Tianpao Chuang (Blistering Disease) presents with lesions resembling water blisters, characterized by thin and lustrous skin. These may appear on the head and face or spread over the entire body. This condition is caused by wind-heat affecting the Taiyin (Taiyin Meridian) and Yangming (Yangming Meridian) channels, thus manifesting in the skin, hair follicles, muscles, and flesh. Treatment should focus on clearing and cooling the blood; once the heat is resolved, the disease will heal. If accompanied by exterior pathogens with fever and a rapid pulse, Jingfang Baidu San (Jing Jie and Fang Feng Decoction for Resolving Toxins) is indicated. If fire pathogen is excessive, herbs such as Huang Qin (Scutellariae Radix), Lian Qiao (Forsythiae Fructus), Jin Yin Hua (Lonicerae Japonicae Flos), Xuan Shen (Scrophulariae Radix), and Lian Qiao (Forsythiae Fructus) may be added. If there is swelling and pain, a rapid pulse, and constipation, this indicates excess in both the interior and exterior; Fangfeng Tongsheng San (Saposhnikovia Powder for Unblocking and Safeguarding) should be used to resolve both layers simultaneously. If abundant toxic fluid oozes externally, Jinhuang San (Golden Yellow Powder) can be applied topically, ensuring recovery.
Xue Lizai stated: Chibai Youfeng (Red and White Wandering Wind) belongs to deficiency of Spleen and Lung Qi, with loose interstices (skin pores and texture). Wind-heat combats each other, or cold closes the interstices causing internal heat to stagnate. It may also result from internal movement of deficient fire invaded by external pathogens, or from Liver fire and blood heat caused by wind-heat. Therapeutic methods: For wind-heat, use Xiaochai Hu Tang (Minor Bupleurum Decoction) with added Fang Feng (Saposhnikoviae Radix) and Lian Qiao (Forsythiae Fructus). For blood heat, use Si Wu Tang (Four Substances Decoction) with added Chai Hu (Bupleuri Radix), Shan Zhi (Gardeniae Fructus), and Dan Pi (Moutan Cortex). For wind-heat combatting each other, use Jingfang Baidu San (Jing Jie and Fang Feng Decoction for Resolving Toxins). For internal heat and external cold, use Jia Wei Qiang Huo Tang (Modified Notopterygium Decoction). For weak Stomach Qi, use Buzhong Yiqi Tang (Decoction for Supplementing the Middle and Augmenting the Qi) with added Qiang Huo (Notopterygii Rhizoma et Radix) and Fang Feng (Saposhnikoviae Radix), along with Xiaofeng San (Wind-Expelling Powder). For blood deficiency, use Jia Wei Xiaoyao San (Modified Free and Easy Wanderer Powder). For Yin deficiency, use Xiaoyao San (Free and Easy Wanderer Powder) and Liu Wei Wan (Six-Ingredient Pill with Rehmannia). For Liver and Kidney deficient heat, use Liu Wei Wan (Six-Ingredient Pill with Rehmannia); then the fire will naturally subside, wind will settle, and itching will cease. If wind-expelling acrid-warm agents are used, Liver blood becomes drier, wind-fire intensifies, Yuan Qi (Original Qi) depletes further, interstices fail to close, wind lodges internally and spreads, Kidney Qi is injured, Mingmen Fire (Ministerial Fire) flares up, blood is consumed by fire, making it difficult to treat.
A man developed nodules in autumn; this was due to deficiency of Yuan Qi (Original Qi) and invasion by external pathogens. Initially, Jiwei Qianghuo Tang (Nine-Ingredient Notopterygium Decoction) was administered for two doses, followed by Buzhong Yiqi Tang (Decoction for Supplementing the Middle and Augmenting the Qi) with added Qiang Huo (Notopterygii Rhizoma et Radix) and Fang Feng (Saposhnikoviae Radix), resulting in cure. Later, due to improper daily routine, he experienced night sweats, afternoon fever, dry mouth, expectoration of phlegm, fatigue, and reluctance to speak. He was treated with Buzhong Yiqi Tang (Decoction for Supplementing the Middle and Augmenting the Qi) and Jiawei Bawei Wan (Modified Eight-Ingredient Pill), leading to recovery.
A woman presented with skin resembling Dandu (Erysipelas); scratching caused pus and fluid to ooze profusely, accompanied by fever, thirst, and dizziness, worsening in the late afternoon. She was cured using Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder).
A young woman exhibited red halos like rosy clouds, with itching and fever. She was cured using Jiawei Xiaochai Hu Tang (Modified Minor Bupleurum Decoction) with added Sheng Di Huang (Rehmanniae Radix), Lian Qiao (Forsythiae Fructus), and Dan Pi (Moutan Cortex). (All cases noted by Xue Ji)
