[General Discussion] This condition initially arises from a preference for drinking cold substances. Or exposure to cold. Or excessive hunger. Or sudden great anger or fright. Or turbulence while traveling by boat or carriage, damaging the Stomach Qi. External pathogenic factors and internal injury lead to a disruption and separation of Yin and Yang, resulting in sudden pain in the heart and abdomen, with vomiting and diarrhea above and below. Predominantly Yang patterns tend toward heat, while predominantly Yin patterns tend toward cold. Heart pain precedes vomiting; abdominal pain precedes diarrhea. At the onset, light ginger-salt soup should be used, allowing the patient to drink slowly and vomit slowly, continuing until nothing remains. If vomiting and diarrhea occur immediately upon pain, the condition is mild, as the ingested harmful substance has been completely expelled. Do not give grain food, rice, or rice water; ingestion can be fatal. Wait until extreme hunger sets in, and two to three hours have passed since the cessation of vomiting and diarrhea, before offering thin porridge. After vomiting stops, do not recklessly use cooling medicines. Because after vomiting and diarrhea, the Spleen and Stomach are even more deficient, treatment should focus on harmonizing the Stomach and strengthening the Spleen.
[Dry Huoluan (Cholera-like Syndrome)] There is churning pain in the chest and abdomen, distension and urgency, with mental restlessness. The desire to vomit exists but cannot be expelled; the desire to defecate exists but cannot pass. This is what is currently called "Intestinal Colic Sha" (Jiaochang Sha), a most critical condition. Immediately stir fried salt into boiling water, add three fen of Gleditsiae Sinensis Endocarp Powder (Zaojia Mo), wait for the liquid to cool, and administer via gavage to induce profuse vomiting, which may save life. Alternatively, use one qian of raw Alum Powder (Baifan Mo) dissolved in boiling water, wait for it to cool, and administer. If symptoms persist, prick the ten fingertips to bleed (as per Sha treatment methods). On the upper part of the patient's ankle crease, pat with warm water-dipped cloth; when purple-red marks appear, prick the skin to release purple blood for immediate cure. Pat the popliteal fossa dozens of times with hand; when greenish numb bumps appear, prick to release black blood for immediate cure. If extremities are cold, tetany extends to the abdomen, accompanied by phlegm obstruction, sweating, cold breath, and near-death state, use a bag of fried salt to moxa-heat the epigastrium and abdomen to penetrate warmth, then use another bag to heat the back. Once extremities are warm, mix equal parts of Eugenia Caryophyllata Flower Bud (Dingxiang) and Amomum Cardamomum Seed (Baidoukou), grind to powder, and take one qian mixed with Ginger Decoction. For severe cold, multiple doses are permissible without restriction. Stir-fried Evodiae Fructus (Wuzhuyu) three to four liang and several liang of salt, heated and applied to below the umbilicus, is also effective. In critical Huoluan nearing death, fill the umbilicus with salt and apply moxibustion fourteen Zhuang (units of moxa cone), leading to immediate recovery. Pulse floating and surging indicates easy treatability. If pulse is faint and slow, accompanied by shortness of breath, lethargy, and muteness, it is difficult to treat. After vomiting, continue to slowly administer rice water as before. For thirst after cessation of vomiting and diarrhea, use Atractylodis Macrocephalae Rhizoma (Baizhu), Poria (Fuling), Polyporus (Zhuling) (each one and a half qian), Alismatis Rhizoma (Zexie) (two and a half qian), and Cinnamomi Cortex (Rougui) (five fen, increase for cold constitution), decoct in water and take. For no thirst, use Atractylodis Macrocephalae Rhizoma (Baizhu) and Ginseng Radix et Rhizoma (Renshen) (each three qian), Glycyrrhizae Radix et Rhizoma Preparata (Zhigancao) (one qian), Zingiberis Rhizoma (Ganjiang) (two to three qian), decoct in water and take. For unceasing diarrhea, add Tangerine Peel (Juhong) and Poria (Fuling) (each one qian), Myristica Fragrans Seed (Roudoukou) (flour-roasted, one qian). For unceasing vomiting, add Eugenia Caryophyllata Flower Bud (Dingxiang) and Pinelliae Rhizoma Preparatum (Jiang Banxia) (each one qian); stability is achieved after two to three doses.
[Heatstroke-induced Huoluan] Luffae Fructus Retinervus Leaf (Siguye) (one piece) and White Frost Plum (Baishuangmei) (one fruit), these two ingredients ground extremely fine, mixed with fresh well-water and taken, leads to immediate cure.
