If a woman frequently experiences restless fetus during pregnancy, it is due to the deficiency of the Chong and Ren meridians, resulting in an unstable conception in the Baomen Zihu (uterus). There are also cases caused by excessive alcohol consumption or sexual intercourse, leading to damage and fetal restlessness. Chao Yuanfang stated: When the Chong and Ren meridians of a woman carry wind-cold during conception, regardless of the number of months pregnant, if the fetus becomes restless due to accidental blows or touches, this may be accompanied by irregular emotions, stagnation of Qi mechanism, injury to the Heart and Liver, disturbance of blood vessels, deficiency of the Chong and Ren meridians, causing the Baomen to be insecure. Or, when climbing high or using the toilet, wind pathogen invades the Yin aperture and enters the uterus. All such conditions cause fetal restlessness. In some cases, a pregnant woman's menstruation continues without stopping, yet the fetus remains undamaged. What is the reason? Generally, women with abundant blood and deficient Qi tend to be obese. After conception, menstruation may continue normally while the fetus remains stable. If mistakenly treated as threatened abortion (Lou Tai), the fetus will inevitably miscarry. Such cases do exist. It is also said that if a woman has amenorrhea but suffers from other discomforts, this indicates pregnancy. This is because menstrual blood accumulates to nourish the fetus and transforms into milk. Once pregnant, the blood is stored to nourish the fetus; how can it scatter and move freely? The reason is that menstruation returning after conception is not necessarily due to abundant blood. It is believed that if there is wind in the Ying (Nutrient) meridian, menstrual blood tends to flow recklessly due to the dominance of wind. Since the Ying meridian is overcome by wind, what flows out is not the blood meant for nourishing the fetus. Based on this differentiation, it should also be treated as threatened abortion, requiring herbs to preserve and tonify the fetus. However, if the fetus is undamaged but strong tonification is forced upon it, this constitutes "reinforcing excess and depleting deficiency" (Shi Shi Xu Xu). The eventual miscarriage would be justified. If a physician understands the principle of wind in the Ying meridian and uses a specific herb to treat the wind, menstruation may stop, or even without medication, the fetus remains safe. Nevertheless, if the fetus was originally unstable due to unregulated sexual activity, presenting first with spotting and then miscarriage, it must be treated as threatened abortion. This requires careful examination. Some women in their prime take warming and tonifying herbs, causing the Baomen Zihu to become dry and constrained by the heat of the medicine, preventing new blood from generating and old blood from descending. If pregnancy occurs, it will either be delayed or result in inevitable miscarriage. After middle age, Qi mechanism gradually declines, leading to disorders such as metrorrhagia, leukorrhea, irregular menstruation, sallow complexion, stiffness of the waist and back, five-palm heat, and diseases of Five Exhaustions and Seven Injuries. These arise not only from wind-cold in the Baomen Zihu. Therefore, *Piao Xiao Dang Bao Tang* (Plumbum Polytmetallatum Uterus-Cleansing Decoction) in *Qianjin Yi Fang* is specifically for such ailments, though modern physicians rarely use it. Regarding fetal restlessness, there are two causes: one is maternal illness causing fetal restlessness; treating the mother's illness will naturally stabilize the fetus. The other is an unstable fetus causing harm to the mother; stabilizing the fetus will naturally heal the mother. The second type includes restlessness due to overexertion, exposure to cold or heat, irregular diet, or improper living environment. Mild cases involve slight movement and discomfort, while severe cases lead to injury or miscarriage. One should observe the mother's complexion and tongue: red face and blue tongue indicate dead fetus and living mother; blue lips and mouth with foam at the corners indicate death of both mother and fetus; blue face and red tongue with foam in the mouth indicate dead mother and living fetus.
Some say that a pregnant woman's silence is not a disease. Occasionally, this occurs and requires no medication; simply taking *Bao Sheng Wan* (Preserve Life Pill) or *Si Wu Tang* (Four Substances Decoction) near delivery allows speech to resume postpartum, which is natural law, not the merit of medicine. Physicians often claim credit for this, administering ordinary herbs near term and attributing postpartum speech recovery to their skill, which is unjustified. According to *Su Wen*, the Yellow Emperor asked: Why does a woman carrying twins lose her voice in the ninth month? Qibo replied: The collaterals of the uterus are obstructed. The Emperor asked why. Qibo explained: The uterine collaterals connect to the Kidney; the Shaoyin meridian passes through the Kidney and connects to the root of the tongue, hence the inability to speak. The Emperor asked how to treat it. Qibo said: No treatment is needed; it will recover after ten months.
It is also stated that proper care during pregnancy harmonizes Qi and Blood, ensuring fetal stability and easy delivery. Improper regulation disrupts Qi and Blood, causing frequent fetal movement and difficult labor. Fetal pressure on the Heart results from excessive effort during delivery, causing Qi rebellion and upward thrust of the fetus against the Heart. This leads to fainting and suffocation until the fetus descends; severe cases may result in death.
