Dysentery with pus and blood (a type of dysentery characterized by the discharge of pus and blood), with an attached theoretical discussion. This occurs because in spring, yang qi floats on the surface. When people engage in physical labor or exercise, their cou li (skin pores and muscle texture) open up. Those with deficient qi and blood are susceptible to wind pathogens. By summer, they are further attacked by heat qi. Blood naturally flows and scatters when exposed to heat. If the large intestine is weak at this time, blood seeps into the intestines, where it clashes with intestinal fluids. Accumulated heat condenses, transforming blood into pus. The weakness of the intestines leads to diarrhea, thus forming dysentery with pus and blood. Therefore, the reason for the prevalence of dysentery with pus and blood in summer is due to the weakness of the stomach and intestines.
During autumn and winter, if the spleen pulse is examined and found to be faint and choppy (wei se), it indicates severe ulceration with profuse discharge of pus and blood. If the pulse is suspended and absolute (xuan jue - extremely weak or irregular), death is imminent; a slippery and large pulse indicates survival; a minute and small pulse indicates survival; a solid and urgent pulse indicates death; a deep, fine, deficient, and slow pulse indicates survival; a rapid, frequent, large pulse accompanied by heat indicates death.
In adults and children, if diarrhea and dysentery persist without cessation, it may later transform into the discharge of white pus, dripping down or appearing at the end of stool. If warming the spleen fails to cure it, the treatment method should focus on warming the kidney. Since the kidney governs the bone marrow, white pus is essentially an alternative name for bone marrow pathology. In summary, when white dysentery first appears, it has a fishy odor. After mild purgative treatment, if the condition lingers for a long time, it is no longer considered typical dysentery. Current cases of white dysentery lack a fishy odor, present with a slightly dark complexion, emaciated bones and muscles, clearly indicating kidney deficiency. The appropriate formula consists of Pogu Zhi (Fructus Psoraleae), Dang Gui (Radix Angelicae Sinensis), Mu Xiang (Radix Aucklandiae), Gan Jiang (Rhizoma Zingiberis Recens), and Guan Gui (Cortex Cinnamomi). Subsequently, administer small amounts of Zhen Ling Dan (Spirit-Settling Pill) and Yu Hua Bai Dan (Jade-Flower White Pill) as adjuvants.
There is also a syndrome in women that resembles dysentery but is not dysentery, characterized by the discharge of white pus and sudden, severe pain in the heart and abdomen, with groaning sounds unbearable to hear. This requires promoting blood circulation using blood-invigorating agents, assisted by Yang Zheng Dan (Righteousness-Nourishing Pill).
