Treat lower back and back pain using Du Huo Ji Sheng Tang. Lower back and back pain are all due to deficiency of Kidney Qi, lying in cold and damp places, and contracting wind pathogens. If not treated promptly, the pathogen tends to flow into the legs and knees, causing hemiplegia, cold bi-syndrome, weakness and pain, or lower back cramping and pain, heavy and numb feet. One should immediately take this formula.
Du Huo (three liang) Ji Sheng (Gu Jin Lu Yan uses Xu Duan) Du Zhong Niu Xi Xi Xin Qin Jiao Fu Ling Gui Xin Fang Feng Xiong Ren Shen Gan Cao Dang Gui Shao Yao Gan Di Huang (each two liang)
Chop the above fifteen ingredients, boil in ten dou of water to obtain three sheng, divide into three doses. Keep the body warm and avoid cold. If prone to deficiency-cold diarrhea, remove Gan Di Huang. After taking the decoction, roast Shuo Luo leaves over fire, place them thickly on the mat, lie on them while hot, re-roast when cold. In winter, use roots; in spring, use stems. Lying on them is best; other thin moxibustion methods are inferior to Shuo Luo steaming. This method is also used for wind-dampness in various parts of the body. (Zhou Hou Bei Ji Fang has one large Fu Zi, without Ji Sheng, Ren Shen, Gan Cao, Dang Gui.) (Qian Jin Fang)
Treat cold stagnation and wind attacking stabbing pain in the waist and body, use Du Huo San Formula.

Cut one dose, decoct with Sheng Jiang and take. (Jing Yue Quan Shu · Yi Jian) Fu Shi's Yi Xue Ji Yao calls it Ji Yao Tang. Ji Shi Quan Shu's Jia Wei Ru Shen San adds Rou Gui to this formula; if caused by emotional distress, add Mu Xiang; if caused by blood stasis pain, add Tao Ren and Hong Hua.
I once treated an old man surnamed Dong, over sixty years old, originally robust constitution. Because he liked drinking strong liquor, damp-heat gathered in the Tai Yang meridian. Suddenly, he suffered unbearable lower back pain, even wishing to commit suicide; his suffering was extreme. I diagnosed him and found his six pulses were extremely surging and slippery. His urination was blocked, and his bladder was distended and urgent. Thus, I used Da Li Qing Yin (containing Fu Ling, Ze Xie, Zhu Ling, Tong Cao, Zhi Zi, Che Qian Zi, Zhi Ke), doubling the dosage of Huang Bo and Long Dan Cao. After one dose, urination suddenly flowed freely, and once urination flowed, the lower back pain disappeared as if lost. (Jing Yue Quan Shu) (Note: Yi Sheng Wei Lun records: Fang Lu Ru suffered unbearable異-pain in waist and knees, Yang excess repelling Yin. Used Huang Bo, Long Dan, Huang Qin, Huang Lian, Zhi Zi, added Sheng Jiang, drank hot in one go. After three doses, pain vanished. Used Ren Shen Gu Ben Wan, recovered in one month. This aligns with Jing Yue's intent.)
Gan Dou Tang treats internal accumulation of wind-heat invading the Kidney causing lower back pain, with blockage of both stool and urine (formula see "Accumulated Heat" section). Add Xu Duan and Tian Ma, alternate with Bai Du San. (Zhi Zhi Fang)
Various Blood-Activating Prescriptions In the year Wu Yin, at Zhi Shan Temple medical service. A person suffered lower back pain for a year without cure. Taking Kidney-tonifying medicines made the pain gradually worse, leading to reduced appetite and emaciation. Various doctors deemed it incurable and asked me to diagnose. I saw his pulse was string-long and large, forceful upon pressure, with a hard and stifling pain in the lower abdomen. At that time, I gave him Tao Ren Cheng Qi Tang. His bowels subsequently moved, discharging half a bucket of black stool, and the pain stopped immediately. Subsequently, he took Tao Ren wine, and fully recovered in half a month. Whenever there is accumulated heat between the intestines and stomach, stagnant and unable to circulate, it can also cause pain. Now giving purgatives does not treat Kidney-deficiency lower back pain, but rather treats accumulated-heat lower back pain. (Yi Yue / Yi Yuan)
Zhang Ling Shi's younger brother had a damaged syndrome from Cold Damage. His two loins were bent and useless, lying in bed crying in pain all night. Hundred treatments failed. He sought my diagnosis. His pulse was smooth and normal, but the pain was greatly reduced compared to before. I said: This is not a fatal disease, but fear he will become disabled. The key to transferring this condition lies in the pain feeling like knife-stabbing, indicating a struggle between pathogenic and healthy qi. If completely painless, then pathogen and healthy qi merge, living in peace. Now that pain sensation is greatly reduced, it is truly worrying, and treatment should be swift. After deep thought, I reasoned that heat pathogen had deeply entered the two loins, blood vessels had been closed for a long time and could not emerge again. Only the method of attacking and dispersing remained. But since the pathogen had invaded for a long time, healthy qi was completely deficient, attacking would surely fail. So I used Tao Ren Cheng Qi Tang, adding large doses of Rou Gui and Fu Zi. After taking two large doses, he could stand up strongly. Then, following the previous idea, made pills. After taking for more than ten days, he was fully restored. This was not a tried-and-true method of ancients, but a temporary expedient. Using Fu Zi together with Da Huang, combined with Tao Ren and Rou Gui to dissipate blood knots in the waist. (Yu Yi Cao)
Li Xiao San treats lower back pain.
Yan Hu Suo Dang Gui Guan Gui
Equal parts, mix fine powder with wine, take two qian. (Other version of Zhong Zang Jing) Passed down by Ge Cheng, named Shu Jin San, treats blood vessel stagnation, tendon constriction, joint pain, difficulty walking. This medicine is the first-class herb for activating blood and transforming qi. (Shi Zhai Bai Yi Xuan Fang) Fu Ren Liang Fang names it Ru Shen Tang.