Furthermore, if a pregnant woman has pre-existing chronic diseases or fails to regulate her lifestyle during pregnancy, leading to illness and organ depletion, causing weak Qi and emaciation, thereby hindering fetal growth, medication is required to remove the disease and tonify Qi and Blood to support the fetus. If a pregnant woman is emaciated, complicated by illness, with depleted organs and exhausted Qi and Blood, unable to nourish the fetus, resulting in persistent fetal restlessness and instability that cannot be stabilized, purgatives may be used to prevent harm to the mother.
Regarding fetal restlessness, it can be caused by maternal illness affecting the fetus, or by fetal issues affecting the mother; neither situation allows for peace. If the mother is ill, treat the mother first, and the fetus will stabilize. If the fetus is diseased, treat the fetus first, and the mother will recover. Occasionally, mistaken ingestion of toxic substances, careless daily life, Qi obstruction, alternating chills and fever, or vaginal bleeding (Lou Tai) cause fetal restlessness. Treatment should follow the cause.
Formulas
**Tong Ling San** (Source: *Yi Xue Lei Zheng*) treats fetal restlessness around three months of pregnancy. Male and female Yin and Yang intersect, and Qi and Blood harmonize to form pregnancy. If vaginal bleeding and abdominal pain occur, it is likely due to long-term coldness of the uterus and deficiency of the Blood Sea, leading to miscarriage, which is more dangerous than normal delivery. Women who have suffered this can take *Tong Ling San* prophylactically to nourish the fetus.
*Ren Shen* (Ginseng Root), *Fu Zi* (Prepared Aconite Lateral Root), *Shu Di Huang* (Prepared Rehmannia Root), *Mu Xiang* (Aucklandiae Radix, baked), *Bai Zhu* (Atractylodis Macrocephalae Rhizoma), *Gan Jiang* (Dried Ginger), *Mu Dan Pi* (Moutan Cortex), *Bai Shao Yao* (White Peony Root), *Chen Pi* (Citri Reticulatae Pericarpium, white part removed), *Shu Yu* (Dioscoreae Rhizoma), *Huang Qi* (Astragali Radix, honey-fried), *Gan Cao* (Licorice Root, fried), *Xiong* (Ligustici Chuanxiong Rhizoma). All ingredients in equal parts, ground into fine powder. Take 2 qian per dose with one cup of water and 49 grains of glutinous rice, decoct to 80%, remove rice, and take warm. Twice daily. Avoid all toxic foods. Another version excludes *Ren Shen*.
**Bao Sheng Wan** (Source: *Wei Shi Fang*) treats improper care during pregnancy, labor-induced fetal restlessness, lower back and abdominal pain, placental obstruction, threatened abortion, intermittent lochia, uterine diseases preventing conception, failure to maintain pregnancy, emaciation and dryness preventing gestation, overdue pregnancy with weak movement, risk of miscarriage, premature rupture of membranes due to early惊吓, difficult labor due to dry placenta, malposition, extreme pain and confusion, prolonged labor with intrauterine fetal death, cold abdomen, blue-black lips, vomiting cold foam, postpartum blood stasis causing syncope, panting and sweating, incomplete expulsion of lochia, umbilical and abdominal pain, alternating chills and fever, or postpartum weakness, sallow complexion, thin body, palpitations, night sweats, poor appetite, progressing to puerperal fever. Take regularly in the eighth month to strengthen Qi, nourish fetus, facilitate smooth delivery, and moisten the womb for easier birth. Postpartum use nourishes Qi and Blood, harmonizes Yin and Yang, secures the exterior, strengthens organs, treats wind-deficiency, and eliminates chronic cold.
*Ma Ren* (Cannabis Seed, shelled, 1 liang), *Dang Gui* (Angelicae Sinensis Radix, half liang, slightly stir-fried), *Gan Jiang* (Dried Ginger, prepared), *Rou Gui* (Cinnamomi Cortex, bark removed), *Shi Hu* (Dendrobii Caulis, roots removed), *Qin Jiao* (Gentianae Macrophyllae Radix, remove eyes and closed-mouth ones, stir-fry until sweat appears), *Shi Gao* (Gypsum Fibrosum, ground to powder), *Huang Qin* (Scutellariae Radix), *Nuo Mi* (Glutinous Rice), *Bei Mu* (Fritillariae Bulbus), *Gan Cao* (Licorice Root, fried red), *Da Dou Huang Juan* (Sojae Semen Preparatum, each 1 qian). Grind into powder, make pills the size of quail eggs with refined honey. Dissolve one pill in warm wine and jujube soup before meals, or chew on an empty stomach. Although *Huang Qin* and *Ma Ren* are cold, they clear fetal heat and wind respectively, stabilizing the fetus. *Shen Ling Bai Zhu San* (Ginseng, Poria, and Atractylodes Macrocephala Powder) is also suitable. Another version adds *Pu Huang* (Typhae Pollen, 1 liang). (*Shen Ling Bai Zhu San* formula is found in the Stomach Cold category of the Great Formula Section).
