Tian Gen Dou (Heaven Root Pox): When all pox lesions fail to rise and become robust, but those on the Tian Ting (Forehead) or Xiao Xing (Star Point) do rise and suppurate, it indicates that essence and qi are generating outwardly. This is called "Tian Gen," and nine out of ten cases will survive.

Tian Kong Dou (Heaven Void Pox): When all pox lesions rise and become robust, but those on the Tian Ting (Forehead) or Xiao Xing (Star Point) fail to do so, it indicates that blood fails to irrigate the head. This is called "Tian Kong," and none of the ten cases will survive.

Ming Lang Dou (Bright and Clear Pox): When all pox lesions fail to rise, but those in the Tai Yin (Lesser Yin) and Tai Yang (Greater Yang) regions rise independently, resembling the brightness of the sun and moon, this is a auspicious sign.

Ming Shi Dou (Eclipsed Brightness Pox): When all pox lesions rise and become robust, but those in the Tai Yin (Lesser Yin) and Tai Yang (Greater Yang) regions fail to do so, resembling an eclipse of the sun and moon, this is an ominous sign.

Hai Yi Dou (Sea Overflow Pox): When all pox lesions fail to rise, but those in a circular area one inch behind the ear rise independently, named "Xing Xiu Hai Yi" (Star Mansion Sea Overflow), this indicates prosperity of the Kidney Meridian and is treatable.

Hai Ku Dou (Sea Withered Pox): When all pox lesions rise, but those in a circular area one inch behind the ear fail to rise independently, named "Xing Xiu Hai Ku" (Star Mansion Sea Withered), this indicates failure of the Kidney Meridian and is untreatable.

You Gen Dou (Rooted Pox): When pox lesions are dense over the entire body and face, which is extremely dangerous, if there are a few pearl-like lesions in the Di Ge (Chin/Jaw) area, all ten cases will survive. Since the Kidney is the root of human life, these lesions indicate strong Kidney Qi, hence "Rooted." Some say having pox on the soles of the feet signifies being rooted.

Wu Gen Dou (Unrooted Pox): When other pox lesions are fine, but the Di Ge (Chin/Jaw) area is depressed, dry, or grayish-white and non-rising, this indicates exhaustion of Kidney Water and is untreatable.

Rao Xi Dou (Knee-Encircling Pox): The knee patella region is where marrow converges and belongs to the Kidney. If the lesions are red, vibrant, and swollen, it is auspicious; if they are scorched purple, one must guard against them becoming Ding Sores (carbuncles).

Bao Bi Dou (Nose-Hugging Pox): Facial lesions are sparse, but those around the nasal bridge are dense like silkworm eggs, indicating toxin accumulation in the Spleen and Stomach. Named "Bao Bi Dou," the condition is critical. However, if the color and form are normal and other areas are smooth, recovery is possible.

Dan Suo Kou (Single Lock Mouth): Facial lesions are sparse, with one large black lesion at the corner of the mouth. This is "Dan Suo Kou." If lesions form a continuous ring around the lips, it is called "She Suo Kou" (Snake Lock Mouth), both are malignant diseases.

Shuang Suo Kou (Double Lock Mouth): One lesion at each corner of the mouth. This is "Shuang Suo Kou," also known as "Bai Hu Xu Chuang" (White Tiger Beard Sore), a particularly malignant disease.

Suo Xiang Tuo Yi Dou (Neck-Locking Chin-Supporting Pox): Also called Pan She (Coiling Snake), Tuo Sai (Cheek-Supporting), or Chan Hou (Throat-Wrapping). Lesions are sparse on the body but densely clustered from the neck to the chin. This is an ominous sign.

Jie Jing Dou (Neck-Knotting Pox): Excessive lesions in the throat and neck hollows. One must urgently administer herbs such as Xuanshen (Scrophulariae Radix), Jiegeng (Platycodonis Radix), Shengdi (Rehmanniae Radix), Gancao (Glycyrrhizae Radix et Rhizoma), Niubangzi (Arctii Fructus), and Shandougen (Sophorae Tonkinensis Radix). Delay allows toxin to knot in the throat, causing death.

