The *Xinfa* states: Shegan (tongue ulceration) is caused by toxic fire of the Heart and Spleen. This condition is extremely severe. Initially, it resembles a bean, then grows into a mushroom-like shape, with a large head and small stalk, also known as "tongue fungus." It causes pain, redness, and ulceration without skin. Symptoms are lighter in the morning and heavier in the evening. Urgently apply Beitanning points to the lesion, which will naturally shrink and heal. If treatment is delayed, swelling protrudes like a floating lotus; or it may resemble a rooster's comb, with the tongue root shortened and unable to extend, hindering diet and speech, and causing frequent flow of foul saliva. Furthermore, due to anger rushing upward, sudden rupture occurs with unceasing bleeding. Over time, this spreads to the neck and jaw, forming hard, painful nodules resembling tubercles. The skin color remains normal, but there is a soft spot at the apex that is dark red and numb. After rupture, foul fluid flows intermittently, and the tissue rots like rotten cotton. Although ruptured, the hardness, swelling, and pain remain unchanged; this is called "cotton-like ulceration" (Miankui). In extreme cases, it penetrates through the tongue and cheek, causing leakage of fluids, hence also named "scrofulous wind" (Luofeng). Generally, the tongue body belongs to the Heart, and the tongue edges belong to the Spleen. Mental agitation generates fire, while overthinking injures the Spleen and causes Qi stagnation. Severe stagnation leads to this disease. Its external appearance closely resembles throat wind (Houfeng), but in throat wind, the throat and pharynx are constantly swollen, preventing swallowing of liquids; in this condition, the throat and pharynx are not swollen, allowing swallowing of liquids, and the stomach desires food. Because the tongue cannot transport food effectively, repeated attempts to eat result in insufficient intake, leading to an empty stomach, exacerbating deficiency syndromes, gradual decline, and submandibular nodules. At the onset, use Daochi Tang added with Huanglian; externally, apply Jindiluo ground with vinegar. For deficiency patterns, use Guipi Tang. For loose stools, use Guishao Yicheng Tang. Although many treatments have existed since ancient times, this condition is almost invariably fatal (one in a hundred survives). Even with medication, it merely prolongs life temporarily.
