There are five types of scabies: dry scabies, damp scabies, insect scabies, sand scabies, and pustular scabies. If the Lung Metal (Lung organ system) is excessively dry, dry scabies arises, characterized by itching with dry skin and white scales; if the Spleen channel has excessive dampness, damp scabies arises, characterized by swelling and pain, with yellow fluid oozing upon rupture, and in severe cases, black fluid; if the Liver channel has excessive wind, insect scabies arises, with itching penetrating to the bone such that scratching does not relieve it or cause pain; if Heart blood stagnates, sand scabies arises, appearing like fine sand, red, itchy, and painful, with water exuding when scratched; if the Kidney channel has excessive dampness, pustular scabies arises, resembling bean-sized nodules, itching during urination and defecation, with clear and white thin pus; alternatively, this may also arise from excessive dampness in the Spleen channel, distinguished by thick pus at the apex and simultaneous itching and pain. All these types contain small insects and are highly contagious. One must strictly avoid soaking and washing with hot water for temporary relief, as this causes heat-toxin to attack inward, driving the insects deeper. Even with good prescriptions, effects cannot be achieved overnight. Patients must take heed. Also avoid all "fa-wu" (trigger foods) and seafood. Treatment methods: Internally administer Jie Ling Dan (Scabies Spirit Pill) or Xiao Feng San (Wind-Expelling Powder); externally apply Xiu Qiu Wan (Embroidered Ball Pill) or Yi Sao Guang (One-Sweep Light), either is acceptable.
Cuo Fei Chuang (Acne-like miliaria), commonly known as "sitting-board sore," occurs on the thighs, dense like scattered millet grains, pointed like thorns, with extreme itching and pain, causing body hair to stand erect like bristles. In severe cases, the skin breaks and adheres to clothing. This condition results from long-term accumulation of damp-heat and damp-toxin in the Spleen channel. It can also be caused by sitting for long periods in damp, low-lying areas, or sitting on stones exposed to the scorching sun, which generates damp-heat. This area belongs to the "extreme yin" region where blood circulation is scarce, so medicinal efficacy reaches it poorly. External treatment should be used: wash with Kushen Tang (Sophora Root Decoction) and sprinkle Ehuang San (Goose Yellow Powder).
Bai Tu Chuang (White Baldness Scab), commonly referred to as [HT HT] Chuang, occurs due to damp-heat affecting the Foot-Taiyang Bladder Channel and the Du Mai (Governing Vessel), generating insects that cause itching, with high piles of crusts. Wind invasion leads to white scales; severe heat leads to baldness; prolonged duration damages the follicles, preventing hair regrowth. Treatment should focus on dispelling wind, removing dampness, killing parasites, relieving itching, and nourishing blood.
Fei Chuang (Greasy Scab) occurs on the crown of the head, caused by qi rebellion from disharmony of the zang-fu organs rushing upward, and blood-heat toxin pouring into the upper region. Children have insufficient yin energy and excess yang fire, making them most susceptible. Internal administration of Jingjie (Schizonepeta Herb), Fangfeng (Saposhnikovia Root), Lianqiao (Forsythia Fruit), Tianhuafen (Trichosanthes Root), Beimu (Fritillaria Bulb), Yuan Shen (Scrophularia Root), Chi Shao (Red Peony Root), Sheng Di Huang (Rehmannia Root), and Niu Bang Zi (Arctium Seed) is suitable to clear heat, detoxify, cool the blood, and harmonize blood. Only after the toxic factors are slightly relieved should external medications be applied. One must absolutely avoid suddenly applying cold-cold substances to suppress the condition, as this may cause heat-toxin to attack inward without hope of rescue. This is because children's zang-fu organs are delicate, allowing pathogens to enter easily but exit with difficulty.
