When latent pathogens reside in the Shaoyin (Lesser Yin) channel, those that emerge outward via the channel qi reach the Three Yang channels; those that transform into heat and obstruct inwardly knot in the Stomach fu-organ: this is the normal pattern of warm-heat diseases. The Shaoyin system connects upward to the Lung. When pathogenic heat reverses upward from the Kidney system to the Lung, Lung disorders manifest. Moreover, as warm pathogens transform into heat, Fire inevitably overacts on Metal (the Lung belongs to Metal); thus, the Lung organ is inherently a target for warm pathogens. Alternatively, if heat obstructs in the Stomach and steams upward to the Diaphragm, pathogenic heat spreads from the Stomach to involve the Lung, which is also a natural progression of the disease. In recent times, with the prevalence of tobacco smoking, the fluids in the Lung are scorched into phlegm, obstructing the Lung passages. Patients often suffer from chronic phlegm and cough. When further subjected to steaming warm-heat, the phlegm becomes more sticky due to heat, and heat lingers longer by adhering to phlegm; consequently, cough and wheezing are inevitable. If the Lung collaterals are blocked, stabbing pain in the chest and hypochondrium occurs; as heat stagnation intensifies daily, sputum becomes foul and purulent, or hemoptysis (coughing up blood) appears, all caused by heat scorching and injuring Metal. At this stage, if the latent pathogen has fully exteriorized, treatment requires only clearing and draining heat from the Lung and Stomach. Why must treatment targeting the Lung also address the Stomach? Because the heat in the Lung entirely stems from steaming upward from the Stomach fu-organ. Clearing the Lung without first clearing the Stomach fails to cut off the source of heat, which is not the method of "drawing firewood from under the cauldron" (removing the root cause). Ancient formulas such as Mahuang Xingren Gancao Shigao Decoction (Ephedra, Apricot Seed, Licorice, and Gypsum Decoction), Yuespi Decoction, Qinglong Decoction, and Qingzao Jiufei Decoction (Dryness-Rescuing Lung-Decoction) all utilize Shigao (Gypsum Fibrosum), demonstrating insight into this principle. For mild cases, Weijing Decoction (Asparagus Twig Decoction), along with fresh Dihu (Dendrobii Caulis) and fresh Shashen (Glehniae Radix), are indispensable. For cases with stabbing pain in the hypochondrium, harmonizing collateral qi is added; for cases with hemoptysis, clearing the blood collaterals is added. Greasy/nourishing drugs are contraindicated as they may promote phlegm; warm/drying herbs are contraindicated as they may exacerbate heat. Furthermore, at the onset of this condition, physicians who are careless and fail to examine properly may mistake it for ordinary external contraction, recklessly using diaphoretic (sweating-inducing) methods. Or, seeing abundant phlegm, they might wrongly use Erchen Decoction (Two-Aged Decoction). Or, observing wheezing and reverse qi flow, they might treat it as an external contraction using Mahuang (Ephedrae Herba) and Guizhi (Cinnamomi Ramulus), or treat it as internal injury using Shengmai San (Generate the Pulse Powder) and Shudi Huang (Rehmanniae Radix Preparata). All these approaches are erroneous. They consume body fluids, assist heat, and generate phlegm, resulting in various complications. Only when the disease worsens day by day do they carefully reassess and switch to clearing and draining methods. However, the Yin of the Lung metal is already damaged and cannot recover quickly. Even if pathogenic heat is cleared, persistent dry cough due to internal heat may linger unresolved, eventually developing into Shang Sun (Upper Consumption/Deficiency Syndrome), leading to incurable outcomes. Such cases are common; whose fault is this?
