Consulting various medical formularies, one finds that these matters are not exhaustively recorded. This is because the disease arises solely from the five Zang organs being affected, transforming from Yin pathogens into accumulation (accumulation of pathological products). The Spleen governs food digestion; consuming cold/raw fruits, sweet substances, or heat-toxic agents leads to non-digestion in the Zhongwan (Epigastrium), which over time becomes a disease, hence named "Ji" (Accumulation). Accumulation disorders vary in severity and present in many forms with diverse symptoms, making it impossible to list them all. Generally, children with Ji-Re (Accumulation Heat) invariably exhibit a hot abdomen and cold feet. To assess severity, one estimates based on the degree of abdominal heat: some cases show 70% distension and 50% heat, while others show 30-50% distension and 70-80% heat. In severe cases, both hypochondriac regions become distended, tight, and full. Given these symptoms, one must carefully examine the clinical presentation before administering medication to avoid misdiagnosis.

It is argued that long-standing Ji diseases transmit and transform into Swelling (Edema/Distension). The swelling conditions in children listed below all stem from Ji-Du (Toxic Accumulation); if untreated, they gradually evolve into specific syndromes, differing according to their severity. Children who consume meat too early invariably develop accumulation; due to undigested accumulation, they invariably develop Gan Syndrome (Infantile Malnutrition); overeating invariably causes Qi Distension; Qi Distension contracting Dampness invariably leads to Edema. Deficient puffiness of the head, face, hands, and feet indicates Dampness in the Spleen; regulating the Spleen and eliminating Dampness yields immediate recovery. However, if the abdomen is distended and swollen, taut and urgent, leading to panting, this is described as empty Qi entering the abdomen. Compared to acute difficulties, moving the Zang-Fu organs is difficult; thus, it must not be treated lightly. One must investigate the deficiency or excess of Ying (Nutritive Qi) and Wei (Defensive Qi). If deficiency reaches a certain degree, one should first regulate the Middle Jiao to consolidate the solid state before employing draining methods. If regulation fails to restore solidity, it results in a deteriorated syndrome or collapse; under such circumstances, how can one rashly employ purgative or moving therapies? Such actions are not only futile but may cause deep harm.

The two conditions of Swelling and Distension arise from deficiency containing accumulation. Long-term neglect allows gradual transformation into syndromes with diverse transmissions. Treatment should be guided by assessing deficiency vs. excess, severity, flourishing vs. declining states, and exterior vs. interior aspects. The primary principle is to first consolidate the Root (Ben) and then correct the Branch (Biao), ensuring safety and recovery.

Swelling:
Dampness-induced Swelling (puffiness of hands, feet, face, and eyes); Food-Toxin/Qi-Vapor Swelling (abdominal, renal, and gu-type distension urgency); Cold Damage Empty-Qi Entering Abdomen Swelling; Diarrhea/Disentery Empty-Qi Entering Abdomen Swelling; Qi-Deficiency Swelling; Blood-Deficiency Swelling; Ying-Wei Dual-Deficiency Swelling.

Distension:
Gan-Qi Distension; Extreme Gan Distension; Gan-Accumulation Distension; Qi-Accumulation Distension; Pi-Qi Distension; Pi-Xie Distension; Zheng-Ji Distension; Suodu (Lock-Belly) Urgent Distension; Upper Ge (Diaphragm) Distension; Zhongwan Distension; Food-Injury Expansion Distension; Roundworm Distension; Spleen-Qi Distension; Cold-Accumulation Distension; Deficient-Accumulation Distension.

For the above Swelling, Distension, and Deficient Accumulation, purgative methods should generally be employed, though each requires specific pharmacological principles.
