Xue said: If there is heat in the bladder (Pang Guang), Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) should be used; if there is deficiency of Qi in the Spleen and Lung, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with Yi Zhi Ren (Alpinia oxyphylla Fructus) should be used; if there is Yin deficiency of the Liver and Kidney, Liuwei Wan (Six-Ingredient Pill with Rehmannia) should be used.
A pregnant woman presented with enuresis (involuntary urination) and internal heat. Her Liver pulse was surging and rapid but weak upon deep pressure. She occasionally experienced pain in both temporal regions and distension in the hypochondrium. I diagnosed this as Liver Fire with Blood deficiency. I administered Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Liuhuang Dihuang Wan (Six-Ingredient Rehmannia Pill). She recovered soon after. Later, she experienced alternating chills and fever, or recurrences of previous symptoms triggered by anger. I prescribed Bazhen San (Eight-Treasure Powder) and Xiaoyao San (Free and Easy Wanderer Powder), combined with methods to clear Liver Fire and nourish Liver Blood, leading to a complete cure.
Baiwei San (White Anemarrhena Powder): Treats urinary incontinence during pregnancy.
Ingredients: Bai Wei (Anemarrhenae Rhizoma), Bai Shao Yao (White Peony Root) (equal parts).
Preparation and Administration: Grind into powder. Take 3 qian per dose, mixed with warm wine before meals. (The Materia Medica states that white Peony Root has tonifying properties while red Peony Root has draining properties, because Metal nature is astringent and Fire nature is dispersing. Therefore, red Peony Root is often used in formulas for painful urinary dysfunction due to damp-heat [Zilin], whereas white Peony Root is used in formulas for enuresis.)
Sangpiaoxiao San (Mantis Egg-case Powder): Treats urinary incontinence during pregnancy.
Ingredients: Sang Piao Xiao (Mantis Egg-case) (stir-fried until yellow, 20 pieces).
Preparation and Administration: Grind into powder. Take 2 qian per dose, taken with rice water on an empty stomach.
Another prescription:
Ingredients: Bai Fan (Alumen), Mu Li (Ostreae Concha).
Preparation and Administration: Grind into powder. Take 2 qian per dose, mixed with wine.
Another prescription: Grind Yi Zhi Ren (Alpinia oxyphylla Fructus) into powder and take with rice water; also effective.
Daquan states: Pregnant women may suffer from meridian damage due to overexertion, with heat residing internally. Heat affects the Blood; when Blood is heated, it flows and overflows, seeping into the Zi Paog (uterine bladder), causing hematuria. (Note: Those who present with hematuria generally come from cases of mental overwork and depletion of essence.)
Xue said: For the aforementioned condition caused by anger stirring up Fire, Xiao Chaihu Tang (Minor Bupleurum Decoction) with Shan Zhi (Gardeniae Fructus) should be used; for that caused by labor stirring up Fire, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) should be used; for that caused by accumulation of heat from rich and greasy foods, Qingwei San (Clear the Stomach Powder) (see Miscellaneous Diseases, Dental Section) with Xi Jiao (Rhinocerotis Cornu), Lian Qiao (Forsythiae Fructus), and Gan Cao (Licorice Root) should be used; for that caused by Blood heat in the Liver Meridian, Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) should be used.
A pregnant woman presented with hematuria due to anger, accompanied by internal heat, thirst, alternating chills and fever, distension in the chest and breasts, and poor appetite. Her Liver pulse was wiry and weak. This indicated Liver Blood deficiency with heat. I administered Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Liuhuang Dihuang Wan (Six-Ingredient Rehmannia Pill). She gradually improved with concurrent use. Subsequently, she was treated with Bazhen Tang (Eight-Treasure Decoction) added with Chai Hu (Bupleuri Radix), Dan Pi (Moutan Cortex), and Shan Zhi (Gardeniae Fructus), resulting in a cure.
Xuduan Tang (Dipsacus Decoction): Treats lower bleeding and hematuria during pregnancy.
Ingredients: Dang Gui (Angelicae Sinensis Radix), Sheng Di Huang (Rehmanniae Radix) (each 1 liang); Xu Duan (Dipsaci Radix) (0.5 liang); Chi Shao Yao (Red Peony Root) (2.5 qian). (The variation in dosage serves as a reference.)
Preparation and Administration: Grind into powder. Take 2 qian per dose, mixed with decoction of green onion whites on an empty stomach.
Another prescription: Treats hematuria during pregnancy.
Ingredients: E jiao (Donkey-hide Gelatin) (stir-fried), Shu Di Huang (Prepared Rehmanniae Radix) (equal parts).
Preparation and Administration: Grind into fine powder. Take 2 qian per dose with porridge water on an empty stomach.
Jiang Mi Tang (Ginger and Honey Decoction): Treats hematuria in urine during pregnancy.
Ingredients: Sheng Jiang (Fresh Ginger Rhizome) (7 slices); Mi (Honey) (0.5 zhan); Bai Mao Gen (Imperatae Rhizoma) (one handful).
Preparation and Administration: Add water and decoct strongly; then drink.
Jiajian Wuling San (Modified Five-Ingredient Powder with Poria)
Remove Gui Zhi (Cinnamomi Ramulus) from the original formula, add stir-fried E jiao (Donkey-hide Gelatin), grind together into coarse powder. Take 4 qian per dose, decocted strongly with Che Qian Zi (Plantaginis Semen) and Bai Mao Gen (Imperatae Rhizoma), and take warm.
Miscellaneous Prescriptions (Properties vary among warming, cooling, slippery, and purgative; selection depends on individual preference)
One prescription: Use Kui Zi (Abutilonis Semen) 1 sheng, grind finely, boil in 5 sheng of water until reduced to 2 sheng. Divide into three warm doses.
Another prescription: Use fresh Ai Ye (Artemisiae Argyi Folium) 1 jin, boil in 5 sheng of wine until reduced to 2 sheng. Divide into three doses.
