Humans live between heaven and earth, inheriting Qi from Yin and Yang. When Qi is harmonious, there is health; when Qi is rebellious, disease occurs. Therefore, within a year, if the solar terms are disharmonious and cold and heat deviate from their proper seasons, all these form the Qi of epidemics (Yi Li). Contracting this pathogen causes disease, hence it is called epidemic. Its symptoms are generally similar regardless of age or stature, and it is commonly known as "Tian Xing" (Heavenly Transmission). This illness is similar to diseases such as seasonal Qi (Shi Qi) and warm-heat (Wen Re). Treatment varies according to the specific syndrome, using formulas tailored to the condition.
There is also a poem stating: "Epidemics in the four seasons are hard to predict; regulation for the old and young is generally the same. Summer ailments target the Heart, winter ailments target the Kidney; late summer targets the Spleen, autumn targets the Lung, and spring targets the Liver."
Another discussion states: For spring epidemics contracted with clear/cold Qi, use Shengma Gegen Tang (Cimicifugae et Puerariae Decoction) and Jie Ji Tang (Muscle-Releasing Decoction). For summer contraction of cold, use Lizhong Tang (Regulate the Middle Decoction), Shexiang Tang (Musk Decoction), and Banxia Guigan Tang (Pinellia, Cinnamon Twig, and Licorice Decoction). For autumn contraction of heat, use Baihu Jia Cangzhu Tang (White Tiger plus Atractylodes Decoction); for those presenting with jaundice, use Yinchin Wuling San (Artemisiae Scopariae and Five-Ingredient Powder with Poria). For winter contraction of warmth, use Ruiyu Tang (Jade Reed Decoction). For all four seasons, Baidu San (Deficiency-Resolving Powder) and Shengma Gegen Tang are universally applicable. Earth Qi has no fixed direction but is named due to Fire (referring to damp-heat or late summer); one should select prescriptions based on symptoms.
Formulas
Niuhuang Qibao Gao (Seven-Treasure Bull Gallstone Ointment): Treats Shanghan (Cold Damage) and seasonal epidemics, heat-toxin and summerheat diseases, chest and epigastric stuffiness, yellow jaundice with macules, mania, restlessness, confusion, and chronic accumulation of toxins causing tidal fever unresponsive to repeated purgation.
Ingredients: Niuhuang (Bovis Calculus, 5 fen), Longnao (Borneolum Syntheticum, 5 fen), Shexiang (Moschus, 5 fen), Zhusha (Cinnabaris, 2 qian 5 fen), Qingfen (Calomelas, >5 qian, all ground), Baifan (Alumen, 5 qian), Hanshuishi (Aqua Marina calcined to redness, 3 liang 5 qian).
Preparation and Administration: Grind all ingredients uniformly, mix with water drops to form 20 pills. Take one pill per dose, dissolved in clear water. Children take half a pill. Take at any time.
Shengsan Zi (Holy Scattered Seeds): Treats Shanghan, seasonal epidemics, Fengwen (Wind-Warmth), and Shiwen (Damp-Warmth). Applicable regardless of whether it is Yin-Yang dual infection or exterior-interior differentiation is unclear, or if there is exterior heat with interior cold, or exterior cold with interior heat. Symptoms include stiffness and spasm of the head, neck, waist, and spine; fever and aversion to cold; pain in limbs and joints; vomiting, regurgitation, wheezing, and coughing; nasal congestion and heavy voice; consumption of raw/cold food leading to epigastric fullness; abdominal and hypochondriac distension and pain; epigastric knotting and stuffiness; cold extremities; borborygmus and diarrhea; undigested food and water; spontaneous sweating; and difficult urination. Based on Mr. Dongpo's annotations, its efficacy is beyond description. Previously in Hangzhou and Huangzhou, during a severe epidemic where the whole city fell ill and many died critically, Shengsan Zi was used successfully regardless of Yin/Yang dual infection or gender. Even in critical cases, continuous doses led to sweating, free Qi flow, and recovery. The formula is slightly warm, yet brings a cooling sensation even to patients with Yang toxicity and mania. It defies common logic. During epidemics, anyone, old or young, rich or poor, drinking one cup prevents seasonal diseases. It is truly a treasure for saving lives and protecting families. Master Chao treasured this formula for generations, achieving near-perfect success rates. While I was demoted to Huangzhou, annual epidemics claimed many lives, but countless were saved by this formula. Master Chao kept it secret until he accidentally revealed it to Pang Jun of Qishui, a renowned physician and author, ensuring the formula's immortality alongside Master Chao. For Ye syndrome (Qi stagnation in the chest/diaphragm, desire to belch but unable to produce sound), Shengsan Zi is indicated. Add Shi Di (Kaki Calyx) to the decoction.
