Qin Zi said: If a person has fever, it indicates illness; if the fever persists, it leads to a critical situation. There is no one who has fever without being ill, nor is there anyone whose fever does not cease and yet survives. Generally, if one is ill but does not have fever, the illness is not severe; however, if one is ill and has fever, even if the condition seems mild, it must not be ignored. Therefore, regarding fever syndromes, I have detailed them in *Shanghan Dabai* (Great Clarity on Cold Damage), and again in the *Dafang Zazheng Men* (Section on Miscellaneous Diseases of Large Formulas). Now, I establish two categories for external contraction and two for internal injury. Although there are many other pathways...
(Cold-Wind, Warmth-Heat)
[Symptoms of Cold-Wind Fever] Headache, body pain, absence of sweating, aversion to cold, fever accompanied by stiffness/tightness of the body. This is fever caused by exterior pathogens. Excessive sweating on hands and feet, thirst, aversion to...
[Causes of Cold-Wind Fever] Either the season is appropriately cold, or sudden unseasonal cold occurs, or one is exposed to cold in the morning or evening. Due to weak constitution and lack of caution, one contracts a cold and develops fever.
[Pulse Signs of Cold-Wind Fever] The pulse appears floating and tight, indicating cold pathogen injuring the Ying (Nutrient) level; the pulse appears floating and moderate/relaxed, indicating wind pathogen injuring the Wei (Defensive) level. Left pulse floating and rapid indicates disease in the Taiyang (Greater Yang) channel; left pulse wiry and rapid indicates disease in the Shaoyang (Lesser Yang) channel. Right pulse floating and rapid indicates disease in the Yangming (Yang Brightness) channel. Six pulses sinking and rapid indicate interior accumulated heat.
[Treatment Methods for Cold-Wind Fever] When cold pathogen injures Ying, with no sweat and aversion to cold, use Qianghuo Tang (Notopterygium Decoction). When wind pathogen injures Wei, with sweat and aversion to wind, use Fangfeng Tang (Saposhnikovia Decoction). Wiry and rapid pulse, disease in Shaoyang, use Chaihu Tang (Bupleurum Decoction); if it is Shanghan (Cold Damage), add Qianghuo (Notopterygii Rhizoma et Radix) and Duhuo (Angelicae Pubescentis Radix); if it is Shangfeng (Wind Injury), add Fangfeng (Saposhnikoviae Radix). Right pulse flooding and rapid, disease in Yangming, use Gegen Tang (Puerariae Decoction), add Qianghuo for cold pathogen. Sinking pulse indicates interior heat, suitable for clearing interior heat.

[Symptoms of Warmth-Heat Fever] Body heat, excessive sweating, no aversion to cold, but rather aversion to heat, thirst, dry lips, lethargic spirit, irritability and restlessness. This is warm...
[Causes of Warmth-Heat Fever] Either the season is excessively hot, with heat pathogen prevailing; or when it should be cold it is instead warm, and when it should be cool it is instead hot. The warmth-heat pathogen invades the muscle surface, transmits into the collaterals/channels, thus causing warmth-heat fever symptoms.
[Pulse Signs of Warmth-Heat Fever] Pulses are mostly flooding and rapid. Floating and rapid indicates exterior heat; sinking and rapid indicates interior heat; if left pulse is rapid, [disease is in] Taiyang, Shaoyang; if right pulse is...
[Treatment Methods for Warmth-Heat Fever] Heat in Taiyang, use Qianghuo Chonghe Tang (Notopterygium Harmonizing Decoction). Heat in Shaoyang, use Xiao Chaihu Tang (Minor Bupleurum Decoction). Heat in Yangming, use Gan Ge Shigao Tang (Pueraria and Gypsum Decoction); sinking and rapid indicates interior heat, if stool is constipated, use Liangge San (Cooling Diaphragm Powder); if downward purgation symptoms are urgent, use Sanhuang Wan (Three Yellows Pill) to purge.
