Guji (Bone Extremity) disease originates from Wind pathogen invading the Kidney. The Kidney governs the bone marrow of the body. When Wind pathogen invades the Zang organ, it damages the bones, thus forming Guji. What is referred to as Guji manifests as emaciation, severe toothache, vexing pain in the hands and feet, inability to stand for long periods, and reluctance to move when lying down. However, Guji can be characterized by extreme deficiency-cold or extreme excess-heat, both resulting from the Kidney receiving pathogenic factors. If Qi leans towards Yin, it leads to deficiency; deficiency generates cold. Cold causes facial edema, dirty black complexion, lower back and spine pain with inability to stand for long, and difficulty in flexion and extension. As Qi declines, hair falls out and teeth become brittle; the waist and back pull each other causing pain, and in severe cases, coughing and expectoration occur. If Qi leans towards Yang, it leads to excess; excess generates heat. Heat causes a dark and dull complexion, urinary bladder dysfunction (inability to urinate), severe pain in teeth and brain, emaciation of hands and feet, tinnitus, and blackish complexion. This represents the utmost severity of Guji. Treatment should follow the syndrome, employing tonification or purgation. For mild cases, treat early; if severe, it may lead to the exhaustion of Qi in the Foot-Shaoyin meridian.
Dihuang Jian Wan (Rehmannia Paste Pill): Treats Guji due to Kidney labor injury, manifesting as shortness of breath, insufficiency of energy, emaciation, weakness, and soreness/pain in limbs and joints. Suitable for consumption.

The above seven ingredients are boiled together in a silver or tin vessel until they form a paste. Then add the following herbs: Lujiao Jiao (Cervi Cornu Colla, 5 liang, crushed and stir-fried to yellow); Shihu (Dendrobii Caulis, 2 liang, roots removed); Fupenzi (Rubiae Fructus, 1 liang); Suanzaoren (Ziziphi Spinosae Semen, 2 liang); Roushoucong (Cistanche Deserticolae Herba, 1 liang, soaked in wine overnight, scraped clean, stir-fried dry); Renshen (Ginseng Radix, 2 liang, rhizome removed); Fuzi (Aconiti Lateralis Radix Praeparata, 2 liang, processed, skin and umbilicus removed); Niuxi (Achyranthis Bidentatae Radix, 2 liang); Baifuling (Poria, 2 liang); Wuweizi (Schisandrae Chinensis Fructus, 2 liang); Shu Dihuang (Prepared Rehmanniae Radix, 3 liang); Buguzhi (Psoraleae Fructus, 3 liang, stir-fried); Ganqi (Drynariae Rhizoma?