Ju Dou (Clustered Pox): Lesions are numerous in the center but sparse or absent at the ends. This is a major ominous sign, often leading to internal attack on toxins, damaging the Heart and Lungs, and causing death.

Liang Tou Dou (Two-Ends Pox): Also called Liang Jie Dou (Two-Cut Pox) or Chun Shui Duan Qiao (Spring Water Broken Bridge). At the initial stage, lesions appear above the chest and below the navel, but the middle section is completely empty. This is due to excessive toxin congestion separating Qi and Blood, preventing their intersection. Yang Qi rises to the upper part, resulting in many lesions on the head and face; Yin Qi stagnates in the lower part, resulting in dense lesions on the legs and feet, while the chest and abdomen have few. This signifies separation of Yin and Yang. If lesions are red, vibrant, with round halos at the base, urgently use formulas to greatly tonify Qi and Blood and reconnect Yuan Qi. Rescue within seven days is ideal; after seven days, it is difficult to save. Only those with sufficient Zheng Qi (Upright Qi) are unaffected. If there are too many on the head/face or density on shoulders/back, death ensues; do not ignore it just because the chest/heart area is sparse.

Ni Dou (Reverse Pox): Few lesions on the upper body and many on the lower body. This causes little harm. Alternatively, lesions appearing from bottom to top, or resembling younger siblings/daughters instead of older brothers/sisters, are considered reverse pox.

Ya Ling Dou (Crow Feather Pox): This occurs when Tian Yuan (Heavenly Origin) is sufficient and Ren-Gui (Kidney Water) is full, with the Kidney Meridian uniquely manifesting. Named "Jing Zhao Yun Qu" (Whale Covering Cloud Sky). From spotting to swelling, lesions turn black, round, smooth, with high peaks and clear rings. As pus forms, they gradually turn yellow and become spirited. This predicts great wealth and honor later in life and is recorded in Zhuangyuan (Top Scholar) illustrations.

Gui Nie Dou (Ghost Pinching Pox): No individual lesions on the body; instead, patches form like bruise marks. Named "Gui Nie," this is absolutely untreatable.

Gui Dou (Ghost Pox): Spotting is complete, with many lesions on the body but none on the head/face. Also called Wu Tou Dou (Headless Pox). It can be saved within five days; beyond five days, it is difficult to save, as Qi and Blood descend and cannot ascend. Urgently use Chuanxiong (Ligustici Chuanxiong Rhizoma), Shengma (Cimicifugae Rhizoma), Ganjie (Licorice and Platycodon), Fangfeng (Saposhnikoviae Radix), Danggui (Angelicae Sinensis Radix), and Jiangcan (Bombyx Batryticatus). If not treated urgently, Du (Gallbladder Meridian) acupoint Quchi develops toxicity, leading to bone exposure and death within a month.

Zei Dou (Thief Pox): Initially, lesions are bright red, bean-sized, growing larger daily. They rise and swell first, then turn white-rooted. The base and apex lack blood color or appear golden-yellow. Though swollen, they feel soft upon pressure. Urgently puncture them; otherwise, bleeding occurs within four to five days, leading to death. Deep red lesions that feel soft under the skin steal systemic Qi and Blood, facilitating rapid pus formation, hence "Thief Pox." They grow faster than other pox. After three days, they may become blisters, even purple or black ones. Large, black, hard lesions resembling shells are Ding Sores (carbuncles). Puncture with a silver needle, rinse with water, suck out foul blood, and fill with Zicao Gao (Arnebiae Unguent) or You Yanzhi (Oil Carmine) mixed with Xueyu Tan (Carbonized Human Hair) and Zhenzhu Mo (Pearl Powder) to promote rising and development.

She Pi Dou (Snake Skin Pox): No empty space on head/face/body; lesions are flat and white. They will inevitably dry up without forming pus, leading to death on days 11-12.