Grind the above into fine powder, take three qian each time, mix with warm wine, on an empty stomach. (Yang Shi Jia Cang Fang)

Grind the above into fine powder, boil with wine and wheat paste to form pills, size of Sophora japonica seeds. Take ten pills each time, swallow with warm wine. If ineffective, increase to thirty pills, take before meals. (Fang Shi Jia Cang Fang)
A friend suffered lower back pain unable to turn sideways. Six pulses were scattered and large; pressing heavily revealed string-small and long pulses. Took Kidney-tonifying medicines for half a year without effect. Someone suggested treating damp-heat. I believed it was evil blood (blood stasis). The patient asked: If there is evil blood inside, can we purge it? I said: Pulse is thin, cannot drive out yet. First use Gui Huang Yi Mu Tang to tonify it, then take Tao Ren wine. After ten doses of Yi Mu Tang, pain gradually decreased, diet improved. Then gave Tao Ren wine drink, and he was peaceful after one month. (Yi Yue)

Various Qi-Regulating Prescriptions Xiao Qi Xiang Wan treats depression, anger, worry, thought, or sprain/fall, all qi-stagnation lower back pain. Formula recorded in He Ji Ju Fang (Note: Uses Gan Song, Gan Cao, Xiang Fu Zi, Ding Xiang Pi, E Zhu, Sha Ren, Yi Zhi Ren). Split one packet into two packets. Ju Ren one qian, salt a little, water one zhong, decoct to seven fen, cool and take, on an empty stomach. (Ji Sheng Fang)

Various Interior-Draining Prescriptions Dou Men Fang treats lower back pain. Use Da Huang half liang, add Sheng Jiang half liang, cut together like soybean size, stir-fry in pan until yellow, pour in two bowls of water. Drink in one dose at early morning (wu geng). At dawn, discharge evil blood matter from waist, collect in basin, looks like chicken liver, then pain stops. (Ben Cao Gang Mu) (Also Tu Jing Ben Cao cites Cui Yuan Liang Hai Shang Fang, treats cold-wind in waist and feet. Fry Da Huang in butter into powder. Add two slices of Sheng Jiang like coin size to water, boil ten-plus boils, remove ginger. Take two qian Da Huang powder, mix with ginger soup, drink in one dose on empty stomach. Should discharge cold pus and evil matter, pain ceases.) San Yin Fang names it Shu Da Huang Tang, treats falling/spraining, lower back pain unable to bend/extend.

