Fanwei (Reversal of the Stomach) is described by four main theories: Qi deficiency, Blood deficiency, Phlegm-dampness, and Heat. This condition mostly stems from Spleen-Stomach deficiency. When Fire moves and Phlegm rises, food cannot be retained; a weak pulse on the right side indicates Qi deficiency, while a weak pulse on the left side indicates Blood deficiency. If the Cun (inch) and Guan (bar) pulses are deep and choppy, treatment should address Qi stagnation; if the Cun and Guan pulses are floating and large or deep, it is due to rising Phlegm causing vomiting and hiccups. Combine Sizijunzi Tang (Four Gentlemen Decoction) with Erchen Tang (Two Aged Decoction), combining Ginseng Radix et Rhizoma (Renshen), Atractylodis Macrocephalae Rhizoma (Baizhu), Poria (Fuling), and Pinelliae Rhizoma preparatum (Banxia). Add Licorice Root (Gancao), Gardenia Fruit (Zhizi), Coptis Rhizome (Huanglian), Angelica Sinensis Radix (Danggui), Immature Bitter Orange (Zhishi), Aucklandiae Radix (Muxiang), and Medicated Leaven (Shenqu) together with roasted Ginger (Hujiang). Decoct with Bamboo Sap (Zhuli) and Ginger Juice (Jiangzhi); one dose should yield results. For the elderly, whose Qi and Blood are already deficient, caution is advised when treating this condition in young people. If stomach heat causes dryness leading to red and difficult urination/defecation, differentiation must be detailed. If saliva flows continuously from the mouth and stools resemble sheep droppings, the prognosis is fatal.
Emergency single prescription for Fanwei: Ma Bo'er (burned to ash preserving its nature) 2 qian, good date flesh, and Pingwei San (Pingwei Powder) each 2 qian. Mix evenly, take with warm wine after meals.
Another prescription: Dry thousand-petal hibiscus flowers (Qianye Bai Jin Hua) in the shade, grind into powder, take with aged rice soup. If no improvement after three to five sips, take again. (Leaves can be used if flowers are unavailable).
Another prescription: Lead(II) Oxide (Huangdan, finely ground) and small dates (pitted and ground), form pills the size of a chestnut. Take one pill each time, burn to ash using a needle over a lamp, and take with milk.

Remaining Discussion
Fanwei and Ge-Ye differ in name but share the same principle. Ye refers to sudden obstruction when eating due to flaming Fire rising. Ge refers to Qi stagnation and Phlegm binding; initially, there is a sensation of obstruction in the throat, unable to vomit or swallow, eventually hindering food intake. Fanwei is caused by Phlegm-Fire knotting in the upper Jiao of the Stomach, causing immediate vomiting after eating; if knotting occurs in the lower Jiao, morning food is vomited in the evening. All result from Yang Fire failing to descend and instead ascending. This is due to worry, thought, grief, fear, anger, cold, heat, labor, and diet stirring up Yang Qi, accumulating into Ge-Heat. Initially, body fluids do not flow, appetite is lost, or belching smells foul with acid regurgitation, discomfort and fullness occur. Misdiagnosed as Cold, patients take acrid-aromatic drying-hot agents. Temporary relief is felt, but emotional fluctuations deepen the old disease. Without a skilled physician, further drying drugs are used, causing Phlegm to bind with Stasis Blood, forming nests/cysts, accelerating the formation of Fanwei. Good practitioners treat potential disease before it manifests. One should study the Spleen-Stomach gates daily, observe various Qi gates carefully, and medicate when acid regurgitation appears to avoid such progression. Dai Shi said: Abundant Phlegm with slippery pulse, use Erchen Tang as main formula; severe Heat with rapid pulse, use detoxification as main; Blood deficiency with rapid and weak pulse, use Siwu Tang as main; Qi deficiency with slow and weak pulse, use Sizijunzi Tang as main. Both Qi and Blood deficient with deep and choppy pulse, and saliva flowing from mouth, is untreatable. Because when Qi and Blood are not yet deficient, using drugs to rob Phlegm prevents Fire disease from recurring. When Qi and Blood are deficient, robbing Phlegm may temporarily suppress Fire, but the disease will recur. This is because Qi deficiency fails to transform and generate Phlegm, while Blood deficiency fails to moisten and generate Fire. Gong Shi advocated clearing Phlegm and descending Fire, using Erchen Tang plus Ginger Juice, Chive Juice, Bamboo Sap, Child's Urine, and Cow's Milk as main. For heat and oppression, add stir-fried Scutellaria Root (Huangqin), Coptis Rhizome (Huanglian), Trichosanthes Fruit (Gualou), Platycodon Root (Jiegeng); for Qi choking, add Cyperus Rhizome (Xiangfu), [missing char]xiong, Aucklandiae Radix (Muxiang), Areca Seed (Binglang), Amomum Fruit (Sharen); for Qi deficiency [missing char] fat and white complexion, combine with Sizijunzi Tang, use Bamboo Sap, Ginger Juice; for Blood deficiency and weak Phlegm, combine with Siwu Tang, add Carthamus Flower (Honghua), Apricot Kernel (Xingren, ground into paste), use Child's Urine, Chive Juice; for dry constipation, use Wine-steamed Rhubarb (Dahuang) to moisten; for Spleen failure to digest, add Medicated Leaven (Shenqu), Malt (Maiya) to assist; if approaching Fanwei, taking much Cow's Milk is the best strategy, but Human Milk should not be used, as it may contain Fire from diet, cooking, lust, and fatigue. Zhang Jifeng said: Ye is caused by excessive mental exertion; only inner contemplation and self-nourishment can bring peace. How true these words are. Do not take acrid-drying drugs again, beware, beware.
