In Taiyang disease (Disease of the Taiyang channel), if the Yang pulse is faint and sweating is scanty, it indicates a state of self-recovery; however, if sweating is excessive, it indicates an excess condition. In Yangming disease (Disease of the Yangming channel), if the pulse is solid and sweating was induced excessively, this also constitutes an excess condition. Excessive sweating leads to the exhaustion of Yang Qi within the interior, which in turn depletes Body Fluids (Jin Ye). This results in Heat binding internally, causing the stool to become hard and difficult to pass.
Constipation in Taiyang disease is due to excess in the Stomach Fu organ. It generally arises because during Taiyang disease, treatments such as sweating, purging, or promoting urination have depleted Body Fluids, leading to dryness in the Stomach. Consequently, the condition transforms into Yangming disease, characterized by infrequent bowel movements, internal fullness, and difficult defecation. This is a Yangming syndrome and should be treated with purgatives. However, there are cases of Yangming syndrome where purgatives are contraindicated. In such cases, one should inquire about the frequency of urination. If the patient originally urinated three or four times a day but now only twice, it can be inferred that the stool will pass soon. This is because reduced urination suggests that Body Fluids are returning to the Stomach, indicating that defecation will occur shortly. Therefore, in patients with frequent vomiting during Taiyang disease, even if Yangming symptoms are present, purgatives are clearly contraindicated. Generally, when dry feces are present in the Stomach, decoctions should be administered. If, after ingesting the decoction, the patient passes gas (flatus), this indicates dry heat binding, and purgatives are safe to use. If no flatus occurs, it suggests that the stool is only hard at the beginning and loose later; purgatives must not be used, as they would cause abdominal distension and loss of appetite. The Classics state: "Frequent urination leads to difficulty in defecation." Pi Yue Wan (Spleen Restriction Pill) formula follows.