Yao Huan Dou (Medicine Affliction Pox): Initially red and moist, but suddenly collapse and fail to rise by days 4-5, approaching interior deficiency. Named "Yao Huan Dou." Treatment should urgently focus on supporting both exterior and interior.

Jiu Jiao Dou (Nine Scorched Pox): Black, depressed lesions appearing singly on the forehead, chin, cheekbones, chest/back, behind ears, hands, and feet. Named "Jiu Jiao," this is an untreatable condition.

Fu Yin Dou (Hidden Yin Pox): Lesions do not suppurate but internally discharge pus and blood. Named "Fu Yin," urgent warming of the interior is required.

Shui Jing Dou (Crystal Pox): Lesions resemble reed catkins, indicating dual deficiency of Qi and Blood. Urgent great tonification is needed, but if the skin is excessively thin, tonification is ineffective.

Kong Cang Dou (Empty Granary Pox): Lesions appear plump but contain no pus or blood internally; the interior is dry and withered with no blood fluid. This is a path to certain death.

Ban Bian Dou (Half-Side Pox): Lesions appear on either the left or right side, skewed and scattered. Although diet, voice, and excretions remain normal, survival beyond seven days is unlikely.

Shi Jiu Dou (Stone Mortar Pox): Lesions are concave in the center and convex at the edges, shiny and attractive, but internally solidified with unliquefied pus. Hard as stone to the touch, named for its resemblance to a stone mortar. Certain death.

Zhu Yu Dou (Cornel Pox): Lesions do not rise much, are concave inside, and wrinkled at the edges, resembling cornel fruits. If the base is red and vibrant, modify Nei Tuo San (Internal Support Powder) for administration.

Chong Dou (Insect Pox): Summer pox often generates insects. Heat excess rots flesh, and rot generates insects. Some cases outside summer show itchy sores. Some argue toxin remains in the skin, rots externally, and resolves externally, avoiding internal complications, thus auspicious. Others compare it to grass rotting into fireflies or wood rotting into insects, thus ominous. Assess form and symptoms to determine fortune/misfortune. Remove insects with a silver needle or lay willow branches underneath to attract maggots out.

Xue Dou (Blood Pox - Type 1): Initial lesions are red-purple, flat like vermilion ink dots on the body. Internal roots rot, external tips wither. Certain death within six days.

Huo Li Dou (Fire Inside Pox): From spotting to scabbing, fever persists while swelling and suppuration proceed according to schedule. Body cools only after scabs fall off. Hence named "Huo Li Dou."

Xue Dou (Blood Pox - Type 2): Blood mixes with heat toxin to form pus. If punctured blood is black, it is untreatable. If red, urgently administer Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction) with Baishao (Paeoniae Radix Alba) to control blood and detoxify.

Xue Ye Dou (Blood Scab Pox): Sparse lesions that swell to pea-size by days 4-5, form bloody scabs by days 6-7 (cinnabar-colored), and shed scabs by day 9. Toxin is light, Qi and Blood are sufficient, scabbing occurs immediately without full pus formation. Considered optimal.

Xuan Pan Dou (Hanging Rock Pox): Lesions cluster at the Yu Zhen (Jade Pillow) point. Purple-red or gray-depressed lesions here are critically dangerous as this is the Naohu (Brain Door) acupoint where meridians converge.

E Kou Dou (Goose Mouth Pox): During the rising phase, lip and mouth lesions mature yellow and carry pus first. This originates in the Spleen and progresses to vomiting and death.

Bai Jiang Dou (White Pus Pox): Lesions carry white pus from the onset. This is Epidemic Pox. Fatal within seven days.

Po Huang Dou (Broken Yellow Pox): A single, unusually large lesion on the Ren Zhong (Philtrum). Despite sparsity, during transmission on days 6-7 or 12, persistent diarrhea occurs due to Spleen corruption.

Si Wei Dou (Four-Sided Pox): At the initial rising stage, small clustered lesions or millet-like bumps emerge around the main lesion's base. Death occurs before pus maturation due to intense itching.

