Qi deficiency accompanied by cold and heat, food stagnation, wind pathogen, fright pathogen, damp-heat, sinking of Yang Qi, and phlegm accumulation. Treatment should be differentiated; diarrhea is mild while dysentery is severe.
**Elevating and Dispersing**: Weifeng Tang (Stomach Wind Decoction), Fangfeng Shaoyao Tang (Saposhnikovia and Peony Decoction), Shenzhu San (Divine Atractylodes Powder), Cangzhu Fangfeng Tang (Atractylodes and Saposhnikovia Decoction), and Baidu San (Decoction to Defeat Pathogenic Factors) can all be treated via sweating.
**Attacking the Interior**: For damp-heat, use Daoshui Wan (Water-Guiding Pill); if accompanied by constraint, use Chengqi Tang (Cathartic Decoction) or Hezhong Wan (Harmonizing the Middle Pill). For stagnation, use Shengbingzi (Holy Cake Pill) or Pi Ji Wan (Spleen Accumulation Pill). For cold stagnation, use Ju Fang Sugan Wan (Bureau Formula Su Gan Pill). After purgation, use medication for regulation. At the onset of dysentery, purgation is mandatory, employing "treating obstruction with unblocking" (Tong Yin Tong Yong). For severe damp-heat, use Xuanming Xuanqing Gao (Xuan Qing Ointment from Xuan Ming's Works).
**Harmonizing Qi and Blood**: Tenesmus and urgency are treated with Muxiang Binglang Wan (Wood Fragrance and Areca Nut Pill). White stool indicates Qi-level involvement; red and white mixed indicates both Qi and Blood levels are affected; red and black mixed indicates damp-heat; green/yellow mixed indicates wind combined with fire-dampness.
**Cooling Blood**: For bloody stools, cool the blood using Danggui (Angelicae Sinensis Radix) and [missing character]. Red indicates Blood level involvement. According to *Baoming Ji*, use Siwu Tang (Four Substances Decoction) plus Huaihua (Sophorae Flos), Huanglian (Coptidis Rhizoma), and Migke (Pericarpium Papaveris).
**Warming**: For a deep, weak pulse with abdominal pain, use Jiangfu Tang (Ginger and Aconite Decoction) combined with Wuling San (Five-Ingredient Poria Powder), Lizhong Wan (Regulate the Middle Pill), or Jiyao Jiangshui San (Jiang Shui Powder from *Jiyao*). Green color indicates cold combined with wind.
**Lifting**: For sinking of Yang Qi, use Shengyang Yiewei Tang (Sheng Yang Yi Wei Decoction) plus Jiegeng (Platycodonis Radix), and Nanxing processed with vinegar. External application of Yiye Mei (One-Leaf Plum) at the affected area is extremely effective; blistering indicates cessation of symptoms.
**Astringing**: Use Shichang San (Solidify Intestines Powder), Taohua Tang (Peach Blossom Decoction), Yangzang Tang (Nourish Viscera Decoction), Yan Shi Wumei Wan (Yan's Mume Pill), Yuanrong Wumei San (Yuanrong Mume Powder), such as Migke (Pericarpium Papaveris), Liupi (Pericarpium Granati), and Longgu (Os Draconis).
**Overcoming Dampness**: Use Pingwei San (Ping Wei Powder), [missing character], Quxiong Wan (Qu Xiong Pill), etc. In the elderly, excessive nourishment damages the Spleen through diet, causing Pi Xie (Spleen Diarrhea); treat with Jiyao Baizhu Shaoyao Tang (Baizhu Shaoyao Decoction from *Jiyao*) or Fuling Tang (Poria Decoction). For excess dampness, use Xianzhu (Atractylodis Rhizoma) stir-fried.
**Percolating and Draining**: When Yin and Yang are indistinguishable, use bland herbs to percolate and drain, such as Wuling San (Five-Ingredient Poria Powder) or Zhuling Tang (Polyporus Decoction). Alternatively, use only Yiyiren (Coicis Semen) stir-fried and powdered.
**Tonifying**: Do not tonify immediately in the early stage. For Qi deficiency, use Buzhong Yiqi Tang (Bu Zhong Yi Qi Decoction). For Blood deficiency with divine spirit weakness, use Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Danggui (Angelicae Sinensis Radix), Chuanxiong (Chuanxiong Rhizoma), fried Shaoyao (Paeoniae Radix Alba), and Fuling (Poria), with a small amount of Huanglian (Coptidis Rhizoma). Use Qian Shi Baizhu San (Qian's Atractylodes Powder) or Shiquan Dabu Tang (Complete and Great Tonifying Decoction).
