People of the world do not understand this pathogenesis and generally use acrid-warm dispersing medicines. Although they may achieve temporary effects, it is mistakenly considered a cold syndrome. Little do they know that the patient originally has stagnant heat within, and the interstices (pores) are often loose, so wind-cold easily enters. The external cold binds the internal heat, causing obstruction and rebellion of qi mechanism, leading to acute pain. Acrid-hot medicines, although capable of opening obstructions and dispersing superficial cold pathogens; however, using heat to treat heat will deepen the root of the disease and exacerbate aversion to cold. One should primarily employ fire-draining and blood-cooling methods, assisted by acrid-warm exterior-dispersing agents, treating according to correct principles, then the disease can be cured and its root eradicated. (*Za Zhuo*)
Miscellaneous discussions on treatment methods: Headache is often treated with Gypsum Fibrosum (Shigao), likely for its ability to sink phlegm-fluids. However, fearing its cold nature, Calcitrum (Zhongru Fen) is used as a substitute, which is particularly suitable for headache caused by Kidney Jue (Ren Jue). Some suspect Calcitrum is a refined alchemical drug, in which case Soft Gypsum (Ruan Shigao) ground into powder can be taken, yielding especially good results. (*Yi Jian*)
There are cases of lateral or frontal headache complicated by Brain Wind (Nao Feng), where Chuanwu (Aconiti Radix Lateralis Preparata) and Fuzi (Aconiti Radix Preparata) fail to cure, but Lianzi (Nelumbinis Semen) and Caowu (Aconiti Kusnezoffii Radix) succeed. This illustrates the principle of "using poison to attack poison," which must not be ignored. For headache due to upper jiao heat, Baidu San (Resist Toxin Powder) should be used, removing Chaihu (Bupleuri Radix) and adding Ganjuhua (Chrysanthemi Flos) in equal quantity. When Tou Feng (Head Wind) manifests with tightness and upward pulling pain in the tendons behind the vertex, one must assess whether cold or deficiency is involved; if so, Da San Wu Qi San (Great Three-Five-Seven Powder) is indicated. Treatments for Tou Feng often use hot medicines; occasionally, patients with heat complication cannot tolerate hot formulas, in which case Xiaofeng San (Eliminate Wind Powder), Tongguan San (Open Passages Powder) (see *Heji*), or Cha Tiao San (Tea-Pacifying Powder) and other clearing-upper-jiao formulas are appropriate. (*Yao Jue*)
Pian Tou Feng (Lateral Head Wind) also manifests one day before exposure to wind. Damp-phlegm and fire lurk in the head; even in summer, the patient desires to wrap their head. Yuebi Tang (Mahuang Decoction for Restricting the Vessel) with modifications is used. For dampness, add Cangzhu (Atractylodis Rhizoma) processed with rice water and Chuanwu processed with black soybeans; for fire, add Zhizi (Gardeniae Fructus) stir-fried with ginger juice; for left-sided pain, add Huangqin (Scutellariae Radix) processed with wine; for right-sided pain, add ginger juice and Shigao; for severe damp-heat with eye swelling, add Dahuang (Rhei Radix et Rhizoma) processed with wine; for evil wind, add Xixin (Asari Radix et Rhizoma), Chuanxiong (Chuanxiong Rhizoma), Fangfeng (Saposhnikoviae Radix), etc. (*Yi Tong*)
Li Dongyuan treats headache mostly with Jiuhuangqin (Scutellariae Radix processed with wine), Jiuhelian (Coptidis Rhizoma processed with wine), Jiuhubai (Phellodendri Cortex processed with wine) combined with wind-dispersing herbs, such as Qingkong Gao (Clear Empty Paste), Anshen San (Calming Spirit Powder), and Qingshang Xiehuo Tang (Clear Upper Drain Fire Decoction). However, he indiscriminately uses Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng, Shengma (Cimicifugae Rhizoma), Chaihu, Baizhi (Angelicae Dahuricae Radix), Manjingzi (Viticis Fructus), etc., lacking discipline. Students may learn from his intent. Yuanzhu Cha Tiao San is concise and usable. (*Jinkui Yi*)
For chronic Tou Feng pain, small amounts of Chuanxiong, Danggui (Angelicae Sinensis Radix), and Honghua (Carthami Flos) must be added. This is not only for treating wind but also for harmonizing blood and stopping pain. Fine tea (Xicha) best clears the upper jiao; for chronic wind-heat headache, it serves as an excellent guiding herb, commonly used by Li Dongyuan and Zhang Qianfu. (Ibid.)