Lou Chuang Dou (Leaking Sore Pox): At pus maturity, a hole appears in the lesion head, leaking pus that dries into scabs resembling Tian Pao Chuang (Blister Sore). This is a favorable type of leaking sore. However, spontaneous rupture leaking clear fluid that turns black without pus maturation is caused by epidemic qi contagion and is untreatable.

Zhu Dou (Borer Pox): Small holes appear on lesions during the swelling phase, neither black nor white. This indicates exterior deficiency and loose pores, leaking Yuan Qi. Urgently administer Bao Yuan Tang (Protect Yuan Decoction) with Dingxiang (Caryophylli Flos) and Rougui (Cinnamomi Cortex). Once holes close, lesions will rise. If only a few lesions have black holes, they are Ding Sores.

Pan Jian Dou (Shoulder-Climbing Pox): Dense lesions on the shoulder tops. Ominous if color is poor.

Yin Xue Ban (Hidden Blood Spots): Lesions resemble shells, gray-white, lacking blood color. Bleeding occurs only upon abrasion, with no pus. Untreatable.

Chi Ping Chuang (Red Duckweed Sore): Lesions resemble floating red duckweed, slightly raised. If scratched and bleeding, urgently rescue with detoxifying and ascending herbs. If they become rotten sores, it is harmless.

Qi Xue Liang Bai Dou (Dual Deficiency of Qi and Blood Pox): Close-up resembles Polygonum hydropiper flowers; distant view looks like prunella purple. Upon pus formation, only clear water appears, no pus. Ancient Sage Shennong had no treatment method.

Zi Ping Chuang (Purple Duckweed Sore): All lesions erupted, purple, non-rising, non-suppurating, adhering to flesh like purple duckweed. Untreatable.

Bai Ping Chuang (White Duckweed Sore): All lesions erupted, white, non-rising, non-suppurating, adhering to flesh like white duckweed. Untreatable.

Yan Wo Dou (Swallow's Nest Pox): Dense lesions between the posterior neck at Fengmen and Dazhui acupoints. Critical if scorched purple.

Chui Zhu Dou (Hanging Pearl Pox): Ears belong to the Kidney, the seat of hidden toxin, unsuitable for pox. Lesions on earlobes, continuous like beads, indicate severe Kidney toxin injury. Zero chance of survival.

Cao Wei Zhu (Grass Tail Pearl): Lesions are depressed everywhere except a plump, pearl-like mass on the thigh bone. Urgent treatment with tonifying and supporting formulas for pus formation may yield survival.

Hei Dou (Black Pox): Mostly depression due to Blood Heat, exacerbated by Tian Gui (Menstruation/Menstrual Power) usurping authority. Originally a malignant sign, but shapes vary. Some with vibrant blood are salvageable; some with distinct colors predict nobility. Must differentiate carefully. Those with non-vibrant blood are untreatable.

Zeng Dou (Gift Pox): "Zeng" means additional emergence. Occurs during pus formation and scabbing. After head/face lesions break, new layers of pus-forming lesions appear on previously clear areas. Also called Bu Kong Dou (Fill-Empty Pox). This indicates Upright Qi is supported and evil toxin is driven outward. Thus, they grow and suppurate easily. If they do not appear after taking tonifying/supportive herbs, or if broken gift lesions do not swell and suppurate again, it is untreatable.

Mu Dou (Wood Pox): Center is slightly depressed, hard like dry rot, with no pus or blood. Caused by phlegm accumulation in children's skin forming toxin. Certain death by day nine.

Wen Dou (Plague Pox): Verse: Mouth silent, eyes unopened, unaware of food/drink, lethargic all day. Severe cases resemble bull madness, limbs convulsing together. Morning sees good flowers, afternoon flowers fall completely. Then comes the top scholar, nailed at the gate threshold. This is Plague Pox; recognition enables cure.

Fo Ding Zhu (Buddha Crown Pearl): Sparse lesions on Tian Ting (Forehead) with moist, beautiful color/form. Most auspicious.