Ingredients: Houpo (Magnolia Bark, peeled, ginger-processed), Baizhu (Atractylodes Macrocephala Rhizome), Fangfeng (Saposhnikovia Root, de-rhizomed), Wuzhuyu (Evodia Fruit, washed 7 times), Zexie (Alisma Rhizome), Fuzi (Aconite Lateralis Radix Praeparata, roasted, peeled, de-navel), Bencao (Ligusticum Chuanxiong Rhizome? Note: Text says 'Ben', likely Ligusticum or Angelica Dahurica contextually, here translated as Ben), Gaoliangjiang (Galangal Rhizome), Zhuling (Polyporus), Huoxiang (Pogostemon Herba), Cangzhu (Atractylodes Lancea Rhizome, or Maozhu in some versions), Mahuang (Ephedra Herb, de-root/joint), Xixin (Asarum Root), Shaoyao (Paeonia Radix), Duhuo (Angelica Pubescens Root, de-rhizome), Banxia (Pinellia Tuber, washed 7 times, ginger-processed), Fuling (Poria), Chaihu (Bupleuri Radix), Zhike (Citrus Aurantium Fructus, de-albedo, bran-fried), Gancao (Licorice Root, roasted), Caodoukou Ren (Cardamomi Fructus Seed), Shichangpu (Acori Tatarinowii Rhizome).
Preparation and Administration: Grind into powder. Take 4 qian per dose with 1.5 cups of water, decoct to 1 cup, strain, and take hot. Take at any time until mild sweating occurs. For seasonal Qi disharmony, take on an empty stomach to dispel evil epidemics.
Xiangsu San (Fragrant Perilla Powder): Treats seasonal plague and Shanghan in adults and children initially contracting wind-cold, before Yin/Yang differentiation. Suitable for Shanghan, Shangfeng (Wind-Cold damage), Shangshi (Damp damage), and Shangshi (Food damage). Symptoms include headache, fever, aversion to cold, back/spine pain, chest fullness, vomiting, and invasion of internal/external pathogens.
Ingredients: Xiangfu (Cyperi Rhizoma, roasted, de-hair, 4 liang), Zisu (Perillae Folium, 4 liang), Gancao (Licorice Root, 1 liang), Chenpi (Citrus Reticulatae Pericarpium, white pelt retained, 2 liang).
Preparation and Administration: Grind into powder. Take 3 qian per dose with 1 cup of water, decoct to 0.7 cup, strain, and take hot. Three times daily. If made into fine powder, take 2 qian with salt. Some versions add Cangzhu and Chenxiang, named Chenxiang Yinzi. For headache, add Chuanxiong, Baizhi, Beixixin, Jingjie Sui (half qian each). For cough with heavy voice, profuse phlegm/thick mucus, add Banxia, Kujie (Bupleuri/Jiegeng?), Wumei (half qian each), Sangbaipi (7 cun). For heart pain, add Shigao, Banxia (half qian each). For diarrhea, add Mugua, Huoxiang (half qian each). For dampness with spontaneous sweating and acute diarrhea, add Shengjiang (3 slices), Cheqianzi (a pinch). Alternatively, roast Xiangfu and Chenpi (1 liang each) with wine in a stone mortar until yellow, grind, and decoct. For chronic dampness causing leg/back pain and difficulty walking, decoct with wine. For lingering dampness causing spleen pain, fatigue, abdominal distension, or rising Qi mimicking deficiency (but not true deficiency), this formula treats it. For excess accumulation with abdominal pain/diarrhea, add Congbai (2 roots), Wumei (1 fruit). For Hongxiangjiao (podagra/gout), add Binglang, Mugua, Dafupi, Zhike, Muxiang (small amounts), decoct with salt/wine/water, take on empty stomach. For wind contraction with undigested food, tidal chills, abdominal pain/diarrhea, aversion to wind, limb spasms, add Shengjiang (3 slices), Wumei (1 fruit), Congbai (2 roots). For malaria, add Taoliuzhi (7 cun), Xiebai (2 bulbs). For headache with nausea, add salt. For cold Qi in men/women, add Zhuyu (a pinch), salt. For women's blood Qi issues, add Ezhu, Huixiang, Wuyao, Danggui. Adjust additions based on deficiency/excess. Removing Xiangfu/Zisu and adding Houpo, Jiegeng, Bencao (4 liang each) creates Hejie San (Harmonizing Decoction), a mild formula. A white-haired elder gave this to a wealthy family who distributed it during an epidemic, curing all patients in the city.
