The Palace of Qian (Qian Trigram, corresponding to the Northwest) presents with smallpox that is red; there are many lesions on the heart, back, and mouth, and they are dense on the four limbs. ("Nong" means numerous.) If there is a single pustule at the Wei Lu point (BL54), and it is not punctured with a needle to drain the toxin, death will ensue.
If scabs do not form within ten days, the patient will die suddenly during Chen hour (07:00-09:00) and Wu hour (11:00-13:00). The Palace of Kan (Kan Trigram, corresponding to the North) presents with smallpox mixed with measles; there will be a dangerous pustule piercing through at the Wei Zhong point (BL40). If the lesions in the Palace of Qian are black, the patient will pass away on the sixth day during Chou hour (01:00-03:00) and Hai hour (21:00-23:00). The lesions in the Palace of Gen (Gen Trigram, corresponding to the Northeast) are not connected; there are none in the epigastrium, only on both sides. Scabs complete by the ninth day, full by the tenth; this type occurs due to sufficient Qi and Blood. Lesions in the Palace of Zhen (Zhen Trigram, corresponding to the East) have scorched tips; there is a cluster on the back. Only one-third of the chest and abdomen have lesions, while the four limbs are as dense as silkworm eggs. By the ninth day, the patient dies like a dream (Nanke Yimeng), and parents wail to heaven. Lesions in the Palace of Xun (Xun Trigram, corresponding to the Southeast) are dense without branching; the chest and back are uniformly covered, erupting all at once. ("Yongchu" means erupting in large numbers.) Even if the patient survives ten days up to fourteen, they will die in a dream during Yin hour (03:00-05:00) and Shen hour (15:00-17:00). Lesions in the Palace of Li (Li Trigram, corresponding to the South) appear in pairs linked together. A dangerous pustule at the Cheng Shan point (BL57) indicates critical condition; (the pustule must be picked out early, do not delay.) Three or four pustules on the chest are tragic if treatment is delayed by even one day. Lesions in the Palace of Kun (Kun Trigram, corresponding to the Southwest) are red and sparse; the number on the four limbs does not seem high; there are about thirty on the chest and back; it is considered unusual for the area in front of the heart to be clear. Lesions in the Palace of Dui (Dui Trigram, corresponding to the West) appear in a continuous string; upon careful consideration, it is pitiable. The patient passes away after nine days amidst itching and collapse, definitely dying during Zi hour (23:00-01:00) and Hai hour. Generally, across the Eight Trigrams, the limbs are crowded. ("Mi er duo" means dense and numerous.) Toxins in the abdomen and throat are internal, hidden in the skin and drilling inward. ("Quanbi" means completely closed.) If the condition turns auspicious within five days, (swelling and distinct grains are auspicious signs.) By the eighth day, the course is clear; fifteen days is the limit. (If it does not turn, life cannot be saved by the eighth day at the latest.)
The Rhyme for the Head and Face Diagram states: The secrets of smallpox and rashes require specialized study; exterior and interior, deficiency and excess must be clearly distinguished. First observe the various areas of the face to foresee good fortune, bad luck, disaster, and safety. The left side corresponds to the Liver and the right to the Lung, requiring careful deliberation; the Nian Shou and Ren Zhong areas belong to the Spleen. The Yin Tang area, being square and broad, is governed by the Heart; the Cheekbones, Kao bone, and ears correspond equally to the Kidney. The Wen Wu Shuang Tai is a true wonderful secret, found on the pearls of the left and right ears. The San Tai is located only on the Zhun Tou (tip of the nose); carefully examine the Eight Trigrams. The method of observing smallpox discusses the Five Meridians; the acupoints of the Five Meridians must be clear. Once clear, carefully observe the Eight Trigrams, and within the Eight Trigrams, determine according to the Five Meridians (Heart, Liver, Spleen, Lung, Kidney). The Heart and Kidney originally do not harmonize; when the Spleen and Lung produce smallpox, they are balanced. Only the Liver and Kidney produce mixed smallpox; calculating the Lung and Kidney, two or three out of ten can be saved. The occurrence of Shaoyang meridian smallpox is hard to grasp; there is vitality between the Nian Shou area. If Yin Tang initially appears purple, the patient collapses like a dream on the sixth day. Only the Liver meridian shows no major changes; combining these four is feasible. The Five Elements generate and overcome each other; do not let Water and Fire contend. Upper and lower resonance should be the main principle; smallpox and carbuncles originally do not follow normal patterns. I have secret words in my heart; using them legally turns life back. Use the secret of revival often in your heart, keep it in mind always. If medication does not go smoothly, employ clever strategies; administering medicine is like using troops. First look at the Tian Ting and Tai Yang areas; second, look at the lips, mouth, and tip of the tongue; third, look above and below the nasal bridge; fourth, look at the throat, forehead, and Tai Yang. Fifth, observe the Qi color of all Yang meridians, then look at the surrounding Eight Trigram positions. First, exert effort to observe shape and color; second, ensure Water and Fire do not invade each other. Third, ensure the Qi color is not withered; fourth, ensure grain and food intake is regular. Fifth, ensure the smallpox peaks are not sunken; sixth, distinguish the truth from miscellaneous symptoms. A string of Kidney meridian lesions reaching Tai Yang indicates change on the sixth day, leaving no room for negotiation. If there are several grains at the Shan Gen, damage is predicted by the twelfth day after the ninth. Dense Liver meridian lesions with internal redness are not afraid of black sunken areas in the Tian Ting; diarrhea on the ninth day causes no great harm, but death comes from the tenth to the twelfth day. Grayish-white Liver meridian lesions, pale red-white Taiyin and Taiyang lesions: throat obstruction on the seventh day, itching on the ninth; strengthening the Spleen and assisting the Stomach is appropriate. Densely packed Heart meridian lesions without gaps, pearl-like red patches on both cheeks: beware of itching around the eighth and ninth days; in standard patterns, pus formation is difficult. Lung meridian lesions connecting to the Zhen palace: only fear phlegm and saliva on the eighth day; ominous on the eleventh, auspicious on the thirteenth; passing these ominous days brings no subsequent disaster. Lung meridian lesions reaching the Li pass: grayish-white like dust with frost inside; if heat boils and rots open revealing frost, any sight of it is desolate. Carefully examine the acupoints at the root, then check if yellow pus forms on the lips; if there is vitality in these two places, peace can still be preserved. In this world, nothing is more heartbreaking than smallpox; even heroes panic.
