Hernia (Shan) is caused by the accumulation of Yin-cold Qi internally, compounded by external cold pathogenic invasion. Therefore, in *Su Wen* (Plain Questions), hernias are all attributed to cold; however, they cannot be solely discussed as cold syndromes. Although arising from cold stagnation, it is also due to excessive drinking and overeating, leading to internal steaming and damp-heat, with phlegm-fluids flowing into the Jueyin Liver Meridian. The nature of Wood (Liver) is rapid and urgent; constrained by cold, severe pain ensues. Although symptoms manifest in the Kidney region, the root lies in the Liver, as the Jueyin Liver Meridian connects to the external genitalia. Thus, internal damp-heat stagnates in the middle, while external cold binds the exterior. Additionally, phlegm-fluids, food retention, and static blood may stagnate and cause disease. Hence, it is said that hernia primarily involves the Liver Meridian and is not entirely unrelated to the Kidney. Manifestations include testicular traction pain in the lower abdomen; sometimes formless and soundless, sometimes shaped like a melon, sometimes sounding like a frog; throbbing and painful without fixed location, unbearable.
There are seven types of hernia syndromes: Cold Hernia, Water Hernia, Fox Hernia, Blood Hernia, Qi Hernia, Sinew Hernia. (From *Feng Shi Jin Nang*)
Cold Hernia: Cold invades the Jueyin Meridian, causing sinew spasm and testicular retraction. Symptoms include a cold, hard scrotum like stone, impotence, or testicular traction pain. This results from sitting long on damp ground, allowing cold to stagnate externally.
Water Hernia: Scrotal swelling and pain, transparent like crystal, sweating from the yin area, itching with yellow discharge, water sound upon pressing the lower abdomen. This results from drunkenness and sexual excess, sweating followed by wind exposure, allowing damp-heat to flow into the scrotum when deficiency exists, causing distension and constipation of urine and stool.
Damp Hernia (implied): Scrotal enlargement like a bucket, neither itchy nor painful, caused by dampness pathogen condensing Qi in the yin core; burning pain may also occur.
Blood Hernia: Shaped like a cucumber, located on both sides of the lower abdomen at the ends of the transverse bone, commonly called "piss abscess" (Bian Yong). Caused by repeated pathogenic invasion during hot labor in spring/summer, causing Qi and Blood to overflow and seep into the bladder and scrotum, forming an abscess.
Qi Hernia: Connects upward to the Kidney area and downward to the scrotum. Crying or anger causes Qi stagnation and urgency, leading to distending pain. This is often a congenital condition due to paternal essence deficiency and forced intercourse during impotence.
Fox Hernia: Shape like a tile; enters the abdomen when lying down, exits into the scrotum when walking, resembling a fox urinating when out of the den by day and not at night. The area surrounding the external genitalia up to the lower abdomen is governed by the Liver Meridian, the site of hernia. All hernias involve either Liver Wood pathogen invasion or self-disease of Liver Wood.
Sinew Hernia: Penile swelling, ulceration with pus and pain, tenesmus and sinew contraction, urethral pain, extreme pain causing itching, or non-retractile yin with white fluid resembling semen discharging with urine. Caused by sexual excess or sorcery.
Wood Kidney (Mu Shen): Normally, Heart Fire descends, warming Kidney Water; True Yang descends, harmonizing Kidney Qi. Hardness/swelling occurs when desire injures internally, causing Kidney deficiency and disconnecting Yin/Yang and Water/Fire, resulting in deep cold, distension, hardness, and pain.
The sinews connecting the testes are not solely governed by Jueyin; Taiyin Spleen and Yangming Stomach sinews also connect. Left testicle issues often present more pain than swelling; right testicle issues more swelling than pain.
Another presentation: Fever, sudden mumps behind the ear (Zha Sai) with red swelling and pain. As the cheek swelling subsides, the testicles suddenly swell, one large, one small, appearing as hernia but distinct. The ear side belongs to Shaoyang Gallbladder Meridian, interior-exterior related to Liver; Shaoyang Wind-Heat affects the Liver Meridian.
