Some cases of deafness are caused by invasion of wind-cold, while others result from insufficiency of Heart Qi. In such cases, Yizui Gao (Intoxicating Paste) should be taken with warm wine; recovery occurs upon sweating. For individuals who indulge excessively in alcohol and sexual activity, leading to Kidney deficiency-induced hearing impairment, it is advisable to first take half a jin of Yanshou Dan (Longevity Pill), followed by Yizui Gao. However, deafness due to excess patterns is difficult to treat. (The Kidneys open into the ears, and the Zong Qi [Gathering Qi] of the Stomach also branches to the ears; therefore, most cases of deafness belong to deficiency patterns. The mention here of Heart Qi insufficiency treated with Yizui Gao is unclear in its rationale. The statement that excess deafness is hard to treat requires further examination. Note by Qi: When a person over sixty remains vigorous and strong like a young adult but suffers only from diminished hearing, this is a sign of solidified Kidney Qi and often indicates longevity, requiring no treatment. The book’s statement "excess deafness is difficult to treat" likely refers to this specific condition.)
For those living in mountainous areas, drinking water from streams containing Jiangli Stone can cause goiter or tumors. Xiao Feng San (Wind-Dispersing Powder) should be administered (likely referring to Xiaoying San, Goiter-Resolving Powder). In early stages, Jiang Fu Tang (Ginger and Aconite Decoction) is taken. Blood goiters and blood tumors cannot be treated; reckless treatment may prove fatal.
The "Three Worms" refer to Ascaris lumbricoides (Roundworm), Enterobius vermicularis (Pinworm), and Taenia saginata/pork tapeworm (Inch-white worm). Excessive consumption of raw, cold, hard foods, as well as greasy or unpalatable substances during childhood, leads to worm infestation over time. Heavy beef consumption specifically generates Inch-white worms. Roundworms are five to six cun long; when active, they cause heart pain and vomiting of clear fluid, and death ensues if the worm penetrates the heart. Inch-white worms resemble gourd seeds, reproducing parent-to-offspring, measuring two to three cun long; their activation causes abdominal pain. Pinworms are fine as hair, circulating throughout the body with Qi and Blood; if residing in the skin, they transform into leprosy-like conditions; if lodged in the abdomen, they form Pinworm Masses (Nuo Jia); if penetrating the large intestine, they cause hemorrhoids and fistulas. All these conditions are suitable for treatment with An Chong San (Worm-Anchoring Powder). If the anus itches and hurts, a small amount of Calomel (Qing Fen) should be ingested; the next day, all worms will be expelled, including the Inch-white worms.
Clinical Case:
A woman suffered from abdominal distension, to which various medicines were ineffective. I instructed her to expose her abdomen for examination. Her skin was yellow and shiny like a mirror surface, indicating a Pinworm Mass. First, one jin of roasted beef was prepared for her to eat. Afterward, she took five fen of Calomel mixed with raw sesame oil. She passed one he of pinworms, thread-like in appearance. Subsequently, she took An Chong San and recovered.
