Pain in the forehead and eyes resembles *Shanghan* (Cold Damage), yet headaches of the Taiyang (Greater Yang) and Yangming (Yang Brightness) meridians do not progress to the point where the head tilts and is difficult to lift. In this condition, however, the headache is like being split by an axe, with dim vision and dizziness, appearing as if the body cannot support itself. This is entirely due to toxic fire reaching these two meridians, with toxin invading the Yang positions. The method of "drawing wood from under the cauldron" (removing the root cause) should be used to thoroughly descend the fire; once done, the pain will stop immediately, and the rash will naturally emerge. Mistakenly using acrid-aromatic dispersing agents is like adding fuel to a blazing fire, inevitably transforming into a *Men Zheng* (Closed Syndrome).
Bones and the lower back belong to the Kidney Meridian. Pain of this nature indicates that excessive heat has flowed into the Kidney Meridian. Mistakenly using cold-natured drugs to release the exterior would lead to death before the day ends.
Heat conditions are suited for harmonizing rather than drying. Regarding general body burning heat, compared to cases of lethargy and cold limbs, this condition manifests with heat flourishing outward because the Qi and Blood can still overcome the toxin. Once cleared and resolved, the rash emerges naturally; recklessly using exterior-releasing methods will cause the toxin to become internally concealed.
There are moments that seem quiet but suddenly become restless, and moments that seem restless but suddenly become quiet, termed "inconstancy." Unlike mania (*Dian Kuang*), which involves flourishing heat, this condition involves suppressed and depressed heat. It is generally caused by internal disturbance of toxic fire, leading to restlessness in sitting or lying down.
Waking follows Yang and governs the upper part; sleeping follows Yin and governs the lower part. The Stomach is the sea of the Six Fu Organs. When toxic fire obstructs and blocks, preventing the communication between upper and lower, insomnia results.
In the early stage, the whole body feels like ice, the complexion looks obscured like dirt, the mouth appears frosty white, the headache is like splitting, there is a desire for hot drinks but aversion to cold, and the six pulses are deep and fine. This is "extreme Yang resembling Yin," indicating hidden toxin. Heavy clearing of internal heat is required to allow the toxic heat to emerge externally. Suddenly, the body becomes very hot, the pulse turns flooding and rapid, accompanied by irritability, delirious speech, and extreme thirst for ice water. Although the symptoms are fierce and malignant, they are relatively easy to treat. If met by incompetent physicians who recklessly administer Guizhi Tang (Cinnamon Twig Decoction) and Fuzi (Prepared Aconite Root), the patient may die as if poisoned before finishing one dose.
The four limbs belong to the Spleen. Cold reversal in the limbs, when seen in miscellaneous diseases, indicates deficiency-cold of the Spleen Meridian and impending collapse of Yuan Yang (Original Yang). Epidemic diseases are different: the whole body is extremely hot, while only the limbs are cold. This is due to fierce toxin obstructing the Spleen Meridian, preventing the evil fire from penetrating. Heavily clearing Spleen heat will naturally warm the hands and feet.
Sinews belong to the Liver and rely on Blood for nourishment. When heat-toxin flows into the Liver Meridian, the rash toxin cannot find an exit through the orifices. The sinews and vessels are impacted by it, resulting in twitching and palpitations akin to fright.
