Hemorrhoidal bleeding originates directly from the large intestine; how could blood return to the large intestine without being expelled, yet instead slowly leak from a fistula opening? Therefore, in treating hemorrhoids, it is crucial to tonify and supplement early, benefiting Qi and preserving Yuan (Primordial) Qi. Bitter-cold herbs should not be used for internal administration, nor applied externally, nor used for douching or washing. Patients must strictly abstain from strong alcohol and sexual activity for one hundred days; under these conditions, there is no reason why wounds would fail to heal. If these taboos are not observed, even miraculous medicines will struggle to achieve efficacy. (*From "Yang Yi Da Quan"*)
Method for Widening the Intestines and Purging Heat: Formula for treating intestinal hemorrhoids with frequent bloody stools.
Cannabis Seed (Hemp Seed), Rhubarb, Aurantium Fructus (Bitter Orange Fruit, roasted).
Each ingredient two liang. Crush and sieve, mix with honey to form pills. Take ten pills daily on an empty stomach, washed down with Rapeseed Decoction. (*From "Qianjin Yue Ling"*)
Dang Gui Tang (Angelica Sinensis Decoction) for treating hemorrhoidal fistulas, reducing swelling and relieving pain. (*From "Sheng Ji Zong Lu"* [Note: This is Si Shun Qing Liang Yin, see formula in "Constipation"])
For treating wind-heat hemorrhoidal fistulas.
Rhubarb, Dang Gui (from Sichuan), Sophora Root (Kushen), Toothed Soap Pod (Ya Zao, peeled).
Grind equal high-quality parts into fine powder. Make pills with vinegar paste, size of Sophora seeds. Take twenty pills per dose, washed down with warm wine on an empty stomach. (*From "Ben Shi Xu Bian"*)
Generally, the method for treating hemorrhoids involves: if surgery (using caustic agents) is planned for tomorrow, administer this medicine today (decocted with Sophora Flower, Rhubarb, Aurantium Fructus, Akebia Caulis, Forsythia Fruit, Dianthus Herb, Dang Gui, half wine half water). This widens the large intestine, making stools soft and slippery, preventing obstruction by hemorrhoids, and avoiding diarrhea. If the hemorrhoidal head has not yet detached, it must be administered daily.
Take one liang of Rhubarb (roasted), Aurantium Fructus (stir-fried), and Dang Gui (wine-washed one liang). Grind together into fine powder, make pills the size of Sophora seeds, and swallow with good wine. (*From "Ben Cao Gang Mu", citing Mr. Zhou*)
A male patient suffered from hemorrhoids with swelling and pain, dry and hard stools, and a rapid, forceful pulse. After taking Nei Shu Huang Lian Tang (Internal Draining Coptis Decoction), bowel movement occurred after two doses, and pain ceased. Subsequently, Si Wu Tang (Four Substance Decoction) was modified with Scutellaria Root, Coptis Rhizome, Aurantium Fructus, and Trichosanthes Root. After several doses, the swelling subsided. Finally, one course of Zang Lian Wan (Intestine-Coated Pill, containing Coptis Rhizome and male pig large intestine) was taken, resulting in no recurrence. (*From "Wai Ke Zheng Zong"*)

This is a general formula for pain caused by blood stasis. The above seven ingredients are burned to ash retaining character. Then add the following.) Cinnamon Twig (three qian), Angelica Dahurica Root (five qian, none exposed to fire).
Grind nine ingredients into powder, make pills with vinegar paste, size of Sophora seeds. Take thirty pills on an empty stomach with rice drink. (*From "Pu Ji Fang/Da Cheng"*)

When Qi and Blood sink downward, clashing to form hemorrhoids, sitting is impossible, walking is intolerable. Standing worsens the sinking sensation. Avoid raw/cold foods, hard items, cold dishes, alcohol, wet noodles, five pungents, spicy/hot foods, strong spices, and dried ginger. Violating these renders treatment ineffective. (*From "Zhun Sheng"*)
Must abstain from alcohol and sex, avoid overwork, and eat bland food for best results. (*From "Yi Zong Jin Jian"*)
For hemorrhoidal fistulas, if one can appreciate the flavor of "no flavor," proper nourishment is sufficient; if one attends to "non-action," all matters are prepared. With proper medication and dietary regulation, and cautious adherence, healing is inevitable. Without caution, forced treatment yields no merit. (*From "Yang Yi Da Quan"*)
To treat hemorrhoidal fistulas and "Stork Mouth" lesions, where the patient cannot sit: Use a cushion made of dense reed flowers, with a hole in the center. Sit so the affected area aligns with the hole, avoiding pressure and injury to the wound. (Same source)
