Dry retching in cold damage (Shanghan) refers to the sensation of wanting to vomit but nothing is expelled. This is usually caused by pathogenic heat lingering in the stomach, or after sweating therapy has resolved the exterior, leading to disharmony in the stomach and retained heat within. The heat steams upward to the Upper Jiao (Upper Burner), causing stagnation and binding in the epigastrium (Xin Xia), thus inducing dry retching. Furthermore, various pathogens invading the stomach cause rebellious Stomach Qi, resulting in vomiting. If due to cold pathogens, it involves congealed cold phlegm, with expulsion of saliva and foam, which consumes body fluids; only when there is purely rebellious Qi without any substance to expel is it termed "dry retching" (Gan Ou). Treatment requires careful differentiation and cannot be overlooked.
The verse states: Vomiting without substance is called dry retching; diarrhea associated with three types of patterns belongs to Shaoyin disease. Those accompanied by hypochondriac pain, headache, and fluid retention have their own discerning experts.
Regarding the three syndromes of Shaoyin diarrhea: If diarrhea persists, vomiting and rebellious Qi occur with a faint or imperceptible pulse, or if there is dry retching accompanied by vexation, use Baitong Tang plus Pig Gallbladder Bile Decoction (Baitong Jia Zhudanzhi Tang). If the pulse is faint, use Baitong Tang. If there is interior cold with exterior heat, a faint or absent pulse, or dry retching, use Tongmai Sini Tang (Pulse-Reviving Four Reversals Decoction). If there is dry retching with expulsion of saliva/foam and headache, use Wuzhuyu Tang (Evodia Decoction). If symptoms worsen or become complex after taking these, use Xiaochaihu Tang (Minor Bupleurum Decoction). If there is sweating, headache, and dry retching, use Guizhi Tang (Cinnamon Twig Decoction). If there is fluid retention in the epigastrium, dry retching, and fever, this belongs to an Exterior Syndrome; use Xiao Qinglong Tang (Minor Blue Dragon Decoction). If the body is cool with sweating, bilateral hypochondriac pain, dry retching, shortness of breath, epigastric fullness/stuffiness, cough, slight diarrhea, and no aversion to cold, the exterior is resolved, and there is fluid retention inside; use Shizao Tang (Ten Jujube Decoction) to purge. If there is cold-fluid retention above the diaphragm causing dry retching, warm methods can be used, such as Sini Tang (Four Reversals Decoction). If there is dry retching with diarrhea, use Huangqin Banxia Shengjiang Tang (Scutellaria, Pinellia, and Fresh Ginger Decoction). Generally speaking, heat in the stomach cavity combining with food Qi causes heat to steam upward, leading to epigastric stagnation and vomiting without expulsion; this falls under Shaoyin diarrhea, treated with ginger and aconite herbs. For Jueyin disease with expulsion of saliva/foam, Wuzhuyu Tang is indicated. For sweating with dry retching, Guizhi Tang treats spontaneous sweating. Once pathogens are eliminated, vomiting ceases naturally.
As for the two patterns caused by fluid retention, they should be distinguished by whether the pathology is in the Exterior or Interior. If there is fever, cough, wheezing, and dry retching, this indicates fluid retention in the Exterior; administer Xiao Qinglong Tang. If there is no fever, no aversion to cold, hypochondriac pain, cough, diarrhea, and dry retching, this indicates fluid retention in the Interior; administer Shizao Tang. This distinction is essential. However, why do all Exterior water-patterns involve cough? The answer is that fluid retention invades the Lung.
