A patient presented with abdominal distension, panting, thirst, alternating chills and fever, and erosion of the buttocks and thighs where necrotic flesh mixed with pus, resembling a leather bag filled with paste. I prescribed Siwu Tang (Four Substances Decoction) decocted with child's urine (Tongbian), along with Tao Ren (Peach Kernel), Hong Hua (Safflower), Chai Hu (Bupleurum Root), Huang Qin (Scutellaria Root), Mai Men Dong (Ophiopogon Tuber), and Hua Fen (Trichosanthes Root). The symptoms subsided rapidly after administration. The patient subsequently applied black sheepskin to the affected area, which worsened the condition. Later, much pus and blood were drained using bian stone therapy. The patient's breath became faint, lips and mouth twitched slightly, jaw was locked, and the affected area appeared dark purple. Some suggested using herbs for Po Shang Feng (Broken Wind/Tetanus). I stated: This is a transformation syndrome due to deficiency of Qi and Blood. I used Ren Shen (Ginseng Root), [missing character in text, likely Huang Qi/Astragalus], Xiong (Chuanxiong Rhizome), Gui (Angelica Sinensis Root), and Bai Zhu (Atractylodes Macrocephala Rhizome), combined with Dushen Tang (Single Ginseng Decoction) added with cow's milk. After Yuan Qi (Original Qi) recovered, all symptoms resolved, and the patient stabilized following regulation with Shiquan Dabu Tang (Perfect Golden Great Tonifying Decoction). If pus and stasis are retained internally in such cases, acupuncture should be employed. If after ulceration, trismus (locked jaw) and enuresis occur, resembling Po Shang Feng or other syndromes, it indicates extreme deficiency of Qi and Blood; urgent use of strong tonifying agents is required. For patients with chronic phlegm suffering from wind syndromes, clearing phlegm and descending fire is appropriate. For those presenting with wind-like symptoms triggered by anger, clearing the Liver and descending fire is indicated. If a patient engages in excessive sexual activity and suddenly develops these symptoms, it stems from insufficiency of Kidney Water and blazing Heart Fire; treatment should focus on nourishing Yin and tonifying Qi and Blood. Misdiagnosing this as a wind syndrome and treating accordingly will result in death. (Note: "Po Shang Feng" here refers to Spasm/Jing).
Another patient had an internal ulcer that yielded three to five bowls of pus upon needle drainage. Large tonifying agents were administered immediately. The next day, high fever and sweating occurred, accompanied by cold feet, trismus, and opisthotonos (backward arching of the back). The physicians intended to administer dispersing agents. I stated: This is a transformation into Jing (Spasm) due to extreme deficiency of Qi and Blood; mistaking it for a Wind syndrome would be an error. The patient was cured using Shiquan Dabu Tang and similar formulas. Such cases often arise from excessive sweating or purgation during Shang Han (Cold Damage) diseases, or from Qi and Blood depletion due to ulcers in postpartum women. The proper approach is solely to regulate and tonify Qi and Blood. Administering harsh draining or attacking agents often leads to untreatable conditions.
Another patient had been ill for over two months with unhealed sores. Anger triggered a spasm and bleeding from the sores. This was caused by anger stirring up Liver Fire. I treated the patient with Chai Hu (Bupleurum Root), Huang Qin (Scutellaria Root), Lian (Coptis Rhizome), Shan Zhi (Gardenia Fruit), Fang Feng (Saposhnikovia Root), Jie Geng (Platycodon Root), Tian Ma (Gastrodia Rhizome), Gou Teng (Uncaria Ramulus cum Uncis), and Gan Cao (Licorice Root), resulting in immediate recovery. Mr. Liu Zongnong stated: Some cases are due to internal heat from Wind-Fire, some arise from emotional anger among the Seven Emotions, and others result from internal excess of Damp-Heat and obstruction of meridians by Phlegm-Saliva. Treatment should exclusively focus on tonifying deficiency and descending fire, strengthening Earth to calm Wood, and clearing Phlegm while removing Dampness.
