A patient presented with fever and irritability. Siwu Tang (Four Substances Decoction), Huangqin (Scutellariae Radix), Honghua (Carthami Flos), Ruan Chaihu (Bupleuri Radix), Shanzhi (Gardeniae Fructus), and Huafen (Trichosanthis Radix) were administered. After the irritability and heat cleared, it was found that blood stasis was deeply accumulated. The intention was to needle out the stasis, but the patient refused. Suddenly, trismus (lockjaw) occurred, and the affected area suffered stabbing pain. Only after needling was performed to drain pus and blood did the condition stabilize. Tuoli Yangxue (Supporting the Interior and Nourishing Blood) methods were then used, and new flesh gradually grew. Suddenly, itching occurred at the affected site, which was attributed to Wind-Heat. A formula for dispelling Wind and detoxifying was administered, leading to recovery.
Another patient presented with distending pain in the buttocks and legs, accompanied by fever and irritability. Dead blood was pricked out, resulting in slight relief of the distending pain, but the fever and irritability worsened. This was due to blood loss causing an excess of pathogenic fire. Dushen Tang (Single Ginseng Decoction) was urgently administered to tonify; symptoms improved slightly. Subsequently, herbs to strengthen the Spleen and Stomach and nourish Qi and Blood were used. As the necrotic tissue gradually sloughed off, the patient recovered. Generally, this type of condition requires preliminary tonification to support consolidation. One must absolutely avoid attacking Qi and Blood or failing to tonify, lest healing be impeded.
A patient injured his hand in mid-summer and developed opisthotonos (back arching) and trismus. The pulse was floating and scattered, indicating a Exterior Syndrome. One dose of Qianghuo Fangfeng Tang (Notopterygii and Saposhnikoviae Decoction) resolved the condition. If this syndrome had occurred in autumn or winter when the pores are tight, herbs like Mahuang (Ephedrae Herba) would have been required to induce sweating. This represents the treatment method for acute trauma where Qi and Blood are not yet depleted.
Another patient suffered a slight break in the skin from a beating but developed this condition. The pulse was surging and forceful, indicating an Interior Syndrome. Da Xionghuang Tang (Major Angelica and Rhubarb Decoction) was administered. After one dose, there was a slight bowel movement, and all symptoms subsided. Administering Exterior-releasing drugs would have been fatal. It is crucial to urgently differentiate between Exterior/Interior and Deficiency/Excess to treat correctly and avoid error.
A patient presented with alternating chills and fever and dry mouth. Siwu Tang (Four Substances Decoction), Renshen (Ginseng Radix), [missing character], Baizhu (Atractylodis Macrocephalae Rhizoma), Ruan Chaihu (Bupleuri Radix), Chao Huangqin (Fried Scutellariae Radix), Maimendong (Ophiopogonis Radix), and Wuweizi (Schisandrae Fructus) were prescribed. After four doses, symptoms slightly receded. I intended to use Bian (stone therapy) to remove blood stasis, but the patient refused. Later, palpitations, insomnia, poor appetite, trismus, headache, eye pain, aversion to cold, and fever developed. This indicated internal accumulation of pus. Bian therapy was used to drain it, bringing immediate relief. Bazhen Tang (Eight Treasures Decoction), Zaoren (Ziziphi Spinosomi Semen), Maimendong (Ophiopogonis Radix), and Wuweizi (Schisandrae Fructus) were administered for twenty doses, gradually resolving previous symptoms. Previous herbs combined with Dushen Tang (Single Ginseng Decoction) were used as necrotic tissue gradually sloughed off. Later, due to overwork, insomnia and night sweats recurred. Guipi Tang (Restore the Spleen Decoction), Maimendong (Ophiopogonis Radix), Wuweizi (Schisandrae Fructus), and Yuanzhi (Polygalae Radix) led to recovery. Subsequently, the patient experienced distension and stuffiness in the jaw, redness of the face and eyes, resembling Lockjaw (Tetanus). However, it was still considered a Deficiency pattern. Bazhen Tang (Eight Treasures Decoction) and other herbs were used, resulting in stability.