In women, swelling and pain near the yin in the lower abdomen, or subcutaneous lumps like goose eggs, result from Cold Qi gathering in Jueyin. Lower abdominal cold triggers this, called Yin Hernia. Women with blood depletion amenorrhea, dry throat/legs, strangury, lower abdominal masses, prolapse of anterior yin, and posterior hemorrhoids suffer from female hernias. However, these are named Zha (masses) rather than Shan.
Ancient formula: Equal parts Aconite (Wutou) and Gardenia (Zhizi) decocted with salt wine, named Zhifu Tang, acts quickly. Modifications based on this formula are effective; treatment must distinguish damp-heat ratios. Zhu Danxi stated: Aconite treats external cold constraint; Gardenia treats internal heat stagnation, clarifying internal heat/external cold dynamics. Both herbs descend rapidly; Gardenia guided by Aconite moves fast through the stomach. If pressure relieves pain, it indicates deficiency-cold; add Cinnamon Bark (Rougui) taken as ginger-pill. For rapid pain relief, use processed seeds of Bitter Orange (Zhihe), Hawthorn (Shanzhahé), Lychee (Lizhihe), Sichuan Chinaberry (Chuanlianzi), and Evodia (Wuzhuyu) powdered and taken with warm water.
Pediatric unilateral scrotal swelling often stems from undigested food retention, Spleen dampness descending into the Liver sector.
Penile retraction with shiny, swollen, painless scrotum indicates Liver Qi deficiency failing to unblock; treat with warming tonification.
Chen Shigong stated: Water Hernia presents with shiny scrotal skin, no heat/redness, intermittent swelling/pain, internal water accumulation. Prick to drain water via discreet areas. If chronic with pus formation and pain, prick immediately; internally administer Shiquan Dabu Decoction plus Cornus (Shanyurou), Moutan (Danpi), Alisma (Zexie). (From *Waike Zhengzong*)
Lin Mu'e asked: Why is Qi termed Hernia? Answer: Accumulated earth forms mountains; accumulated Qi forms hernia. Qi accumulates month after month, gathering不散 (not dispersing) to form hernia, like earth piling up. Qi is formless; due to deficiency, it condenses and causes pain where it gathers. Thus, all five Zang organs can have hernias, representing Yuan Qi losing its warming/nourishing nature and transport function. Deficiency leads to stagnation; cold leads to coagulation. From nothing to something, subtle to obvious, blocking until obstruction and pain.
Further query: If formed by accumulating formless Qi causing pain, why is there no pain sometimes? If dispersed, how does it form hernia? If not dispersed, where does it go? Is it temporary dispersion or latent Qi? Answer: If Qi could disperse, it wouldn't form hernia. It becomes painful/hard only because it accumulates without dispersing. Severity depends on Yuan Qi deficiency depth and disease duration. Pain patterns depend on individual Qi/Blood deficiency/excess and Zang organ involvement.
Query: Hernia blamed on Cold-Damp. Cold belongs to Taiyang Bladder (Water); Damp to Taiyin Spleen (Earth). Why attribute to Jueyin Liver? If Liver, shouldn't it be Damp-Heat, unrelated to Cold-Damp? Answer: Five-Zang Hernia originates from *Nei Jing*; cannot limit to Liver. Cold-Damp stagnating long transforms into Heat. Five Elements/Six Qi transformations vary; students must expand understanding.
Query: Men have testicular size variations and Small Intestine/Bladder/Kunlun Qi hernias. If Five Zang have hernia, do women too? How to diagnose/treat? Answer: Male/female share unity; all Zang have hernia potential. Though external kidneys differ, Shaoyin/Jueyin meridians exist. Treatment follows Five Zang principles; gender distinction in pathology is minimal, though social norms obscure this.
Query: If all Zang have Qi accumulation diseases, why specifically call this "Hernia"? Answer: Accumulation不散 (not dispersing) is Ji (mass); accumulated high/protruding shape is Shan (hernia). Five Acculations stem from meridian Qi affected by Seven Emotions/Six Depressions. This condition involves not just meridian deficiency but also external contact and transmission between deficiency/excess. Seven Hernia names appear in *YiTong*, *Zhunshe*, *Zhangzhi*. Below details Five Zang Hernias for learners.