Descending and Rebellious Formulas: Toutong Liuhuang Wan.
Liuhuang (Sulfur, 2 liang, finely ground), Xiaoshi (Natrii Sulfas, 1 liang).
Make water pills the size of a fingertip. Chew one with empty stomach using winter tea. My cousin suffered from Tou Feng for over 20 years, headache like splitting, unable to eat for days, hundred remedies failed. Physician Tian Zi saw him and said: "My old mother had this for decades, found one medicine that worked." I obtained ten pills, took one daily. After ten days, Zi asked: "What foods trigger your headache?" Answer: "Wine and fish are worst." Zi told him to eat fish and wine freely, saying: "After taking ten pills of this medicine, will you still have headache?" He ate as instructed, never relapsed, and was cured. Knowing Zi for years, he gave me this formula upon parting. Physician Huai from Chenzhou makes this pill the size of Sophora seeds, take 15 pills per dose. For summer heat confusion, take with ice-cold water, instantly refreshing. For cold injury, take with boiling mugwort water. (*Su Shen Liangfang*)

Grind uniformly to fine powder. Make pills with ginger juice and paste, size of Sophora seeds. Dry in shade. Take 30 pills per dose with ginger water or rice drink. (*Benshi Fang*) *Weisheng Shiquan* treats Kidney Jue headache, feeling like axe-chopping, unbearable. Remove Banxia from above formula, add equal parts Tian Nanxing (Arisaematis Rhizoma) (*Sanyin Fang* names it Tian Nanxing Wan). Yuzhen Wan: Liuhuang and Banxia have strong warming-descending power; Shigao and Xiaoshi have strong cold-purging power. When Yin qi rebels upward violently, treating with pure Yang causes rejection. Thus, Shigao and Xiaoshi serve as assistants to allow entry without conflict. Heixi Dan also reflects this intent. (*Jinkui Yi*)
Heixi Dan (Black Lead Pill) treats True Headache. Take 60-70 pills with salt water, before meals. (*Wan'an Fang* citing *Keyong Fang* Vol 12)

Liver Jue Headache: Longhui Wan + Ganjuhua, Lingyangjiao (Saiga Antelope Horn). Use for excess Qi and hard stool. For deficiency, use Shengdi (Rehmanniae Radix), Lingyangjiao, Ganjuhua, Maidong (Ophiopogonis Radix), etc., to nourish and clear, making Liver soft so Jue stops naturally. (*Jinkui Yi*) (Note: *Shifang Miaoyong* cites Longhui Wan: This method comes from Yu Jiayan's *Yuyi Cao* treating Mother of Official Wu Tian.)

(*Jinkui Yi*) Also for Blood Deficiency headache, remove Zhi mu, Bohe, Zhizi, Jingjie, Honghua from above formula, add Danggui, Baishao (Paeoniae Radix Alba), Chuanxiong, use Zhi Gancao.
Harmonizing Middle and Guiding Out Phlegm Formulas: Banxia Baizhu Tianma Tang treats Phlegm Jue headache, spinning vision/blackout, oppression/nausea, short breath, confused speech/mind, eyes dare not open, feeling like in wind/clouds, headache like splitting, body heavy as mountain, cold limbs.
