Ke Ni (Cough with Qi Reversal) refers to coughing accompanied by upward reversal of Qi. Qi belongs to Yang, circulates through the Zang-Fu organs, and disseminates through the Cou Li (interstices); this is governed by the Lung. Cough diseases arise from Lung deficiency combined with slight cold invasion. Cold condenses with Qi, preventing its dissemination; Stomach Qi reverses and accumulates, rushing upward, causing distension in the lungs and lateral abdomen. The reversed Qi does not descend but again combats the Stomach. Due to weak Stomach Qi and latent cold in the Spleen, pathogenic Qi rushes into the Stomach tube. The Stomach fails to control it, causing Yin and Yang Qi to clash, resulting in Ke Ni. Symptoms include cough, chest fullness, Qi reversal, shoulder and back pain, sweating, and pain in the buttocks, yin region, thighs, knees, and feet. Whether in men or women, Ke Ni is colloquially called "Chi Te" (hiccup/hem), caused by disharmony between Yin and Yang. Fire desires to ascend but is suppressed by cold; since cold cannot overcome fire, it produces a stagnant sound. This syndrome can also occur in Shang Han (Cold Damage) and should be treated with Jiji San.
Youqi Tang: Treats Jue Ni (Reversal Cold/Extremity Cold). Caused by excess organ Qi, cold-induced consumptive disease, worry, and fright. The patient is prone to palpitations, feels cold in the chest intermittently, experiences unpredictable upper and lower sensations, often feels sad, has wandering Qi flowing through the four limbs and around the umbilicus, often presents with nodular wandering swellings, and suffers from constipation.

Grind the above herbs into powder. Add 9 sheng of water, decoct to obtain 3.5 sheng, strain, and divide into five doses. Take three times during the day and twice at night. If abdominal pain occurs, remove Huangqin and add Shaoyao (Paeoniae Radix).