Lung Hernia: Deep Wind-Cold entering Lung Shu points, cold drinks reaching Lung orifices, inherent Yuan Qi deficiency compounded by Cold. Initially stagnant, then accumulates. Three Lung conditions: 1. Lung Bi (closure), breathing difficulty, throat obstruction—not Hernia. 2. Xi Ben (breath surging), Lung Qi full unable to descend, chest obstruction, speech difficulty—not Hernia. 3. Lung Hernia: Rough nasal breathing, shortness of breath, blockage/pain between Xige, Yunmen, Zhongfu—this is Hernia. Pulse at Qikou stringy-tight-forceful indicates treat as Hernia.
Heart Hernia: Long-term Heart Qi stagnation accumulates into Hernia; Righteous Qi declines, accumulation grows. Minor triggers cause cutting/sharp pain, cold limbs, profuse sweating. Left Cun/Guan pulse sunken-stringy-tight indicates Deficiency-Cold Hernia. Stringy-choppy-rapid pulse with vexation/irritability indicates Yin Deficiency/Internal Heat with Qi Stagnation transforming into Fire.
Spleen Hernia: Turbid Qi rises from below, belching, distension/pain, vomiting clear/bitter fluid/phlegm. Guan pulses stringy-tight indicate Excess; sunken-slippery indicate Deficiency.
Liver Hernia: Red/swollen/hard painful scrotum, pain radiating to hips/lower abdomen due to Jueyin connection to yin. Right Guan/Cun pulses stringy-tight-rapid with chills/fever indicate Damp-Heat unable to vent, transmitting to Scrotal Abscess. In women: lower abdominal pain, hip sinew drooping, waist/hip pain, Liver Qi sinking causing vaginal prolapse/pain, Damp-Heat sinking causing vulvar swelling/itching/worms/foul odor—all Liver Qi accumulation akin to male hernia. Left Guan/Cun pulses sunken-stringy-rapid or stringy-tight-rapid.
Kidney Hernia: Common in those with sexual excess; internal deficiency allows Cold-Damp invasion, accumulating into Hernia. Pain from kidney down to penis, strangury/cloudy urine. Pulse micro-choppy or sunken-thin-soft.
Treatment: Lung Hernia – Pungent-dispersing, Bitter-smoothing. Heart Hernia – Cold: Warm-dispersing; Heat: Tonify + Clear. Spleen Hernia – Warm-dispersing; if severe Damp-Heat, resolve Damp first. Liver Hernia – Raise Clear, drain Turbid, separate Damp-Heat. Kidney Hernia – Warm-tonify with draining aids. Reference ancient Seven Hernias for comprehensive treatment.
Query: *Danxi Zuanyao* claims Hernia is Liver-only, unrelated to Kidney, citing "Liver has no tonification method," using bitter-pungent breaking herbs. You use Ginseng/Atractylodes/Angelica/Poria and create Kidney Hernia category, contradicting Danxi? Answer: Liver/Kidney Hernia based on *Nei Jing* and clinical observation. Jueyin Liver Meridian loops around yin, connects to lower abdomen/flanks, ties to testicular roots; Liver Hernia features testicular swelling/drooping/lower abdominal pain. Kidney opens to anterior yin, meridians to loins; Kidney Hernia pain starts at penis, ascends to loins/kidneys. Liver is Child of Kidney; Child deficiency makes Mother weak; Mother strength makes Child solid. Since many hernias stem from Kidney deficiency, treating Liver must consider Kidney, and vice versa.
