After typhoid fever transforms into bloody dysentery, it is because after the original disease has healed, the heat toxin has not yet dissipated. It takes advantage of the body's deficiency to attack the intestines and stomach, struggling with body fluids, thus causing dysentery with pus and blood. If the toxic qi is severe, high fever and abdominal pain occur; if damp-toxin is added, the discharged feces resemble fish brains or rotten meat. Additionally, when typhoid fever is unresolved and Shaoyin disease presents with dysentery discharging pus and blood, this is also due to the struggle between dampness and heat.
The song states: Dysentery with pus and blood accompanied by delirious speech indicates severe damp-toxin like foul intestinal mucus. Polyphagia, dry stool, and accumulation below the umbilicus indicate heat binding in the Bladder meridian among the six meridians.
The Chong Mai (Chong Vessel) is called the Blood Chamber. Both men and women possess this blood qi and Chong Mai. When the Chong Mai receives heat, blood will flow recklessly. In men, it manifests as lower bleeding and delirium; in women, during the time when menstruation has just ceased, the meridian qi is still deficient, allowing pathogenic factors to enter due to deficiency. Or, after heat subsides, chest fullness and delirium occur, or blood stasis causes alternating chills and fever resembling malaria. All these are referred to as "Heat Entering the Blood Chamber." I privately wonder why common secular talk always prioritizes regulating qi when treating diseases, rarely considering "blood." Regarding the theory of blood in the Three Powers, it is said that only women have this condition, failing to realize that blood qi is yin and yang. Humans bear yin and embrace yang, standing between heaven and earth; who does not possess this blood qi? Otherwise, formulas such as Taoren Chengqi Tang (Peach Kernel Decoction for Ordering the Qi) and Didang Wan/Pills (Defeat Stasis Pills) mentioned in the books of Zhang Zhongjing and Li Dongyuan, would be set exclusively for women. However, how do blood syndromes present? The answer is: those with blood involvement have pulses that are intermittently choppy and rapid, flickering bright and dim, or deep, fine, and hidden. If blood heat attacks mutually, the Cun and Guan positions are洪大 (vast and vigorous), mostly seen on the left hand. The left hand governs blood, indeed so. Classics state: Blood reversing upward leads to madness; blood accumulating downward leads to mania. Lower Jiao blood stasis results in free urination. Locations of blood binding should be inferred accordingly. External signs appear in hematemesis resolved by sweating; Yangming disease with lower bleeding and delirium, or chest and hypochondriac fullness resembling Jie Xiong syndrome, seeing ghosts at dusk—this is Heat Entering the Blood Chamber. With head sweating, puncture Qimen (LR14) to drain Liver fire. If not punctured, combine with Xiaochaihu Tang (Minor Bupleurum Decoction), adding Sheng Dihuang (Radix Rehmanniae Preparata), Zhike (Fructus Aurantii), and Zhizi Ren (Semen Gossypii Herbacei). Abdominal pain, body heat, dysentery with pus and blood like fish brains or rotten meat juice is called Damp-Toxin. Use Taohua Tang (Peach Flower Decoction), Diyü San (Sanguisorba Powder), Huanglian Ejiao Wan (Coptis and Donkey-Hide Gelatin Pills), Zhiyao Fang (Essential Treatment Formula using Weiqi Tang plus Muxiang), or Cherere Tang. Heat entering intestines and stomach due to deficiency, heat below the umbilicus, diarrhea with red/yellow/white intestinal mucus, use Huangqin Tang (Scutellaria Decoction), Baitouweng Tang (Pulsatilla Decoction), Baipi Tang (Phellodendron Bark Decoction). Without exterior-interior signs, after purgation, pulse remains rapid, polyphagia with frequent hunger but no bowel movement for many days, indicating blood stasis. Use Taoren Chengqi Tang, Didang Tang, or Xiaochaihu Tang plus Taoren and Dahuang. There is also a Spleen Weakness syndrome where Stomach heat causes polyphagia, covered in the relevant section. Lower Jiao blood stasis makes the person manic, Small Intestine tense, urination free. Give Didang Wan or Xiaochaihu Tang plus Taoren and Dahuang. Didang Tang is too harsh; use Didang Wan instead. Taiyang disease unresolved, heat binds in Bladder, person becomes manic, spontaneous lower bleeding heals it; if not healed, use Guizhi Tang (Cinnamon Twig Decoction). For blood stasis, use Zhi Gancao (Honey-fried Licorice Root) 0.5 qian, Chuanxiong (Ligusticum Rhizome) and Xiangfu (Cyperus Rhizome) doubled, Zhike doubled. Decoct in fresh well water, add vinegar. For blood loss, scant blood or red blood, slow pulse or choppy pulse, pulse not emerging, add Renshen (Ginseng Root) as an assistant in decoctions. Qimen (LR14) is the mansion of the Triple Burner. To locate the acupoint, use the middle segment of the patient's middle finger as one cun. Have the patient lie supine. From the center of the navel, measure up five cun vertically, mark with ink. From the ink mark, measure horizontally two and a half cuns to each side, roughly aligned with the nipples. This is the Qimen acupoint. Insert needle four fen. This treatment discussion and acupoint method can be used in conjunction with the Coughing Reversal section and Xiaru acupoints.
