In miscellaneous diseases, there are cases of essence and fluid depletion where fluids fail to ascend, causing dryness in the throat and a desire to drink, yet the patient cannot consume more than half a cup. In contrast, patients with epidemic macules (Yizhen) crave ice water, finding even a hundred cups insufficient. This is because toxic fire roasts within; only ice water can rescue the dryness, and only Shigao (Gypsum Fibrosum) can suppress the flame. Quack doctors forbid raw and cold foods, and patients' families revere this as a divine method, not even daring to offer warm water, resulting in cracked lips and a black tongue.
The stomach qi is the root of all diseases across the four seasons. While inability to eat seems difficult to treat, this does not apply to cases of stomach heat. When evil fire invades the stomach and hot toxins rush upward, frequent dry heaving or immediate vomiting after eating may occur. Once the stomach qi is cleared, food will be accepted naturally without force-feeding.
The chest is the region of the upper jiao, housing the heart and lungs, where pathogens rarely invade directly. However, when fire flares upward, it easily affects the heart, creating a situation where fire fuels fire; heat transferred to the lungs scorches the Metal element, intensifying restlessness. The patient must sigh frequently, feeling chest fullness, distension, and Qi stagnation.
The voice originates from the heart but is expressed through the lungs. If orifices are closed due to Qi blockage, and Qi is stagnant due to toxicity, the heart becomes confused and spirit unclear, leading to closed orifices and an inability to produce sound.
The Stomach belongs to damp Earth, located in the middle jiao, serving as the sea of water and grain; the Qi of the five zang organs, six fu organs, and twelve meridians all derive from here. Pathogens generally do not interfere unless the body is weak, allowing pathogens to settle and cause disease. Whether biased towards cold or heat, or involving water retention and food accumulation, these factors struggle with true Qi causing pain; this describes the source of ordinary illnesses. However, abdominal pain caused by epidemic macules, occurring on either side or radiating to the small intestine, results from conflicting toxic fire with no outlet for release. Treating this according to meridians for ordinary abdominal pain will lead to death. In the initial stage, using Baidu San (Poison-Resolving Powder) or Liangge San (Cooling the Diaphragm Powder) added with Huanglian (Coptidis Rhizoma) will stop the pain immediately.
In Shanghan (Cold Damage), excessive sweating leads to Yang collapse, but this is not the case here. Sweat is the fluid of the Heart, transformed from blood. Blood is generated by the Heart, stored in the Liver, and controlled by the Spleen. When blood is roasted by toxicity, tendons lose nourishment, causing muscle twitching.
Patients claim cold air emanates from the stomach, but this is not true cold. It is ascending Qi originating from the Liver, carrying Ministerial Fire, rising from below upwards, generating intense heat. This phenomenon represents "fire transforming into water" at its extreme, a sign of extreme heat with hyperactive Yang and deficient Yin, thus creating the sensation of cold air.