**Purgative/Drastic Method**: For diarrhea/dysentery with a deep, hidden, fine, weak, and sinking pulse, use Laifu Dan (Laifu Pill) or Lingsha Dan (Lingsha Pill) to drastically purge pathogens.
**Clearing Summerheat**: For summerheat stroke with a deficient pulse, use Xiangru San (Elsholtziae Herba Powder), Guiling Ganlu Yin (Guiling Ganlu Drink), or Qingshu Yiqi Tang (Qing Shu Yi Qi Decoction).
**Moxibustion**: For sinking conditions, apply moxibustion. Points: Pishu (BL20), Guanyuan (CV4), Shenshu (BL23), Fuliu (KI7), Fu'ai (ST25), Changqiang (GV1), Taixi (KI3), Dachangshu (BL25), Zusanli (ST36), Qishe (ST10), Zhongwan (CV12).
**Resolving Heat**: For external heat, first resolve heat using Chaihu (Bupleuri Radix), Huangqin (Scutellariae Radix), and Huanglian (Coptidis Rhizoma). For internal heat, use Xianglian Wan (Xianglian Pill), Shaoyao Bapi Wan (Shaoyao Baipi Pill), or Huanglian Ejiao Wan (Huanglian Ejiao Pill). For Jin Kou Li (Dysentery with inability to eat), detoxify.
**Separating and Benefiting**: Use Weiling Tang (Weiling Decoction), combined with bland-draining herbs and Wuling San (Five-Ingredient Poria Powder). Or use alone [missing character] Xiang San.
**External Application**: Apply Yiye Mei (One-Leaf Plum) to Yintang (EX-HN3).
**Inducing Emesis**: If there is phlegm-fluid retention above the diaphragm, preventing Qi descent, leading to large intestine failure to consolidate and resulting in diarrhea/dysentery. If phlegm is in the Lung, to unblock the instability of the Large Intestine, induce vomiting.
**Umbilical Covering**: For Jin Kou Li (Dysentery with inability to eat), place a snail on the umbilicus to cover it, drawing heat downward. Add more Huanglian (Coptidis Rhizoma) with Renshen (Ginseng Radix et Rhizoma) and Baizhu (Atractylodis Macrocephalae Rhizoma), sip throughout the day. If vomiting occurs, administer again with Kidney Tonification.
**Tonifying Kidney**: For chronic Shen Xie (Kidney Diarrhea) with a deep, fine, and forceless pulse, use Ershen Wan (Ershen Pill), Ben Shi Wuweizi San (Ben Shi Schisandrae Fructus Powder), or Jin Suo Zhengyuan Dan (Jinsuo Zhengyuan Pill).
**Harmonizing Spleen Qi**: For disharmony of Spleen Qi with turbid Qi descending, which cannot heal, ask the patient what they crave to eat, provide it, and the Spleen Qi will harmonize, leading to recovery.
**Eliminating Accumulation**: For cases caused by accumulation, use Pi Ji Wan (Spleen Accumulation Pill), formula seen below.
**Ingredients**: Qingpi (Citri Reticulatae Pericarpium Viride), Chenpi (Citri Reticulatae Pericarpium), Sanleng (Samoli Rhizoma, fried), Pengzhu (Curcumae Rhizoma, roasted, each 2 qian), Shashi (Cyperi Rotundi Rhizoma, 2 liang, fried), Rouguo (Myristicae Semen), Shandou (Sophorae Tonkinensis Radix, fried), Shanzha (Crataegi Fructus), Xiangsharen (Amomi Fructus), Binglang (Arecae Semen), Jianghuang (Curcumae Longae Rhizoma), Houpo (Magnoliae Officinalis Cortex, fried, 2 qian), Huanglian (Coptidis Rhizoma, 3 qian), Muxiang (Aucklandiae Radix, 1.5 qian), Tanxiang (Santalum Album Lignum, 2 qian), Chengqie (Litseae Fructus, 1 qian), Baudoukou (Cardamomi Albi Fructus, 1 qian), Mai (Tritici Venetini Fructus, fried, 2 qian).
Grind all into powder, make pills with wax, size of mung beans. Take 20-30 pills per dose with warm water.
