All diseases of lower back pain (Yao Tong) originate from the Kidney. This is because the Kidney connects to the lumbar spine. The manifestations of lower back pain vary among different meridians; treatment should be based on syndrome differentiation.
When lower back pain radiates to the neck and spine, feeling heavy as if bearing a burden, it is a disease of the Foot-Taiyang Meridian. Prick the central meridian to bleed. In spring, avoid bleeding (referring to Weizhong BL-40).
When lower back pain feels like needle pricks, preventing flexion and extension, and inability to look back, prick the end of the "Chenggu" bone of the Shaoyang meridian with slight bleeding. In summer, avoid bleeding ("Chenggu" refers to the prominent bone on the lateral side of the knee).
When lower back pain prevents looking back, accompanied by frequent sadness and weeping, it is a disease of the Yangming meridian. Prick the Yangming meridian three cun anteriorly, stabbing up and down to bleed. In autumn, avoid bleeding (referring to Zusanli ST-36).
When lower back pain radiates to the medial aspect of the spine, it belongs to the Shaoyin meridian. Prick the acupoint two cun above the medial malleolus to bleed. In spring, avoid bleeding. Excessive bleeding cannot be recovered (referring to Fuliou KI-7).
When the waist feels tight like a drawn bowstring, it belongs to the Jueyin meridian. Prick the lateral side of the "fish belly" at the heel. Follow the sensation until nodules are felt, then prick (referring to Ligou LR-5).
When lower back pain radiates to the shoulder and eyes, causing blurred vision and occasional enuresis, the disease is in the Jie Mai. Prick between the muscles inside the knee joint, bleeding from the lateral side. Stop when the blood color changes (this is also a Taiyang acupoint).
When lower back pain feels like being pulled by a belt, often feeling as if the waist is broken, and prone to fear, it is also a disease of the Jie Mai. Prick the knotted collaterals in the popliteal fossa resembling millet grains. Blood will shoot out; stop when dark blood appears (referring to Weizhong BL-40).
When lower back pain feels like a small awl is inside, with local swelling, the disease is in the Tongyin meridian. Prick the end of Jiegu (Gaohuang GB-39), inserting three needles.
When lower back pain presents with local swelling, it belongs to the Yangming meridian disease. Prick the area about one chi below the calf muscle belly (referring to Chengshan BL-57).
When lower back pain prevents flexion and extension, and fear of falling upon extension, caused by lifting heavy objects injuring the waist, severing collateral vessels, and causing blood stasis, prick between the Yang tendons, several cun above, horizontally inserting two needles (referring to Weiyang BL-38).
When upper lower back pain is accompanied by profuse sweating, thirst after sweat dries, and restlessness after drinking, it belongs to the Huiyin meridian. Prick the Qiao vessel five cun above and below, horizontally inserting, and bleed where vessels are full (referring to Chengjin BL-56).
When lower back pain is hot and irritable, leading to sorrow and fear, the disease belongs to the Feiyang meridian. Prick five cun above the medial malleolus, at the intersection of the Shaoyin meridian and Yinwei vessel (referring to Fuliou KI-7 and Zubin KI-9).
When lower back pain radiates to the chest, with blurred vision, severe cases involving opisthotonos and inability to speak, the disease is in the Changyang meridian. Prick anterior to the large tendon above the medial malleolus, posterior to the Taiyin meridian, about two cun above the ankle (referring to Jiaoxin KI-8).
When lower back pain is accompanied by heat, irritability, a sensation of a horizontal block in the lower waist, and severe enuresis, the disease is in the San Mai. Prick the space between bone and muscle anterior to the knee, on the lateral aspect of the collateral vessels, inserting three needles (referring to Diji SP-8).
When lower back pain prevents coughing, and coughing causes muscle spasm, the disease is in the Rouli meridian. Prick posterior to the lateral Taiyang and distal Shaoyang (Jiegu), inserting two needles (referring to Fenrou).
When lower back pain involves the spine, stiff neck, blurred vision, and desire to fall, prick an acupoint to bleed.
For coldness in the upper waist, prick Foot-Taiyang and Foot-Yangming meridians; for heat in the upper waist, prick Foot-Jueyin; for heat in the middle waist with panting, prick Foot-Shaoyin. Prick an acupoint to bleed.
For coldness in the upper waist preventing looking back, prick Foot-Yangming; for difficult defecation, prick Foot-Shaoyin; for lower abdominal fullness, prick Foot-Jueyin.
If lower back pain feels like breaking, preventing flexion/extension and lifting, prick Foot-Taiyang and Foot-Yangming.
If pain radiates to the lower abdomen and testicles, preventing extension, prick the intersection of the waist and sacrum, above the malleoli. Determine the number of insertions based on the lunar cycle. Pain stops immediately upon needling. Treat left disease with right points, and right disease with left points. This is the method for treating lower back pain. Besides these acupoints, there are remaining methods attached below.

Wang Shi said: A woman with chronic illness had severe lower back pain. After moxa on Yao Yan, the doctor heated a needle in fire and pricked the spot. Initially shallow, pain stopped later. Thus, "fire does not fail people" is credible.
Xu Zhike once had abdominal roaring after flood waters. After recovery, he developed lower back pain preventing flexion/extension. This was likely Kidney meridian affected by water qi. Moxa Shenshu BL-23 thirty-seven times, took Deer Antler Pill to cure. Wang Shi said: I believe moxa on Shenshu is effective for lower back pain preventing flexion/extension; Deer Antler Pill is optional.
Wang Shi said: My brother had severe lower back pain making walking difficult. I used fire needle to frequently prick Shenshu BL-23 lightly, restoring normal gait. No moxa initially. Many people with hunchback came for point moxa. I suspected tendon disease, pointed Yanglingquan GB-34, told them to moxa at home. It cured. Yanglingquan is the Hui of Tendons. Thus, lower back pain should not rely solely on Shenshu BL-23, ignoring other points.
