Wang Kentang stated: Carbuncle and furuncle on the back (Yong Ju Fa Bei) are all generated by the failure of Ying Qi (Nutritive Qi) to flow smoothly, reversing within the muscle tissue. After formation, swelling and pain are severe. Within five to seven days, either one head opens from above, or two heads expand outward, with invasion and expansion continuing up and down without cessation. The Yellow Emperor asked: What qi causes this? Qibo replied: Inability to maintain proper regimen, disturbance by the Seven Emotions (Joy, Anger, Worry, Thought, Sorrow, Fear, Fright), changes due to sexual exhaustion, collision by foul qi, and attack by malignant qi, all cause the wound to open and fail to heal. One should add herbs such as Bai Lian (Bletilla Rhizome) and Bai Ji (Bletilla Striata Tuber) to internal large-tonifying and supporting formulas, and apply external astringent medicines for dressing and washing; thus, the flesh will gather and not continue to ulcerate. (From *Zhun Sheng*)
Chen Shigong stated: For large sores like the "Duikou" carbuncle, when internal and external necrotic flesh has been completely removed, but during the scabbing phase, the internal new flesh fails to adhere, place seven or eight layers of soft silk gauze over the affected area, tie it tightly with silk strips, and press the affected area firmly several times. The internal and external flesh will naturally adhere into one piece, growing like normally formed skin. If there are still incomplete areas at the wound opening, apply Yuhong Gao (Jade Red Ointment); the flesh will naturally level out. (From *Zheng Zong*)
Chen Shigong stated: For diseases such as back carbuncles and Duikou sores, when they have ulcerated and are discharging pus, in both winter and summer, it is suitable to place four or five layers of silk gauze outside the applied plaster. Then, use a square cotton cloth measuring eight inches on each side, secure the four corners with fine snake-skin straps, cover the gauze, and tie the straps across the anterior chest to bind the wound firmly. This prevents the loss of warm qi within the wound, facilitating putrefaction and suppuration. When washing the wound, prepare two sets of bandages for replacement, ensuring that the interior of the affected area remains warm. This method is most excellent. (From *Zheng Zong*)
Gu Shicheng stated: Whenever a back carbuncle ulcerates, resulting in a small opening but a large internal cavity, after the discharge of abundant pus, if the internal new flesh does not unite, one should use a lead sheet as smooth as a mirror, drill an eye-sized hole in the center, and pierce holes around the edges. Thread needle through sewn cotton cloth and lay it over the area; in summer, use two layers of cloth. Attach six wide silk straps to the six sides. First, cover with plaster, add fresh cotton, and place the lead sheet over the wound. Tie the left and right straps tightly across the chest. Then, tie the top-left strap diagonally from the left shoulder to the right hypochondrium, and the bottom-right strap similarly; tie the top-right strap diagonally from the right shoulder to the left hypochondrium, and the bottom-left strap similarly. Thus, the two layers of new flesh will merge into one block. If the left side has more concave (deficient) areas and the right side has more convex (solid) areas, pad the cotton outside the plaster: make the padding thicker on the left and thinner on the right. If the right side has more concave areas, follow the same principle. Ultimately, while inspecting the wound, draining pus, and cleaning, careful observation of the deficiency and excess conditions will clarify the situation.
