficiency of the Heart and Kidney; taking five liang of Grand Elixir (Da Dan) is indicated, and in severe cases, moxibustion on Jumen (ST16) for fifty moxa cones. (Zhang Zhongjing stated: "Coldness in the Yin causes spontaneous emission, with soreness and emaciation preventing walking." From this, we know that the insecurity of semen is due to the lack of Yang's sealing function. The Master said: "When Kidney Qi is exhausted and cold semen issues spontaneously, warming and tonifying the Lower Yuan is the appropriate method." If physicians of the world understood this principle, they would not exclusively use cooling herbs to treat nocturnal emission, thereby avoiding causing premature death.)
This condition arises from insufficient Kidney Qi combined with dampness invading the Small Intestine, causing pain connected to the navel, moving up or down like a running piglet, or pain extending to the external genitalia forming hernia. In such cases, take Taqi San, Zhuyu Wan, Jinlingzi Wan, or Pansong San. (Ben Tun [Rushing Pig] and Shan [Hernia] are different and must not be confused. When Qi rushes from the lower abdomen up to the heart, it is Ben Tun [Rushing Pig]; when it rises from the lower abdomen to the navel with transverse and taut sensations, it is Shan [Hernia]; when it rushes from the scrotum up to the chest and diaphragm with fatal pain, it is Chong Shan [Rushing Hernia]; when it descends from the lower abdomen to connect with the kidney region, it is Xiaochang Shan [Small Intestine Hernia] and Hu Shan [Fox Hernia]. These have distinctions and must be carefully examined.)
This syndrome is caused by Lung Qi deficiency, where Qi fails to descend, leading to congealing of Cold, causing pain in the chest and diaphragm radiating to the back, or vomiting of cold, sour water. Taking Wuge San will lead to self-healing. (If this syndrome is treated improperly, over time it will inevitably develop into Ge Syndrome [Dysphagia/Obstruction].)
This is due to extreme exhaustion of Kidney Qi. Since the Kidney governs the bones, once Kidney Water is depleted, all bones become dry, gradually shortening and shrinking. Delayed treatment leads to death. Moxibustion must be added, and internally administered pills containing Cinnabar (Dan) and Aconite (Fu) are required; ordinary herbal medicines cannot treat this. (It seems that as people age, they gradually become shorter and stouter; could this be similar to the Bone Shrinking Disease?)
Clinical Case
A man was five chi tall. Due to excessive alcohol and sexual indulgence, he gradually felt his muscles wasting away. I instructed him to undergo moxibustion on Guanyuan (CV4) for three hundred cones and to take one jin of Baoyuan Dan. Afterwards, his stool became loose, urination increased, food intake gradually rose, and muscles regenerated. Within half a year, he returned to his normal state. (This spontaneous emaciation differs from Bone Shrinking Disease; it is unclear why it is attached here, suggesting possible missing text in the original manuscript.)
