Turbid discharge (Zhuo Bing) refers to cloudy and impure urine. Formulary texts generally attribute this condition to the Kidney, whereas current practice seeks its origin in the Spleen. Damp-heat from the Spleen Earth descending downward results in turbid discharge. When dampness exceeds heat, it manifests as white turbidity; when heat exceeds dampness, it manifests as red turbidity. The method of treatment is nothing more than guiding out and clearing the damp-heat. Once the damp-heat is eliminated, the turbid urine becomes clear.
*Yi Jian* states: Regarding Lin syndrome (Strangury) and Turbid discharge, a fine and rapid pulse is not necessarily detrimental. In mild cases of Shaoyin disease, it is due to Qi obstruction in the Bladder. If seen in women accompanied by sores in the vagina, it is a substantial excess pattern that is easy to cure. However, a fine and choppy pulse indicates death.
1. For early-stage turbid discharge, one should guide out the damp-heat. The appropriate formula is Erchen Tang (Two-Aged Decoction), modified with Cangzhu (Atractylodis Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Huangbai (Phellodendri Cortex), and Bibing (Dioscoreae Hypoglaucae Rhizoma). For red turbidity, add Danshen (Salviae Miltiorrhizae Radix et Rhizoma). (See Article 83)
2. For turbid discharge persisting for some time, one should consolidate the essence pathway and promote water metabolism. The appropriate formula is Bixie Fenqing Yin (Cocculus Decoction for Clarifying Turbidity). (See Article 83)
3. *Neijing* states: Deficiency of Middle Qi causes abnormalities in urination, which differs from turbid discharge. The appropriate formulas are Sijunzi Tang (Four Gentlemen Decoction) or Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). (See Article 5)
4. Decline of Mingmen Fire (Gate of Life Yang) leads to an inability of Qi to secure essence, often resulting in degenerated essence transforming into turbidity. The appropriate formula is Guifu Bawei Wan (Cinnamon Twig and Aconite Pill containing Eight Ingredients), added with Tusizi (Cuscutae Semen) and Cheqianzi (Plantaginis Semen), to guide out the degenerated essence.
5. Turbid discharge exiting from the essence orifice (seminal duct) differs from Lin syndrome exiting from the urinary orifice. In conditions lasting longer, one should tonify the Kidney and avoid using diuretics. This is a crucial principle.
In the Treatise on Cold Damage section, spontaneous sweating is attributed to Wind injury, and night sweating to Shaoyang syndrome. In other miscellaneous diseases, spontaneous sweating is attributed to Yang deficiency, and night sweating to Yin deficiency. However, Yin and Yang are mutually rooted; spontaneous sweating can also result from Yin deficiency, and night sweating from Yang deficiency. These must be differentiated and treated accordingly.
1. Spontaneous sweating due to Yang deficiency: The patient often fears cold. The appropriate formulas are Shenfu Tang (Ginseng and Aconite Decoction), Shufu Tang (Atractylodes and Aconite Decoction), or Fuzi Tang (Aconite Decoction). (See Article 105)
2. Night sweating due to Yin deficiency: The patient often experiences fever. The appropriate formula is Danggui Liuhuang Tang (Angelica Hexa-Huang Decoction). (See Article 6)
3. Deficiency of both Yin and Yang, presenting with spontaneous sweating, night sweating, palpitations, insomnia, and irritability: The appropriate formula is Guipi Tang (Restore the Spleen Decoction), added with Maidong (Ophiopogonis Radix) and Wuweizi (Schisandrae Fructus). (See Article 20). Renshen Yangrong Tang (Ginseng Nourishing the Vigor Decoction). (See Article 12)
4. Spontaneous sweating accompanied by fever, caused by improper treatment of previous Wind injury: The appropriate formula is Yupingfeng San (Jade Windscreen Powder). (See Article 87)