Shengseng San (Holy Monk Powder, from *Experience Good Prescriptions*): Treats plague, seasonal Qi, and all Shanghan, regardless of severity.
Ingredients: XiangBaizhi (Angelica Dahurica Root, 2 liang), Gancao (Licorice Root, 5 qian, both raw).
Preparation and Administration: Grind into fine powder. Take 2 qian per dose with Shengjiang (3 slices), Zao (1 fruit), Congbai (3 cun), and 1 cup of water. Decoct to 0.8 cup, take hot. Cover to sweat for relief. Repeat after a while to correct Qi.
Guizhi Huangqin Tang (Cinnamon Twig and Scutellaria Decoction, from *Wei's Prescriptions*): Treats Fengyi (Wind Epidemic), floating rapid pulse, headache, neck/back pain, fever, and aversion to wind. Symptoms resemble Taiyang Shangfeng, except the Yin pulse is not weak, and transmission differs.
Ingredients: Guixin (Cinnamon Twig, peeled, 5 qian), Shaoyao (Paeonia Root, 5 qian), Huangqin (Scutellaria Root, 5 qian), Gancao (Licorice Root, 1 liang, roasted).
Preparation and Administration: Grind into powder. Take 5 qian per dose with 1.5 cups of water, Shengjiang (3 slices), Zao (1 fruit). Decoct to 0.8 cup, strain, take before meals.
Woxue Tang (Snow-Melting Decoction, from *Three Causes Formulas*): Treats Shanghan, Wenyi (Warm Epidemic), Shiwei (Damp Epidemic), Reyi (Heat Epidemic), and Shanlan Zhangqi (Mountain Miasma).
Ingredients: Cangzhu (Atractylodes Rhizome, 6 liang), Gange (Pueraria Root, roasted, 6 liang), Gancao (Licorice Root, roasted, 6 liang), Fangfeng (Saposhnikovia Root, 4 liang), Gange (Pueraria Root, 4 liang - *Note: Duplicate ingredient listed in source*), Houpo (Magnolia Bark, processed, 4 liang), Shaoyao (Paeonia Root, 4 liang).
Preparation and Administration: Grind into powder. Take 3.5 qian per dose with 2 cups of water, decoct to 0.7 cup, strain, and take.
Shiwei Xiongsu San (Ten-Ingredient Chuanxiong and Perilla Powder, from *Eternal Class Guide Formulas*): Treats seasonal plague and Shanghan with fever/headache. Suitable for adults/children initially contracting wind-cold.
Ingredients: Chuanxiong (Ligusticum Chuanxiong Rhizome), Zisu (Perillae Folium), Gange (Pueraria Root), Gancao (Licorice Root), Qianhu (Peucedani Radix, or Chaihu in some versions), Fuling (Poria), Banxia (Pinellia Tuber), Zhike (Citrus Aurantium Fructus), Jiegeng (Platycodon Root), Chenpi (Citrus Reticulatae Pericarpium). Equal parts.
Preparation and Administration: Cut into pieces. Take 3 qian per dose with 1 cup of water, Shengjiang, and Zao. Decoct, strain, take hot to induce sweating.
Shenzhu San (Divine Efficacy Powder, from *Yang's Family Collection*): Treats seasonal Shanghan/plague, headache, stiff neck, fever, aversion to cold, body pain. Also treats Shanghan with nasal congestion, cough, and dizziness.
Ingredients: Bencao (Ligusticum/Angelica, de-soil, 1 liang), Qianghuo (Notopterygium Rhizome, de-rhizome, 1 liang), Gancao (Licorice Root, roasted, 1 liang), XiangBaizhi (Angelica Dahurica Root, 1 liang), Xixin (Asarum Root, de-leaf/soil, 1 liang), Chuanxiong (Ligusticum Chuanxiong Rhizome, 1 liang), Cangzhu (Atractylodes Rhizome, 5 liang, soaked in rice water overnight, cut, roasted).
Preparation and Administration: Grind into powder. Take 3 qian per dose with 1 cup of water, Shengjiang (3 slices), Congbai (3 cun). Decoct to 0.7 cup, take warm. At any time. For wind-cold nasal congestion, take with Congcha (green tea). Variations exist without Bencao, or with different ratios of Cangzhu, Chuanxiong, etc.
Shenshou Taiyi San (One Name: Taiyi Shishen San): Treats irregular seasonal cold/warmth, rampant epidemics, and widespread disease. Applicable for Yin/Yang dual infection, wind-cold-damp bi, etc.
