The *Daquan* records: Heart pain during pregnancy is caused by the intermingling of wind pathogens and phlegm-fluids. If the primary meridian of the Heart is damaged, it is called "True Heart Pain" (Zhen Xin Tong); if onset occurs in the morning, death follows in the evening, and if onset occurs in the evening, death follows in the morning. If the collateral vessels of the Heart are damaged, the pain comes and goes intermittently. If the uterus (Zi Zang) is damaged, it leads to fetal movement and vaginal bleeding.
Xue Ji stated: For the aforementioned conditions, if caused by dietary injury, use Pingwei San (Calm the Stomach Powder) with added Zhike (Aurantii Fructus) and Shanzha (Crataegi Fructus). If caused by a complex of various pathogens, one must examine the etiology and treat accordingly.
In one case, a pregnant woman suffered from heart pain accompanied by vexation, heat, and thirst; she recovered after taking Baizhu San (Atractylodis Macrocephalae Powder). Later, due to food stagnation, the pain recurred with fullness and distension in the chest and abdomen, which was aggravated by pressure. This was due to dietary stagnation, and she was treated with Renshen Yangwei Tang (Ginseng Decoction for Nourishing the Stomach). In another instance where there was no pain upon pressure, indicating injury to the Spleen and Stomach, she recovered after being supplemented with Liujunzi Tang (Six Gentlemen Decoction).
In another case, a pregnant woman experienced pain in the heart and abdomen, distension in the chest and hypochondrium, acid regurgitation, and anorexia. This was due to Qi stagnation in the Liver and Spleen. She recovered after treatment with Erchen Tang (Two-Aged Decoction) plus Shanzha (Crataegi Fructus), Shanzhi (Gardeniae Fructus), Qingpi (Aurantii Fructus Pericarpium), and Muxiang (Aucklandiae Radix). Subsequently, due to anger, the pain recurred with fetal restlessness and anorexia, and her complexion was greenish-yellow. Her Liver pulse was wiry and tight, and her Spleen pulse was wiry and long. This indicated Liver Wood overacting on Earth (Spleen/Stomach). She recovered after treatment with Liujunzi Tang (Six Gentlemen Decoction) plus Shengma (Cimicifugae Rhizoma), Chaihu (Bupleuri Radix), and Muxiang (Aucklandiae Radix).
