Wendu (Warm Toxin) arises from abnormally warm weather in winter, where the human body contracts a perverse and chaotic pathogenic factor. By the transition between spring and summer, upon contracting additional warm-heat pathogens, the latent toxin erupts from within, resulting in heat affecting both the exterior and interior. Additionally, conditions such as Fengwen (Wind-Warmth), Wenbing (Warm Disease), and Dongwen (Winter-Warmth), if mistakenly treated with acrid-warm formulas, akin to adding fire to fire, can also result in this disease. The pulse is vigorous at both superficial and deep positions. Symptoms include mental restlessness, fever, thirst, cough, and sore throat; the tongue is crimson with a yellow coating. The appropriate treatment method is Clearing Heat and Resolving Toxicity (Qing Re Jie Du), supplemented with Gancao (Glycyrrhizae Radix et Rhizoma) and Jiegeng (Platycodonis Radix). However, one must be aware that Wendu may manifest as macules (Ban), papules (Zhen), parotid swelling (Fayi), or throat swelling.
Generally, when warm-heat toxin reaches the Yangming channel and manifests in the muscles, it forms macules. Red color indicates mild Stomach heat (light condition); purple indicates severe heat (heavy condition); black indicates extreme heat (critical condition); bright red indicates the pathogen is being expelled outward (auspicious prognosis). When macules are about to erupt but have not yet appeared, the treatment method of Cooling and Clearing to Transfuse Macules (Qing Liang Tou Ban) should be used. If macules appear accompanied by mental confusion, Xijiao (Rhinocerotis Cornu) and Yuanshen (Scrophulariae Radix) should be added. *Yizong Xinfa* states: "Papules arise in the Ying (Nutrient) level; Ying governs blood, hence the red color." *Banghe* states: "When pathogenic factors remain unresolved and heat enters the blood collaterals, papules form." Papules follow the same pattern: red is light, purple is heavy, black is critical. Although the pathogen remains unresolved and heat resides in the Ying level, the Wendu has manifested on the skin and hair (exterior), which differs significantly from macules located in the muscles. Since the Lung governs the skin and hair, and the Stomach governs the muscles, ancient practitioners stated that macules belong to the Foot-Yangming Stomach disease, while papules belong to the Hand-Taiyin Lung disease. Their boundaries are distinct and cannot be confused. Wu Jutong conflated them without distinction; Xu Lingtai (Xugu) has already refuted this error, which is indeed correct. When papules are about to erupt but have not yet appeared, rapidly apply the Acrid-Cool Exterior-Releasing method (Xin Liang Jie Biao), adding Sheng Dihuang (Rehmanniae Radix) and Lv Dou Yi (Phaseoli Semen Integrum). For severe cases, add Qingdai (Indigo Naturalis) and Lianqiao (Forsythiae Fructus).
Furthermore, warm-heat toxin may accompany Shaoyang Ministerial Fire attacking upward, causing hardness, swelling, and pain below the ears. This is Fayi (Parotitis). Although Fayi belongs to the Yangming channel, the areas anterior and posterior to the ear are traversed by the Shaoyang meridians. It must be dissipated quickly; otherwise, pus will form, causing harm. Internally, use the Clearing Heat and Resolving Toxicity method, removing Yangshen (American Ginseng Radix) and Maidong (Ophiopogonis Tuber), and adding Mabo (Calamitidis Spora), Qingdai (Indigo Naturalis), and Heye (Nelumbinis Folium). If the face is also swollen, add Baizhi (Angelicae Dahuricae Radix) and Loulu (Euphorbiae Humifusae Radix). If the swelling and hardness do not subside, add Shan Jia (Manis Squama) and Zao Ci (Gleditsiae Spina). Externally, use Xiannhua Gen (Narcissi Bulbi Radix), peel off the red skin and rootlets, pound into a paste in a mortar, and apply to the swollen area. Change it when dry, continuing until millet-sized yellow pustules appear on the skin.
Additionally, warm-heat toxin may ascend and knot in the throat, forming Zhongbi (Throat Obstruction). *Neijing* states: "The binding of one Yin and one Yang is called Hou Bi (Throat Bi)." One Yin refers to the Hand-Shaoyin Ministerial Fire; One Yang refers to the Hand-Shaoyang Ministerial Fire. The meridians of these two channels both connect to the throat. Now, with Wendu gathering here, both Ministerial Fires arise together. Fire generates phlegm; phlegm obstruction causes swelling; severe swelling causes obstruction; severe obstruction leads to death. Urgently use Yuyaochi (Jade Key Powder) to open the throat, followed by the Clearing Heat and Resolving Toxicity method, removing Yangshen (American Ginseng Radix) and Maidong (Ophiopogonis Tuber), and adding Jiangcan (Bombyx Batryticatus), Jiegeng (Platycodonis Radix), Niubangzi (Arctii Fructus), and Shegan (Belamcandae Rhizoma).
Wendu involves numerous variable manifestations. Since macules, papules, parotid swelling, and throat obstruction occur frequently, they are specifically highlighted here for clarification.
