For the treatment of acute convulsive disease (Ji Jing Feng), dosage should be adjusted according to the condition, with the goal of achieving bowel movement as the limit. This formula is also extremely effective for treating "hanging intestine" (Diao Chang), "locked belly" (Suo Du), and "drawn mouth" (Cuo Kou), with unparalleled efficacy. The pills are made the size of hemp seeds: infants three days old take three pills; five days old, five pills; seven days old, seven pills. Adding Qingdai (Indigo Naturalis) creates the "Qingdai Wan".
Tian Nan Xing (Arisaema Cum Bile, prepared), Tian Ma (Gastrodiae Rhizoma), Bai Fu Zi (Typhonii Rhizoma, prepared, each 1 fen), Ni Fen (Calomel, half a fen in weight), Ba Shuang (Crotonis Fructus Extract, one zi amount), Wu Yi (Chloranthi Fructus, stir-fried), Quan Xie (Scorpio, flour-stir-fried), Hua Shi (Talcum, each 1.5 fen).
Process the above ingredients into fine powder. Use thin wheat starch paste boiled in water to form pills the size of large sesame seeds. Dosage: one-year-old infants take five pills; two-year-olds take ten pills. Adjust up or down based on age. Administer with Bohe Tang (Mint Decoction) or weak tea.
Discussion: This formula is used to purge wind from convulsions and clear phlegm-heat. One must first administer formulas to intercept wind and stop spasms, then use this purgative method. It is crucial not to cause excessive diarrhea; merely ensuring bowel movement is sufficient. One should inquire whether previous physicians have already administered purgatives, lest ignorance by the family lead to errors. If I were the primary physician, I would follow the symptoms to intercept wind and stabilize spasms first. If phlegm-heat remains, it is appropriate to purge with this formula to prevent the development of wind disorders. Moreover, when children develop wind disorders due to fright, one must understand that heat exists within the zang organs beforehand. Knowing there is internal heat prevents panic. Excessive heat depletes Heart Qi; sudden fright disturbs the Heart spirit, causing Heart Qi to scatter, resulting in a cyan face and white lips. After a long time, the fright qi subsides. Alternatively, if Liver Qi is deficient, fright qi enters the Liver; if Lung Qi is deficient, it enters the Lung. The five zang and six fu organs all allow fright qi to enter through areas of deficiency, thereby causing disease. As stated in the preceding song, the "urgency" of acute convulsive disease arises from phlegm-heat triggering each other, disturbing mental state, causing chaotic qi and pulse, and heating the meridians. Extreme heat generates wind. If wind-heat does not dissipate, tendons become tense and vessels contract, leading to spasms, rigidity, or traction, varying by severity. Skilled physicians intercept wind and stop spasms first; if heat and phlegm exist, they are purged downward. Consequently, spasms cease, clenching relaxes, tension loosens, and wind-heat dissipates. How could one worry about failure to recover consciousness and mental clarity? These words are simple yet profound; virtuous physicians should agree.
For infantile acute convulsive disease presenting with spasms, high fever, and flooding saliva, purgation is indicated.
Tian Nan Xing (Arisaema Cum Bile, four pieces, prepared, peeled, and powdered), Ba Dou (Crotonis Fructus, four pieces, processed until like frost), Da Ban Xia (Pinelliae Rhizoma, ten pieces, steamed in Licorice water, sliced, dried, and powdered), Bai Jiang Can (Bombyx Batryticatus, deflossed, stir-fried, 1 fen), Jin Xie (Scorpio, stir-fried, 2 fen, tail removed).
Mix uniformly. Each dose is approximately one zi amount, taken with Yinjinbohe Tang (Silver Flower and Mint Decoction).