Grind above into fine powder, make pills with wine-wheat paste, size of Sophora japonica seeds. Take thirty pills each time, send down with salt soup, on an empty stomach or before meals. (Fang Shi Jia Cang Fang)

Dai Ren's (Zhang Cong Zheng) female servant came from the road in winter, face red as fire. Arriving near Yang Ling Quan, she suffered severe pain in waist and hips, tenesmus, seeing ghosts when in pain. Dai Ren said: This is Shaoyang Meridian disease, located on the side of the body, belonging to Minister Fire. Ordered her to take Zhou Che Wan and Tong Jing San, purged several basins, illness still not cured. People were strange, thinking it was possessed. Dai Ren angrily said: It's a donkey ghost. Again ordered Tiao Wei Cheng Qi Tang two liang, add Qian Niu Tou Mo one liang, decoct together and take. Great purgation (Note: Excessive purging) dozens of times, about one or two fan capacity, only then did she abandon her cane, only felt thirsty. Dai Ren allowed her to drink water, watermelon, pear, persimmon, etc. Dai Ren said: To treat fire, nothing is better than water; water is the supreme yin of heaven and earth.
Drank about one or two buckets of water, still felt slight pain. Dai Ren then punctured her Yang Ling Quan acupoint to extend the stagnation. Yang Ling Quan is an acupoint of Foot-Shaoyang Gallbladder Meridian. From then on, she was calm. The servant said herself: This illness requires purging five to seven times every year. This year she didn't purge, so it was like this. Chang Zhong Ming understood her words. Believing himself had damp disease, he also purged ten-plus times a year, and the illness finally ceased. This can be discussed with the wise, difficult to discuss with the foolish. (Shi Xing San Liao) (Note: Also records a man over sixty, suffering severe pain in waist, buttocks, spine, and hips. Pulse sunken, stagnant, firm, and tough. Stool hard and dry. Used Da Cheng Qi Tang, decocted with ginger and jujube, added Qian Niu Tou Mo two qian. Purged four to five times, all dry feces mixed with blood stasis and filth, pain removed.)
Du Huo Tang treats lower back pain like breaking, heavy as mountain, due to labor and fatigue.

Stir-fry together until hot, wrap in silk cloth, moxa/compress painful area. Cold, then stir-fry and compress again. (Bai Wen Da Yi Chao)
Moxibustion Method In August of Wu Xu year, Huainan had great floods, city submerged for months. I suddenly had organ dysfunction, belly roaring like water. After several days, regulated and healed. Since then, lower back pain unable to bend/straighten, even affecting face and cheeks. Took all medicines without effect, lasting three months. I later reflected: This must be water qi yin excess, Kidney meridian contracted this pathogen. Thus, I moxa'd Shenshu acupoint thirty-seven moxa cones. Took this medicine and recovered. (Ben Shi Fang) (Note: This medicine is Mi Rong Wan, using Mi Rong, Bo Hui Xiang, Tu Si Zi, Yang Shen.)
My younger brother had lower back pain, entering/exiting very difficult. I used fire needle to slightly and frequently prick Shenshu acupoint, then walking returned to normal. Initially did not moxa. Many people with curved spines came seeking point-moxibustion. I thought it was tendon disease, pointed to Yang Ling Quan for them to moxa themselves, and they recovered. Yang Ling Quan is the meeting point of tendons. However, lower back pain cannot stick solely to Shenshu acupoint, nor ignore other acupoints. (Zi Sheng Jing)