**An Tai Yin**
*Da Fu Pi* (Arecae Pericarpium, washed), *Ren Shen* (Ginseng Root, rhizome removed), *Chen Pi* (Citri Reticulatae Pericarpium, white part removed), *Bai Shao Yao* (White Peony Root), *Dang Gui* (Angelicae Sinensis Radix, middle section), *Chuan Xiong* (Ligustici Chuanxiong Rhizoma, stir-fried), *Xiang Fu* (Cyperi Rhizoma, stir-fried), *Suo Sha* (Amomi Fructus Rotundus, stir-fried), *Zi Su* (Perillae Folium, stems and leaves), *Chi Fu Ling* (Poria with Red Hosta, black skin removed), *Gan Cao* (Licorice Root, fried, equal parts). Chop, decoct with 7 stalks of Juncus effusus and a pinch of glutinous rice. Drink. Also consume with pork and *Lu Sheng Gong San*. Take *Shen Gong San* to clear heat toxins. For threatened abortion, add *E Jiao* (Donkey-hide Gelatin) and *Bai Cao Shuang* (Ash of Hundred Herbs).
**Sang Ji Sheng Tang** (Mulberry Mistletoe Decoction) treats fetal restlessness during pregnancy.
*Sang Shang Ji Sheng* (Mulberry Mistletoe, chopped), *Dang Gui* (Angelicae Sinensis Radix, sliced and baked), *Chi Fu Ling* (Poria with Red Hosta, black skin removed), *Mu Tong* (Akebiae Caulis, chopped), *Sheng Gan Di Huang* (Rehmanniae Radix, dried and baked), *Chen Ju Pi* (Citri Reticulatae Pericarpium, white part removed and fried, 1 liang each), *Bai Zhu* (Atractylodis Macrocephalae Rhizoma), *Xiong* (Ligustici Chuanxiong Rhizoma, 1.5 liang each), *He Li Le* (Terminalia Chebulae Fructus, prepared, peel 1 liang), *Sha Cao Gen* (Cyperi Rotundi Rhizoma, hair removed, stir-fried, half liang), *Mu Xiang* (Aucklandiae Radix, 1 qian). Coarsely grind and sieve. Take 3 qian per dose with one cup of water and 2 slices of ginger, decoct to 60%, strain, and take warm. Three times daily.
**Jian An Tai Yin** (Modified Fetus-Stabilizing Decoction, Source: *Wei Shi Fang*) is for women who sleep during the day, feel tired of grooming, experience dry heaving, food aversion, and crave salty/sour tastes, indicating confirmed menstruation. Suitable for those with weak Qi and Blood. It strengthens the Spleen, stabilizes the fetus, warms and nourishes, balancing organ Qi and Yin-Yang to prevent miscarriage. For robust women conceiving without illness, warming formulas are inappropriate. This formula treats fetal restlessness, abdominal pain, and bleeding.
*Tiao Shen* (Codonopsis Radix, rhizome removed), *Nen Huang Qi* (Young Astragali Radix, rhizome removed), *Shao Yao* (Peony Root), *Da Chuan Xiong* (Large Ligustici Chuanxiong Rhizoma), *Shu Di Huang* (Prepared Rehmanniae Radix, wine-washed, sliced, baked), *Fen Cao* (Licorice Root), *Chuan Xu Duan* (Dipsaci Radix, rhizome removed), *Ce Bai Ye* (Biota Tops, stir-fried), *E Jiao* (Donkey-hide Gelatin, bran-fried), *Dang Gui* (Angelicae Sinensis Radix, tail removed, equal parts). Take 4 qian per dose with 1.5 cups of water and 3 slices of ginger, decoct with gold and silver. If bleeding persists, add a handful of *Shu Ai* (Mugwort Leaf, prepared). If fetal restlessness causes jaw locking, blue lips, and diarrhea, use 1 liang of *Shu Ai* boiled in 3 cups of wine down to 2 cups, strain, and administer.
**Sang Ji Sheng San** (Mulberry Mistletoe Powder, Source: *Dan Liao Fang*) treats fetal restlessness, continuous vaginal bleeding, threatened abortion, and abnormal uterine bleeding.
Answer: When the fetus forms but respiration is not solid, trauma from sex, shock, overexertion, falling from heights, or heavy pressure can injure the fetus. Ingestion of toxins can cause uterine laxity and dripping blood. Untreated, stagnant blood affects the Heart, endangering mother and fetus, leading to fetal drying and imminent death.