[Dampness-induced Huoluan with Vomiting and Diarrhea] Watery diarrhea with little abdominal pain indicates Dampness. Common in summer and autumn. Use Atractylodis Rhizoma (Cangzhu) (stir-fried), Citri Reticulatae Pericarpium (Guangpi), Pinelliae Rhizoma Preparatum (Jiang Banxia) (each one and a half qian), Agastaches Herba (Huoxiang) (two qian), Glycyrrhizae Radix et Rhizoma Preparata (Zhigancao) (five fen), add Zingiberis Recens Rhizoma (Shengjiang) (three to five slices) and two Jujubes (Zao), decoct in water and take. One formula adds Perillae Folium (Suye), Platycodonis Radix (Jiegeng), and Magnoliae Officinalis Cortex (Houpo).
[Unceasing Huoluan Vomiting and Diarrhea] Tangerine Peel (Chenpi) and Agastaches Herba (Huoxiangye) (each three to five qian), decoct in water and take. Or grind to powder, take mixed with hot wine. Effective for any case retaining even a trace of Stomach Qi. For unceasing dysentery and diarrhea with tetany extending to the abdomen nearing death, crush three liang of Zingiberis Recens Rhizoma (Shengjiang), add two bowls of wine, boil three to four times, take in one dose to resolve. Alternatively, take one handful of Artemisia Argyi Herb (Aiye) decocted in three bowls of water down to one bowl, taken in one dose, also effective.
[Unceasing Thirst and Irritability in Huoluan] Three he of Glinis Japonici Semen (Nuomici) and one he of White Honey (Baimi), ground together, boiled for juice, and drunk stops the condition. If vomiting and diarrhea have ceased but irritability, heat, and excessive thirst persist, with difficult urination, use Ophiopogonis Radix (Maidong), Poria (Fuling), Pinelliae Rhizoma (Banxia), Citri Reticulatae Pericarpium (Chenpi), Atractylodis Macrocephalae Rhizoma (Baizhu) (each one and a half qian), Ginseng Radix et Rhizoma (Renshen) and Triticum Aestivum Fructus (Xiaomai) (each one qian), Glycyrrhizae Radix et Rhizoma (Shenggancao) (six fen), add three slices of Zingiberis Recens Rhizoma (Shengjiang) and half a Prunus Mume Fructus (Wumei), decoct in water and take.
[Unceasing Vomiting/Diarrhea with Tetany] One bowl of juice from捣碎 Dolichos Lablab Leaf (Baibiandouye), mixed with a small amount of Rice Vinegar (Micu), taken warm. Effective immediately for cases arising from Summerheat. For tetany causing spasm of the ten fingers unable to flex or extend, moxa-burn five Zhuang on the tip of the lateral malleolus of the foot (using moxa cones the size of wheat grains). Stops immediately. For unceasing tetany, boil a large handful of Polygonum orientale Herb (Liao) in water for strong fumigation and washing, or soak cloth in the decoction to apply heat to the affected area.
[Ordinary Tetany] Occurring in all seasons not caused by Huoluan. Caused by Blood deficiency, applying crushed Garlic (Dasuan) to the sole of the foot stops it. Follow up with Siwu Tang (see Stroke section), adding Scutellariae Radix fried in wine and Carthami Flos (Honghua) (each one to two qian), decoct in water and take. Some cases involve tendon movement starting from the big toe and stopping at the waist, caused by excessive rich nourishment leading to Wind-Cold-Damp invasion. Take one liang of Clematis Chinensis Radix et Caulis (Weilingxian) daily with two cups of aged wine; removing Dampness stops the condition. For tetany occurring during stretching/yawning in sleep, for men, pull the genitals with hand; for women, pull the breasts near both sides; this stops it.
[Tetany Extending All Over Body and Abdomen, Intolerable] Soak affected area in heavy salt water, or strong Polygonum orientale Herb decoction or Alum solution, all effective. Or rub salt on painful areas for thirty to fifty circles; even if skin breaks, it is harmless and can eradicate the root cause.
[Liver Deficiency Tetany] Three liang of Polygonum orientale Stem and Leaf (Chiliao), one cup of water, one bowl of wine, decoct to six parts and take.
[Tetany Pain and Spasm] Two liang of Pinus Massoniana Node (Songjie), cut like rice grains, add one qian of Boswelliae Resina (Ruxiang), stir-fry slowly in a clay vessel until scorched (remove fire toxicity, then grind to powder). Take one qian per dose, gradually increasing to two qian, taken with hot Quince Wine (Muqiu Jiu).
[Tetany Unresponsive to Various Medicines] Two whole male Pig Kidneys (Xiong Zhushen), thinly sliced, fried in Sesame Oil (Xiangyou) until cooked, chewed finely on an empty stomach, washed down with Salt Water three to four times. Effect like a deity.
