[Symptoms of Headache] The pain may occur on one side or on both sides; it may last for two or three days, or seven or eight days, and in severe cases, even longer than several days; after the pain subsides, the patient returns to a normal state, but upon any subsequent trigger, the pain immediately returns. If Qi is deficient and spirit is depleted, pain occurs immediately upon exertion, radiating to the temporal region (fish tail), this is headache due to Qi deficiency. If there is vexatious heat in the five centers (palms, soles, and chest), accompanied by frequent pulling stabbing pain, this is headache due to Blood deficiency. If there is thirst, dry and cracked lips, and red, scanty, and painful urination and defecation, this is headache due to accumulated heat. If there is nausea and vomiting, this is headache due to phlegm-fluid retention. If triggered by anger, with pain radiating to the hypochondrium, this is headache due to Liver Fire attacking upward. All of the above are symptoms of internal injury headache.
[Etiology of Headache] It may be caused by deficiency-cold of Yuan Qi, occurring immediately upon exertion; or by insufficiency of the Blood aspect, with Yin Fire attacking upward; or by accumulated heat that cannot vent outward; or by retained phlegm and fluid; or by food stagnation in the Middle Jiao; or by emotional disturbance such as anger, leading to depression of Liver and Gallbladder Fire. All these factors can ascend and cause headache, resulting in the syndrome of internal injury headache.
[Pulse Diagnosis of Headache] A pulse that is empty, large, and lacking spirit indicates Qi deficiency. A fine and choppy pulse indicates Blood deficiency. A surging and rapid pulse indicates accumulated heat from rich and greasy foods. A slippery and large pulse indicates internal accumulation of phlegm-fluid. Surging and large pulses at both Cun positions indicate Fire in the Upper Jiao. A wiry and rapid pulse at the left Guan position indicates depression/stagnation in the Liver and Gallbladder.