My treatment of Liver Hernia Excess uses dispersing/warming to expel pathogen. Chronic Liver Hernia with Righteous Qi deficiency (pulse not deeply tight, or empty/weak, blue face/cold limbs) requires Ginseng/Atractylodes tonification plus Cinnamon/Aconite warming. This embodies "Tonify Mother, Child becomes solid." Secular adherence to "Liver no tonification" leads to overusing attacking herbs regardless of Liver/Kidney or Deficiency/Excess, damaging Liver which steals Mother Qi, depleting Kidney. This causes Liver disease to trigger Kidney Cold rushing to Heart/Stomurgency. This is "Child making Mother weak." Zhang Zhongjing used Dang Gui Ginger Lamb Soup for Cold Hernia abdominal/flank pain—proof of Liver tonification. Wang Kentang/Miao Zhongchun used Eight Flavor Rehmannia Pill for Hernia Qi attacking heart/abdomen—proof of Kidney tonification. For constipation in hernia, Xu Shumi/Luo Qianfu used Genkwa (Yuanhua) to purge bowels. For strangury, Xu Shumi used Wuling San for Song Xunfu. If *Zuanyao* were correct (no Kidney link), Wuling San and Yuanhua would be invalid. Most formulas use bitter-pungent Qi-breaking, mixed hot/cold, failing to tonify Liver/Kidney Yin or open orifices to drain pathogens, merely wasting Qi. An old cure for chronic hernia involved rinsing mouth with salt water daily and swallowing; cured within half-year, preventing recurrence. Teeth are bone surplus; saliva/garlic forms from concentrated essence; using this to tonify Kidney is appropriate, another proof of Kidney tonification.
*Yi Lun Xuan Yao* states: Hernia is testicular/lower abdominal acute pain. Seven Hernias: 1. Jue Hernia: Reversal, heart pain, cold feet, vomiting. 2. Zheng Hernia: Abdominal distension, epigastric pain, mass like arm. 3. Han Hernia: Cold food accumulation, hypochondriac/abdominal pain. 4. Pan Hernia: Abdominal pain, coiled around umbilicus. 5. Qi Hernia: Fluctuating abdominal distension/pain. 6. Fu Hernia: Mass below umbilicus. 7. Yelang Hernia: Lower abdomen/yin牵引 pain, difficult defecation. Other terms: Xin Hernia, Ji Hernia, Shan Pi, Small Intestine Qi, Bladder Qi, Ben Tun Qi, Heng Xian, Shu Xian, Unilateral Droop, Mu Shen, Kidney Remainder, Yin Swelling—all types of Hernia. Originates from Seven Emotions/Four Qi disturbances. Ren Mai disease: Men internal Seven Hernias; Women leukorrhea/masses. He Jian: Cold accumulation. Danxi: Damp-Heat. Wu Ze: Cold/Heat. He Zi: Jueyin Liver exclusive. *Nei Jing*: All Zang have Hernia; Taiyang Small Intestine/Bladder and Jueyin Liver are primary, interconnected, with external kidneys tied to testes. Liver governs sinews, gathering at yin; thus Hernia roots in Liver Meridian. Treatment: Guiding/Unblocking primary, or Warming/Dispersing according to meridian syndrome.
Ancients used Wuling San with Qi-moving herbs: Polyporus/Alisma regulate Yin/Yang, harmonize Heart/Small Intestine; Atractylodes drains damp/stasis in waist/umbilicus, boosts Middle Qi to aid transport; Poria drains Bladder water; Cinnamon attacks Liver pathogen, warms/unblocks; Fennel treats Small Intestine Qi; Golden Bell Fruit (Jinlingzi)/Tangerine Seed remove Bladder stagnation; Areca Nut directs Qi downward; slight Akebia (Mutong) guides Small Intestine fire. Formula is stable. Hernia stems from internal deficiency/external attack; must first unblock Qi ("Unblocked means no pain"). Sudden tonification attacking heart/abdomen creates critical conditions.
Large scrotum with retracted penis, dribbling urine, gait obstruction indicates Bladder Qi. Kidney/Bladder interior-exterior; Kidney fails to store, pathogen resides, urine leaks into scrotum. Treat with draining/promoting. Penile retraction with shiny swollen painless scrotum indicates Liver Qi deficiency; treat with warming tonification.
Hui people health practices: Protect external kidneys from cold; dislike southern summer linen pants fearing cool injury. Sleep holding genitals warm, as this is life's root requiring protection. (From *Gu Bu Gu Lu*)
