Emaciated individuals (those with thin constitutions) tend to have excessive fire and blood deficiency; blood deficiency constitutes yin deficiency, which leads to hyperactive fire, and hyperactive fire causes fever. Due to emaciated sinews and bones that cannot withstand cold, combined with loose skin and weak flesh, if sores and ulcers occur, they are mostly *ju* (deep-seated abscesses). The term "ju" implies obstruction. It belongs to the yin category, caused by nutritive blood being affected by pathogens, leading to stagnation and failure to circulate smoothly. This is essentially yin stagnation obstructing yang, and blood stagnation obstructing qi, resulting in *ju*. Another view states: *ju* is the toxicity of the five zang organs, while *yong* (superficial abscesses) is the toxicity of the six fu organs. Therefore, *ju* often attaches to the bones, with unchanged skin color, making it difficult to suppurate; even after suppuration, it is difficult to heal. Treatment should preferably involve modifying Bazhen Tang (Eight Treasures Decoction) by adding Honeysuckle Flower (Jinyinhua), Forsythia Fruit (Lianqiao), Aconite Root (Fuzi), etc.

Infants and young children have unfulfilled qi and blood, undeveloped sinews and bones, and fragile spleen and stomach functions. Any occurrence of *yong* or *ju* is mostly due to fetal toxin, or improper care by the mother during pregnancy, causing qi and blood stagnation, which easily leads to sores and swellings. Treatment should only employ mild formulas such as Neituo Neishu Tang (Internal Support and Internal Dispersal Decoction); harsh and potent agents must be avoided to prevent damaging stomach qi. Additionally, innocent glandular phlegm-toxin (Wugu Gandu) in children differs from adult treatment methods; it can be cured simply with tonifying and eliminating phlegm-toxin formulas. Gu Shicheng stated: Infants' skin and flesh are delicate; Baijiang Dan (White Reduction Pill) should not be used lightly, as it causes unbearable pain and severe swelling that is frightening. He also said: For pediatric sores and toxins, mercury (Shuiyin), calomel (Qingfen), and sulfur (Liuhuang) must never be used recklessly, as they astringe toxic vapors and often lead to death.

One asked: After a child's smallpox has fully erupted and the fetal toxin has transformed, why do sores, *ju*, carbuncles, and *ding* (hard boils) reappear? The answer is: Although the fetal toxin has transformed, qi and blood are already depleted. This results from neglect in care, causing yin and yang to become obstructed in the meridians, and nutritive qi failing to follow its proper course, reversing within the muscle tissue, thereby generating *yong*, swellings, *ding*, and toxins, and even producing abnormal manifestations, let alone ordinary sores and ulcers. Therefore, careful nursing is essential after smallpox. Gu Shicheng stated: I often see children developing *yong* or *ju* more than a month after smallpox. Everyone attributes this to insufficient pus from the smallpox and residual toxicity harming the body; however, they fail to realize that this is caused by unrestored qi and blood and dysfunctional spleen and stomach after smallpox. If one recklessly uses detoxifying and cooling agents for internal administration or external application, few will avoid critical danger. One must cultivate and tonify qi and blood, support the spleen, and employ internal support methods. Mild lesions will resolve spontaneously, and even if they suppurate, they will heal easily.
