Edema and fullness during pregnancy arise from the inherent weakness of the Zang-Fu Qi, exacerbated by the profound deficiency following childbirth. When Earth fails to control Water, blood scatters into the four limbs, leading to abdominal distension and edema of the hands, feet, face, and eyes, accompanied by scanty and difficult urination. Chen Wuyue stated: In women who have a history of wind, cold, dampness, or cold-damp pathogens, pregnancy often presents with foot swelling, commonly known as "wrinkled feet" (Zhoujiao). There are also cases of generalized edema and fullness with urgent abdominal distension, termed "fetal water" (Taishui). Pregnant women should avoid extreme emotional agitation; if the mind is calm and Qi is harmonious, fetal Qi will be stable. If exposed to wind-cold pathogens or physical trauma, illness will follow the pathogen. During pregnancy, menstrual blood is retained to nourish the fetus. Sudden virtual edema indicates water retained within the fetus, where water and blood contend. The Spleen dislikes dampness and governs the muscles of the body. Dampness infiltration weakens Qi, causing muscular flaccidity and floating edema as water flows溢出 (overflows), resulting in generalized swelling. However, causes vary: some result from diarrhea or dysentery causing visceral laxity and damaging the Spleen and Stomach; others stem from cold-heat malaria, thirst, and excessive drinking, which infiltrate the Spleen and Stomach with dampness, causing edema of the head, eyes, hands, or feet. If water infiltrates the uterus before the fetus is formed, the fetus is likely damaged. At term, slight foot swelling after conception at three months, spreading from the dorsum of the feet to below the knees, making walking difficult, accompanied by panting, chest oppression, and poor appetite, resembles water-Qi disease. If yellow fluid oozes from between the toes, it is called "child Qi" (Ziqi), resolving only after delivery. This is due to pre-existing wind pathogens or blood-wind in the Chong and Ren meridians. Decoctions should not be administered blindly, especially in severe cases near delivery, to avoid exertion and unforeseen dangers; thus, treatment before delivery is essential. The Spleen corresponds to Earth and governs muscles. Harmonious Earth Qi controls Water, allowing normal transformation without accumulation. If pregnant women have Spleen-Stomach Qi deficiency and retained blood, water drinks fail to transform, dampness overflows, attacking the exterior form and interior uterus. Early pregnancy swelling may damage fetal Qi; late-pregnancy slight foot swelling can be cured by promoting urination.
Formulas
Mutong Tang (from *Shenghui Fang*), treating pregnancy body edema, chest/abdominal fullness, and urinary obstruction.
Ingredients: Mutong (cut), Xiangru, Sanggen Baipi (cut, each 1 liang), Muxiang, Helilepi, Huangqin (each 3 liang), Zhike (pulp removed, bran-fried), Binglang (each 0.5 liang), Zisu Jingye (0.5 liang).

Danggui Yin (from *Daquan Liangfang*), treating various antepartum disorders. Caused by anger filling the Zi Zang (uterus) or Pao Mai (bladder vessels), leading to urgent abdominal distension, periodic waist/abdominal pain, anorexia, limb edema, shortness of breath, sudden constipation, sudden dysuria, and sudden swelling of the Yumen (vaginal opening).
Ingredients: Danggui (1 liang), Chi Fuling, Zhike, Baishyaoyao, Chuanxiong (each 2 liang), Chuan Baijiang (processed), Muxiang (roasted), Fencao (each 0.5 liang).
Method: Cut into pieces. Take 3 da qian per dose, with 1.5 sheng of water and 3 slices of Shengjiang, decoct to 8 fen, remove dregs, take warm at any time. For those with weak constitution, use Bazheng San.
Bazheng San (from *Weishi Fang*), treating pregnancy heart-Qi stagnation affecting fetal Qi, fetal descent at eight months, painful limb edema, restlessness, and difficult labor.
Ingredients: Qumai (cut), Mutong (cut, skin and nodes removed), Huashi, Dahuang (cut, wrapped in flour and roasted), Cheqianzi, Bianxu (also known as Dibianzhu), Shansheng Jin San (Note: Text lists ingredients for Shansheng Jin San separately later, but here lists it under Bazheng San contextually or erroneously; extracting as listed).
Shansheng Jin San (from *Jisheng Fang*), addressing empty distension of the lower abdomen. Likely caused by hard food injuring the fetus, transmitting disease to the Spleen and Stomach, causing Qi deficiency and cold forcing the lower abdomen, resembling Ben Tun (running piglet) syndrome, or heavy waist, constipation, and hollow ringing in the hypochondrium. Take Shengjin San to warm the middle and descend Qi.
Ingredients: Wuzhuyu (branches/stems removed, wine-soaked and fried), Chenpi (peel removed), Ganshengjiang, Ganjiang (processed), Chuanxiong (each 1.5 qian), Houpo (skin removed, ginger-fried), Suosharen (fried), Gancao (roasted, each 3 qian).

Avoid peaches, plums, sparrow meat, and vinegar. Another version adds a little Chenpi. Case report: A pregnant woman with abdominal distension, dysuria, and vomiting was treated with various warming/breath-regulating formulas, worsening her condition with fetal death. Lyu Tang resolved the edema and expelled foul water, allowing delivery of the dead fetus, saving the mother's life. This highlights that large abdomens in pregnancy are often mistaken for normal pregnancy rather than Taishui disease; physicians must alert patients to self-monitor.
Fangji Tang (from *Shenghui Fang*), treating pregnancy generalized edema, rapid breathing, and dysuria.
